One aspect is a programmable device having machine executable instructions for performing a method to assist with managing a patient's health. In various embodiments, at least one previously-defined event is detected based on a number of acquired health-related parameters. The at least one detected event is recorded with an associated time when the at least one detected significant event occurred. An action is triggered based on the at least one detected event. The at least one detected event is displayed with at least one trended health-related parameter in a single display area. Other aspects and embodiments are provided herein.

A device for monitoring a patient's health condition, comprising: a parameter acquisition module to acquire two or more health-related parameters; anda predetermined event detection module to communicate with the parameter acquisition module to determine whether the two or more health-related parameters correspond to at least one predetermined event, wherein in determining that the two or more health-related parameters corresponds to at least one of the number of the predetermined events, the predetermined event detection module is configured to determine a correlation coefficient for two or more trended health-related parameters to identify the predetermined event if the correlation coefficient reflects a significant correlation between the two or more trended health-related parameters. 2. The device of claim 1, wherein the parameter acquisition module is configured to acquire two or parameters selected from the group of parameters consisting of: activity, heart sounds, respiration, pulmonary edema and posture, and wherein the at least one predetermined event includes heart failure decompensation. 3. The device of claim 1, wherein the WMD includes a programmer for an implantable medical device. 4. The device of claim 1, wherein the WMD includes a portable phone. 5. The device of claim 1, wherein the WMD includes a computer. 6. The device of claim 1, wherein the device is an implantable medical device (IMD). 7. The device of claim 1, wherein the at least one health-related parameter includes at least one health-related parameter from an implantable medical device (IMD) and at least one health-related parameter from an external data source. 8. The device of claim 1, further comprising an action trigger module to communicate with the predetermined event detection module to trigger a desired action based on a detected predetermined event. 9. A wellness monitoring device for monitoring a patient's health condition, comprising: a communication module to receive two or more health-related parameters;a parameter acquisition module to communicate with the communication module to acquire the two or more health-related parameters;an input module to receive manual input data;a predetermined event definition module to communicate with the input module to define a number of predetermined events for the patient's health condition using the manual input data;a timer module; anda predetermined event detection module to communicate with the parameter acquisition module and the predetermined event definition module to determine that the two or more health-related parameters correspond to at least one of the number of predetermined events, and to communicate with the timer module to associate a time with the at least one of the number of predetermined events, wherein in determining that the two or more health-related parameters corresponds to at least one of the number of the predetermined events, the predetermined event detection module is configured to determine a correlation coefficient for two or more trended health-related parameters to identify the predetermined event if the correlation coefficient reflects a significant correlation between the two or more trended health-related parameters. 10. The device of claim 9, wherein the parameter acquisition module is configured to acquire at least one parameter selected from the group of parameters consisting of: activity, heart sounds, respiration, pulmonary edema and posture, and wherein the at least one predetermined event includes heart failure decompensation. 11. The device of claim 9, wherein the parameter acquisition module is configured to acquire two or parameters selected from the group of parameters consisting of: activity, heart sounds, respiration, pulmonary edema and posture, and wherein the at least one predetermined event includes heart failure decompensation. 12. The device of claim 9, further comprising a predetermined event set module to communicate with the predetermined event definition module to define a set of predetermined events, wherein the input module is adapted to communicate with the predetermined event definition module to modify the set of predetermined events. 13. The device of claim 9, further comprising an action trigger module to communicate with the predetermined event detection module to trigger a desired action based on a detected predetermined event. 14. The device of claim 13, further comprising a display area, wherein the action trigger module is adapted to provide a signal to display the detected predetermined event along with a trend for the at least one health-related parameter on the display area. 15. The device of claim 13, wherein the action trigger module is adapted to provide a signal to send an alarm in response to the at least one detected predetermined event. 16. The device of claim 13, wherein the action trigger module is adapted to provide a signal to change device therapy in response to the at least one detected predetermined event. 17. The device of claim 9, wherein the manual input data further includes at least one manually-inputted health-related parameter, the parameter acquisition module to communicate with the input module to acquire the at least one manually-inputted health-related parameter. 18. The device of claim 17, wherein the parameter acquisition module is adapted to communicate with the communication module and the input module is adapted to acquire at least one of an environmental parameter, a diet parameter, a mental/emotional parameter, a medication compliance parameter and a symptom parameter. 19. The device of claim 9, wherein the communication module to receive at least one health-related parameter from an implantable medical device (IMD). 20. The device of claim 9, wherein the communication module to receive at least one health-related parameter from an external data source, including at least one health-related parameter from an external sensor, or at least one health-related parameter from patient history database.
