The bag can be contained within the piercing member such that the bag is introduced to the vial when the vial adaptor is coupled with the vial. In some embodiments, the bag expands within the vial as liquid is removed from the vial via the adaptor, thereby regulating pressure within the vial. A vial adaptor capable of removing medicinal fluid from a vial, the adaptor comprising: a base having a cap connector configured to couple the adaptor with the vial;a medical connector interface configured to couple the adaptor with a medical device;a piercing member having a sheath extending from the base of the vial adaptor, the piercing member configured to be inserted into the vial when the adaptor is coupled with the vial; andan extractor comprising a generally rounded aperture and a groove disposed along a sidewall of the sheath, wherein groove has an elongate dimension extending distally from the aperture, wherein the extractor is configured to provide a fluid passageway to an extractor channel extending between the extractor aperture and the medical connector interface, even if the extractor aperture is partially obstructed by a septum of the vial when the adaptor is coupled with the vial, wherein the aperture has a width that is greater than a width of the groove, and wherein the extractor is configured to permit medicinal fluid to be removed from the vial through the extractor while leaving no more than a minimal amount of medicinal fluid in the vial when the vial adaptor is coupled to the vial. 2. The adaptor of claim 1, wherein a housing portion comprises the cap connector, the medical connector interface, and the sheath. 3. The adaptor of claim 2, wherein the housing portion further comprises a regulator aperture configured to permit ambient air to pass therethrough as medicinal fluid is withdrawn from the vial, thereby equalizing the pressure within the vial when the adaptor is coupled with the vial. 4. The adaptor of claim 1, wherein the medical device is capable of withdrawing medicinal fluid from the vial, such that the entire volume of medicinal fluid is transferred from the vial to the medical device when the adaptor is coupled with the vial. 5. The adaptor of claim 1, wherein the aperture of the extractor is positioned near the septum when the adaptor is coupled with the vial, such that the adaptor is capable of removing medicinal fluid from the vial while leaving no more than a minimal amount of medicinal fluid in the vial. 6. The adaptor of claim 1, wherein a length of the extractor is between about 0.15 inches and about 0.30 inches when the adaptor is coupled with the vial. 7. The adaptor of claim 1, wherein a width of the extractor is between about 0.01 inches and about 0.03 inches when the adaptor is coupled with the vial. 8. The adaptor of claim 1, wherein a depth of the extractor is between about 0.02 inches and about 0.04 inches when the adaptor is coupled with the vial. 9. The adaptor of claim 1, wherein an edge of the extractor is angled sufficiently sharply to prevent the septum from completely conforming to the shape of the extractor aperture when the adaptor is coupled with the vial. 10. The adaptor of claim 1, wherein the depth of the extractor is sufficiently large to prevent the septum from completely conforming to the shape of the extractor aperture when the adaptor is coupled with the vial. 11. The adaptor of claim 1, wherein a depth dimension of the groove follows a chord offset from a central axis of the piercing member. 12. A vial adaptor capable of removing medicinal fluid from a vial, the adaptor comprising: a cap connector configured to couple the adaptor with the vial;a sheath configured to be inserted into the vial when the adaptor is coupled with the vial;a medical connector interface configured to couple the adaptor with a medical device; andan extractor comprising an aperture and a groove disposed along a sidewall of the sheath, wherein the aperture and the groove are positioned and configured such that the adaptor is capable of removing medicinal fluid from two or more different vials while leaving no more than a minimal amount of medicinal fluid in the vials when the two or more different vials vary by size, by cap dimensions, or by size and cap dimensions, thereby affecting the position of the extractor aperture with respect to the septum when the adaptor is coupled with the vial, and wherein a depth dimension of the groove follows a chord offset from a central axis of the sheath. 13. The adaptor of claim 12, wherein the extractor comprises a groove having an elongate dimension extending distally from the aperture, such that the extractor is configured to maintain a fluid passageway to an extractor channel extending between the extractor and the medical connector interface, even if the extractor is partially obstructed a septum of the vial. 14. The adaptor of claim 12, wherein the extractor extends along a length of the sheath. 15. The adaptor of claim 12, wherein the extractor extends at an angle with respect to the length of the sheath. 16. The adaptor of claim 12, wherein the extractor is straight. 17. The adaptor of claim 12, wherein the extractor is curved. 18. The adaptor of claim 12, wherein the extractor is configured such that at least a portion of the extractor extends past a lower surface of the septum when the adaptor is coupled with the vial. 19. The adaptor of claim 12, wherein the extractor extends into the sheath at an angle, thereby increasing a depth of the extractor. 20. The adaptor of claim 12, wherein the extractor is configured such that a fluid passageway remains between the septum and a volume of the extractor that is not filled in by the septum when the adaptor is coupled with the vial. 21. The adaptor of claim 12, wherein the extractor is positioned within a region covering half of a distance from a base of the sheath to an end of the sheath. 22. The adaptor of claim 12, wherein the extractor is positioned within a region covering one-quarter of the distance from the base of the sheath to the end of the sheath. 23. The adaptor of claim 12, wherein the extractor is positioned within a region covering one-eighth of the distance from the base of the sheath to the end of the sheath. 24. A method of withdrawing medicinal fluid from a vial, the method comprising: providing a vial adaptor for coupling with the vial, the adaptor comprising: a cap connector configured to couple the adaptor with the vial,a medical connector interface configured to couple the adaptor with a medical device,a piercing member having a sheath extending from a base of the piercing member, the piercing member configured to be inserted into the vial when the adaptor is coupled with the vial, andan extractor comprising a generally rounded aperture and a groove disposed along a sidewall of the sheath, the extractor configured to permit medicinal fluid to pass therethrough, wherein the aperture has a width that is greater than a width of the groove;coupling the adaptor to the vial, such that the extractor is in fluid communication with an interior of the vial; andwithdrawing medicinal fluid from the vial while leaving no more than a minimal amount of medicinal fluid in the vial. 25. The method of claim 24, wherein the groove extends distally from the aperture, and wherein the groove is configured to provide a fluid passageway to the aperture even if the aperture is partially or completely obstructed by a septum of the vial when the adaptor is coupled with the vial. 26. The method of claim 24, wherein withdrawing medicinal fluid comprises withdrawing a maximum amount of medicinal fluid. 27. The method of claim 24, wherein a housing portion comprises the cap connector, the medical connector interface, and the sheath. 28. The method of claim 27, wherein the housing portion further comprises a regulator aperture configured to permit ambient air to pass therethrough as medicinal fluid is withdrawn from the vial, thereby equalizing the pressure within the vial when the adaptor is coupled with the vial. 29. The method of claim 24, wherein the medical device is capable of withdrawing medicinal fluid from the vial, such that the entire volume of medicinal fluid is transferred from the vial to the medical device when the adaptor is coupled with the vial. 30. The method of claim 24, wherein the aperture is positioned near the septum when the adaptor is coupled with the vial, such that the adaptor is capable of removing medicinal fluid from the vial while leaving no more than a minimal amount of medicinal fluid in the vial. 31. The method of claim 24, wherein the extractor is positioned and configured such that the adaptor is capable of removing medicinal fluid from two or more different vials while leaving no more than a minimal amount of medicinal fluid in the vials when the two or more different vials vary by size, by cap dimensions, or by size and cap dimensions, thereby affecting the position of the aperture with respect to the septum when the adaptor is coupled with the vial.
