A tool set for implanting a rod in a human spine in conjunction with bone screws includes a pair of end guide tools that receive opposite ends of the rod in channels and under manipulation by a surgeon facilitate transport of the rod toward the bone screws attached to the guide tools. Each end guide tool includes a laterally extending, rigid, rod holding structure located near a bottom surface thereof. Intermediate guide tools having pass-through slots are utilized to guide the rod to the bone screws at intermediate locations. For bone screw implantation, the end guide tools and intermediate guide tools are assembled with a driver. The driver is coaxial with the respective guide tool and rotatingly mateable thereto. The driver includes a stem and a nut-type fastener, the stem receivable in the guide tool. The fastener allows for rotation of the bone screw without rotating the attached guide tool. For reducing a rod into the bone screw, a rod pusher is deployed that is coaxial with a respective guide tool and includes an outer sleeve that abuts the rod as the sleeve is translated along the guide tool and toward the bone screw. A method utilizing the tool set allows a surgeon to percutaneously implant the bone screws and the rod in the patient.

A spinal rod implantation tool assembly comprising: a) an elongate guide tool having: i) an upper end;ii) a lower end configured for releaseable attachment to a spinal implant bone screw head with a first rod-receiving channel in the screw head;iii) a driving tool-receiving central bore with a central axis; andiv) an elongate second rod-receiving channel extending along a length of the guide tool and opening radially outward relative to the central axis, a substantial portion of the second rod-receiving channel including a width greater than a rod received therein; whereinv) when said lower end and said bone screw head are in engagement, said second rod-receiving channel is adapted to align substantially parallel with the screw head first rod-receiving channel so as to guide a rod received in the second rod-receiving channel along at least a portion thereof and toward the screw head first rod-receiving channel; andb) a driver having a stem with a spinal implant bone screw engagement end structure, the stem being removably receivable in the guide tool central bore during installation of a bone screw, and the engagement end structure being adapted for operable engagement with the bone screw to rotate and drive the bone screw into bone and thereafter be removed from the guide tool, so as to make the guide tool second rod-receiving channel ready to receive the rod therein. 2. A spinal rod implantation tool assembly comprising: a) an elongate guide tool having an upper end and a lower end configured for releaseable attachment to a spinal implant bone screw and having a central axis; said tool including an elongate channel opening radially outward relative to the central axis and extending the length of the tool; said channel being adapted to receive an implant rod within the channel such that the implant extends generally radially relative to the central axis and so as to guide the rod along at least a portion of the channel;b) a driver having a fastener, a stem and a spinal implant engagement structure, the fastener being releaseably attachable to the guide tool, the stem being removably receivable in the guide tool channel during installation of a bone screw, and the spinal implant engagement structure configured for operable engagement with the bone screw to rotate and drive the bone screw into bone and thereafter be removed from the guide tool, so as to make the guide tool channel ready to receive the rod;c) an elongate rod pusher having a sleeve and a rod pushing end, the sleeve receivable over the guide tool and rotatably attachable to the upper end of the guide tool, with rotation of the sleeve about the guide tool selectively translating the rod pushing end toward and away from the spinal implant; and whereind) the sleeve has an inner guide and advancement structure and the guide tool upper end has an outer guide and advancement structure mateable with the inner guide and advancement structure. 3. The assembly of claim 2 wherein the fastener is a nut coaxial and freely rotatable with respect to the stem, the nut having an inner guide and advancement structure, the guide tool upper end having an outer guide and advancement structure mateable with the inner guide and advancement structure. 4. The assembly of claim 2 further comprising: a) an elongate torquing tool receivable in the elongate guide tool, the torquing tool having an end configured for engagement with an implant fastener; andb) an elongate anti-torque tool receivable about the elongate guide tool, the anti-torque tool having an end configured for engagement with a rod. 5. The assembly of claim 2 wherein the guide tool is an intermediate guide tool having first and second legs defining a longitudinal pass-through slot. 6. A spinal rod implantation tool assembly comprising: a) an elongate guide tool having an upper end and a lower end configured for releaseable attachment to a spinal implant; the guide tool having a central axis and an elongate channel extending from the lower end thereof; said channel opening radially outward relative to said axis and being adapted to receive an implant rod therein such that the