Disclosed are systems and methods of preparing dialysate for use in a home dialysis system that is compact and light-weight relative to existing systems and consumes relatively low amounts of energy. The method includes coupling a household water stream to a dialysis system; filtering the water stream; heating the water stream to at least about 138 degrees Celsius in a non-batch process to produce a heated water stream; maintaining the heated water stream at or above at least about 138 degrees Celsius for at least about two seconds; cooling the heated water stream to produce a cooled water stream; ultrafiltering the cooled water stream; and mixing dialysate components into the cooled water stream in a non-batch process.

A method as in claim 16, wherein at least one of the fluid pathways is a microchannel. 18. A method as in claim 16, wherein at least one of the fluid pathways comprises multiple pin-shaped members around which the fluid flows. 19. A method as in claim 16, wherein the fluid is water. 20. A method as in claim 16, wherein the fluid is used in a dialysis procedure. 21. A method as in claim 16, fluid flows through the inlet portion in a counter-flow relationship with fluid flowing through the outlet portion. 22. A method as in claim 16, wherein heat is transferred from fluid in the outlet portion to fluid in the inlet portion across a transfer layer that separates the outlet portion from the inlet portion. 23. A method as in claim 16, further comprising: passing the fluid through a pump prior to providing the fluid to the microfluidic heat exchanger; andpassing the fluid through a throttling valve after cooling the fluid, wherein the pump and the throttling valve collectively maintain the fluid at a flow rate and pressure that inhibits the fluid from undergoing a phase change.
