An endoscopic surgical instrument includes a flexible tube, a grasping and fastening end effector coupled to the distal end of the tube, and a manual actuator coupled to the proximal end of the tube. The manual actuator is coupled to the end effector by a plurality of flexible cables which extend through the tube. The tube contains a lumen for receiving a manipulable endoscope and the end effector includes a passage for the distal end of the endoscope. The end effector has a store for a plurality of male fastener parts, a store for a plurality of female fastener parts, a rotatable grasper, a rotatable fastener head for aligning a female fastener part and a male fastener part with tissues therebetween, and a firing member for pressing a male fastener part through tissues grasped by the grasper and into a female fastener part. According to a presently preferred embodiment, the overall diameters of the flexible tube and the end effector (when rotated to the open position) do not exceed approximately 20 mm so that the instrument may be delivered transorally to the fundus of the stomach. The manual actuator is provided with a lock-out feature which prevents firing male fastener parts before the fastener head is closed. The instrument is advantageously utilized in a fundoplication procedure.

A method of fastening two tissue portions using a surgical instrument comprising an elongated tube and a distal member coupled proximate a distal end of the tube, wherein the distal member includes a movable member having a connected end and a free end, the movable member moving between a first position in which the free end is located distally of the connected end and a second position different than the first position, the method comprising: inserting the surgical instrument into a patient transorally while the movable member is in the first position;bringing a first tissue portion of the patient towards a second tissue portion to create a fold of tissue; andfastening the first tissue portion to the second tissue portion with a fastener. 2. The method of claim 1, further including engaging a region of tissue including the first tissue portion and the second tissue portion with a tissue engaging member coupled to one of the elongated tube and the distal member. 3. The method of claim 2, wherein the distal member further includes a stationary member, and the movable member has a first surface facing a second surface of the stationary member in the second position, the tissue engaging member being disposed between the first and second surfaces in the second position. 4. The method of claim 1, wherein the fastener is a two-part fastener including a male part and a female part. 5. The method of claim 4, wherein the distal member includes a store of male parts and a store of female parts. 6. The method of claim 5, wherein fastening further comprises firing the male part through the first tissue portion and the second tissue portion and into the female part. 7. The method of claim 6, wherein one of the first and second tissue portions is a portion of a stomach. 8. A method of fastening two tissue portions using a surgical instrument comprising an elongated tube, a distal member coupled proximate to and extending distally of a distal end of the tube, wherein the distal member includes a stationary member, a selectively actuatable movable member coupled to the stationary member, and a selectively actuatable grasper coupled to the distal member, wherein the movable member includes a connected end and a free end, the movable member moving between a first position in which the free end is located distally of the connected end and a second position different than the first position, wherein each of the movable member and the grasper is configured to engage tissue, and wherein the movable member is independently actuatable of the grasper, the method comprising: inserting the surgical instrument into a patient through a natural orifice while the movable member is in the first position;grasping a region of tissue having a first tissue portion and a second tissue portion of the patient;bringing the first tissue portion toward the second tissue portion to create a fold of tissue; andplacing a first part of a two-part fastener through the first tissue portion in order to fasten the first tissue portion to the second tissue portion. 9. The method of claim 8, wherein a first part of the two-part fastener is a male part and a second part of the two-part fastener is a female part. 10. The method of claim 9, wherein placing further comprises firing the male part through the first tissue portion and the second tissue portion and into the female part. 11. The method of claim 10, wherein one of the stationary member and the movable member includes a store of male parts and the other of the stationary member and the movable member includes a store of female parts. 12. The method of claim 11, wherein one of the first and second tissue portions is a portion of a stomach. 13. The method of claim 8, wherein in the second position, the connected end of the movable member is located distally of the free end of the movable member. 14. The method of claim 13, wherein the movable member has a first surface facing a second surface of the stationary member in the second position, the grasper being disposed between the first and second surfaces in the second position.