implant rod extends generally radially outwardly from the channel and such that said channel guides said rod toward said spinal implant;b) an elongate rod pusher having a sleeve and a rod pushing end, the sleeve being rotatably sleeved over the guide tool and removably threadedly attachable to the guide tool so as to allow rotation of the pusher relative to the guide tool and movement of the pusher along the central axis of the guide tool, the pushing end of the rod pusher being aligned so as to be adapted to engage the rod within the guide tool channel so that movement due to the rotation of the pusher toward the implant urges the rod into the implant, and reversal of rotation translates the rod pushing end away from the spinal implant;c) a driver having a fastener, a stem and a spinal implant engagement structure, the stem being freely rotatable about an elongate axis with respect to the fastener, the fastener attachable to the upper end of the elongate guide tool, the stem receivable in the guide tool and the spinal implant engagement structure configured for operable engagement with the spinal implant to rotate and drive the implant into bone; and whereind) the fastener is a nut coaxial and freely rotatable with respect to the stem, the nut having an inner guide and advancement structure, the guide tool upper end having an outer guide and advancement structure mateable with the inner guide and advancement structure. 7. The assembly of claim 6 wherein the sleeve has an inner guide and advancement structure and the guide tool upper end has an outer guide and advancement structure mateable with the inner guide and advancement structure. 8. The assembly of claim 6 further comprising: a) an elongate torquing tool receivable in the elongate guide tool, the torquing tool having an end configured for engagement with an implant fastener; andb) an elongate anti-torque tool receivable about the elongate guide tool, the anti-torque tool having an end configured for engagement with a rod. 9. The assembly of claim 6 wherein the guide tool is an intermediate guide tool having first and second legs defining a longitudinal pass-through slot. 10. A spinal rod implantation tool assembly comprising: a) an elongate guide tool having an upper end and a lower end configured for releaseable attachment to a spinal implant bone screw and having a central axis; said tool including an elongate channel opening radially outward relative to the central axis and extending the length of the tool; said channel being adapted to receive an implant rod within the channel such that the implant extends generally radially relative to the central axis and so as to guide the rod along at least a portion of the channel;b) a driver having a fastener, a stem and a spinal implant engagement structure, the fastener being releaseably attachable to the guide tool, the stem being removably receivable in the guide tool channel during installation of a bone screw, and the spinal implant engagement structure configured for operable engagement with the bone screw to rotate and drive the bone screw into bone and thereafter be removed from the guide tool, so as to make the guide tool channel ready to receive the rod; and whereinc) the guide tool is an end guide tool having an elongate body, an outer surface and a channel with a lateral opening extending through the outer surface and along a longitudinal axis an entire length of the guide tool, the end guide tool having a rigid holding structure disposed opposite the lateral opening near the lower end of the body and projecting laterally outwardly from the elongate body outer surface, the holding structure sized and shaped to closely receive and abut against an end of a rod. 11. A spinal rod implantation tool assembly comprising: a) an elongate guide tool having an upper end and a lower end configured for releaseable attachment to a spinal implant; the guide tool having a central axis and an elongate channel extending from the lower end thereof; said channel opening radially outward relative to said axis and being adapted to receive an implant rod therein such that the implant rod extends generally radially outwardly from the channel and such that said channel guides said rod toward said spinal implant; andb) an elongate rod pusher having a sleeve and a rod pushing end, the sleeve being rotatably sleeved over the guide tool and removably threadedly attachable to the guide tool so as to allow rotation of the pusher relative to the guide tool and movement of the pusher along the central axis of the guide tool, the pushing end of the rod pusher being aligned so as to be adapted to engage the rod within the guide tool channel so that movement due to the rotation of the pusher toward the implant urges the rod into the implant, and reversal of rotation translates the rod pushing end away from the spinal implant; and whereinc) the guide tool is an end guide tool having an elongate body, an outer surface and a channel with a lateral opening extending through the outer surface and along a longitudinal axis an entire length of the guide tool, the end guide tool having a rigid holding structure disposed opposite the lateral opening near the lower end of the body and projecting laterally outwardly from the elongate body outer surface, the holding structure sized and shaped to closely receive and abut against an end of a rod.
