A tissue fixation system is provided for dynamic and rigid fixation of tissue. A fastener connected with an elongate fastening member, such as a cable, wire, suture, rod, or tube, is moved through a passage between opposite sides of tissue. A medical device is used to secure the fastener to the elongate fastening member. The medical device includes a tensioning mechanism for tensioning the elongate fastening member. As crimping mechanism is used to secure the fastener to the elongated member, where a cutting mechanism cut the excess portion of the elongated member.

1. A medical device for securing a fastener to an elongate member, comprising: a housing;an elongate introducer including proximal and distal ends and defining a longitudinal passage along a longitudinal axis;a tensioning mechanism disposed on or adjacent the housing and configured to tension the elongate member; anda fastening mechanism at least partially disposed in the distal end of the elongate introducer, wherein at least a portion of the fastening mechanism is configured to be slidable relative to the longitudinal axis of the elongate introducer;wherein the fastening mechanism is configured to lock the fastener and the elongate member together as the fastening mechanism slides while resisting rotation relative to the elongate introducer and wherein the tensioning mechanism is configured to tension the elongate member independently of the locking by the fastening mechanism. 2. A medical device as set for the in claim 1, wherein the fastening mechanism comprises: a collet positioned in the distal end of the elongate introducer and defining a collet passage longitudinally aligned with the central longitudinal axis, wherein an end portion of the collet is configured to receive the fastener. 3. A medical device as set forth in claim 2, wherein the collet includes first and second collet arms each having a force application end portions combining to form a fastener aperture, the fastener aperture being configured to receive the fastener therein. 4. A medical device as set forth in claim 3, wherein the fastening mechanism comprises a collet closer affixed to the distal end of the elongate introducer, disposed about the collet, the elongate introducer and the collet closer being moveable from a first position to a second position such that the collet closer compresses the force application end portions of the first and second collet arms about the fastener aperture. 5. A medical device as set forth in claim 4, further comprising a trigger operably connected to the elongate introducer, the trigger positionable from a first trigger position to a second trigger position with respect to the housing, such that the collet closer is moved from the first position to the second position. 6. A medical device as set forth in claim 5, further comprising a trigger bias member interposed between the trigger and the housing, wherein the trigger bias member biases the trigger into the first trigger position. 7. A medical device as set forth in claim 6, further comprising a cutting mechanism disposed in the elongate introducer, proximal to the collet, and operably connect to the trigger, such that movement of the trigger from the second trigger position to a third trigger position actuates the cutting mechanism. 8. A medical device as set forth in claim 5, wherein the tensioning mechanism further includes a torque controller. 9. A medical device as set forth in claim 1, the tensioning mechanism comprising a locking assembly and a rotation assembly. 10. A medical device for securing a fastener to an elongate member, comprising: a base;a tensioning mechanism connected to the base and configured to tension the elongate member about a tensioning axis; anda tubular member including a first end proximate the base and a second end extending distally from the base, the tubular member defining a longitudinal passage along a longitudinal axis;a crimping member including an aperture sized to receive the fastener and configured to crimp the fastener when the crimping member is moved along the longitudinal axis, wherein the crimping member is configured to resist rotation of the tubular member and crimp the fastener independently of the tensioning by the tensioning mechanism. 11. The medical device of claim 10, wherein the crimping member includes a tapered surface engageable by the tubular member. 12. The medical device of claim 10, further including cutting cams operative to cut the elongate member after the fastener has been crimped. 13. The medical device of claim 12, wherein at least a portion of the cutting cams pivot within the tubular member. 14. The medical device of claim 12, further including a trigger pivotable in connection with the base and having a first engaged position wherein the fastener is crimped. 15. The medical device of claim 14, wherein the trigger has a second engaged position wherein the elongated member is cut. 16. The medical device of claim 10, wherein the crimping member includes a bisected collet having an aperture through which the elongate member may extend. 17. The medical device of claim 10, wherein the tubular member is moveable in a first direction to crimp a fastener, and in a second direction to release the fastener, and further including a biasing element operative to move the tubular member in the second direction. 18. The device of claim 17, wherein the tubular member includes a ferrule affixed to the first end, the biasing element engaged to the base at one end, and the ferrule at an opposite end. 19. The device of claim 18, further including a ferrule engagement member disposed between the trigger and the ferrule. 20. The device of claim 10, wherein the tensioning mechanism includes inclined surfaces cooperative with roller bearings to maintain a tension of the elongate member as the tubular member is moved to engage the crimping member.
