초록

A reciprocating surgical file system for precisely removing bone and/or other tissue is described. The system allows a user to maneuver the system and navigate into hard-to-access sites. The tool may be used to perform a spinal decompression by removing tissue that impinges a spinal nerve.

1. A method of performing a spinal decompression on a patient comprising: inserting a distal tip portion of a powered surgical instrument into a neuroforamen of a vertebra, the powered surgical instrument having a handle assembly comprising a drive output configured to output linearly reciprocating motion parallel to a drive axis, a blade guide mounted to the handle assembly, the blade guide having a proximal portion defining a proximal guide axis and a distal portion corresponding to the distal tip portion defining a distal guide axis, the proximal guide axis being angled relative to the drive axis and the distal guide axis being angled relative to the proximal guide axis, a blade having a distal cutting portion, the blade being mounted in the blade guide and coupled to the drive output, the blade being constrained by the blade guide to bend from the drive axis to the proximal guide axis and to bend from the proximal guide axis to the distal guide axis, the blade guide constraining the distal cutting portion of the blade to reciprocating motion parallel to the distal guide axis;positioning the distal cutting portion of the blade within the neuroforamen adjacent tissue that is impinging on a spinal nerve;actuating the blade so that it linearly reciprocates relative to the impinging tissue to remove the impinging tissue while preserving the nerve. 2. The method of claim 1 further comprising placing a tubular introducer communicating from the surgical site to a position outside of the patient and wherein the step of positioning the blade comprises inserting the blade guide into the tubular introducer. 3. The method of claim 1 wherein the blade guide has a first side and an opposite second side, an opening being formed in the first side for exposing the distal cutting portion and the second side forming a tissue shield, the method further comprising positioning the blade guide with the first side toward impinging tissue and the second side toward the nerve to shield the nerve from the blade. 4. The method of claim 1 wherein the powered surgical instrument further comprises an irrigation pathway operable to deliver fluid from the proximal portion of the blade guide to the distal portion of the blade guide the method further comprising delivering fluid to the blade via the irrigation pathway. 5. The method of claim 1 wherein the powered surgical instrument further comprises a fluid removal pathway operable to deliver fluid from the distal portion of the blade guide to the proximal portion of the blade guide the method further comprising evacuating fluid from the blade via the fluid removal pathway. 6. The method of claim 1 further comprising: positioning the proximal guide axis outside but adjacent to the neuroforamen and the distal guide axis inside the neuroforamen; andactuating the blade to reciprocate the cutting portion to remove impinging tissue around a corner and opposite the spinal nerve. 7. The method of claim 1 wherein the powered surgical instrument includes an optical pathway allowing visualization of the distal tip portion, the method further comprising visualizing the impinging tissue as it is removed by the reciprocating blade. 8. The method of claim 1 wherein the blade guide proximal portion extends from the handle assembly at approximately a right angle. 9. The method of claim 1 wherein the distal guide axis is approximately parallel to the drive axis. 10. The method of claim 1 wherein the powered surgical instrument further comprises an irrigation pathway operable to deliver fluid from the proximal portion of the blade guide to the distal portion of the blade guide and a fluid removal pathway operable to deliver fluid from the distal portion of the blade guide to the proximal portion of the blade guide, the method further comprising delivering fluid to the blade via the irrigation pathway and evacuating fluid from the blade via the fluid removal pathway. 11. A method of performing a spinal decompression on a patient comprising: inserting a distal tip portion of a powered surgical instrument into a neuroforamen of a vertebra, the powered surgical instrument having a handle assembly comprising a drive output configured to output linearly reciprocating motion parallel to a drive axis, a blade guide mounted to the handle assembly, the blade guide having a proximal portion defining a proximal guide axis and a distal portion defining a distal guide axis, the distal guide axis being angled relative to the proximal guide axis, a blade having a distal cutting portion, the blade being coupled to the drive output and extending through the blade guide from the blade guide proximal portion to the blade guide distal portion, the blade being constrained by the blade guide to reciprocate the distal end of the blade parallel to the distal guide axis;positioning the distal cutting portion of the blade within the neuroforamen adjacent tissue that is impinging on a spinal nerve;actuating the blade so that it linearly reciprocates relative to the impinging tissue to remove the impinging tissue;transmitting fluid between the proximal portion of the blade guide and the distal portion of the blade guide in a first direction while blocking fluid transmission between the proximal portion of the blade guide and the distal portion of the blade guide in a second, opposite direction by way of a check valve.
