The present invention relates generally to a vascular access device having a septum positioned within a lumen of the device, the device further having a pusher for providing a fluid pathway through the septum. In particular, the present invention relates to systems and methods for improving flow of a fluid through the access device, while minimizing stagnation of fluids within the device. These systems and methods include various modified fluid pathways and interactions between the vascular access device body, the septum and the pusher.

1. A vascular access device, comprising: an intravenous catheter adapter having an inner surface, a proximal end, a distal end and a first lumen extending therethrough;a septum positioned within the first lumen, the septum dividing the first lumen into a distal portion and a proximal portion, the septum having an aperture; anda pusher disposed within the proximal portion of the first lumen, the pusher having an outer surface, a distal end, a proximal end, and a second lumen extending therethrough, the outer surface including one or more fenestrations and one or more ribs that extend proximally from the proximal end of the pusher, the one or more ribs maintaining a space between the proximal end of the pusher and a luer connecter when the luer connector is inserted into the first lumen forcing the distal end of the pusher through the aperture of the septum and into the distal portion of the first lumen to thereby allow fluid flowing from the luer connector to pass between the outer surface of the pusher and the inner surface of the intravenous catheter adapter and then pass into the second lumen of the pusher through the one or more fenestrations such that the fluid bypasses the septum into the distal portion of the first lumen via the second lumen of the pusher. 2. The device of claim 1, wherein the aperture of the septum comprises a slit. 3. The device of claim 2, wherein the one or more ribs comprise a plurality of ribs, and wherein each of the one or more fenestrations is positioned between two of the plurality of ribs. 4. The device of claim 1, wherein at least a portion of the one or more fenestrations remains in the proximal portion of the first lumen when the pusher has bypassed the septum. 5. The device of claim 1, wherein the inner surface of the intravenous catheter adapter includes one or more undercuts which interact with the one or more ribs to limit the proximal movement of the pusher within the first lumen. 6. The device of claim 5, wherein the proximal movement of the pusher is limited prior to the pusher bypassing the septum. 7. The device of claim 5, wherein the proximal movement of the pusher is limited subsequent to the pusher bypassing the septum. 8. The device of claim 1, wherein the one or more ribs extend distally from the proximal end of the pusher but terminate prior to reaching the distal end of the pusher. 9. The device of claim 1, wherein the one or more ribs extend distally from the proximal end, the one or more fenestrations extending distally farther than the one or more ribs. 10. The device of claim 1, wherein the one or more ribs comprise four ribs. 11. The device of claim 10, wherein the four ribs are spaced 90 degrees apart. 12. The device of claim 1, wherein the pusher further comprises an elastomeric stopper formed at the distal end of the pusher. 13. The device of claim 1, wherein the elastomeric stopper includes a hole. 14. A vascular access device, comprising: an intravenous catheter adapter having an inner surface, a proximal end, a distal end and a first lumen extending therethrough;a septum positioned within the first lumen, the septum dividing the first lumen into a distal portion and a proximal portion; anda pusher disposed within the proximal portion of the first lumen, the pusher having an outer surface, a distal end, a proximal end, and a second lumen extending therethrough, the outer surface further including a plurality of ribs that extend proximally from the proximal end of the pusher, the plurality of ribs maintaining a space between the proximal end of the pusher and a luer connecter when the luer connector is inserted into the first lumen to thereby allow fluid flowing from the luer connector to pass between the outer surface of the pusher and the inner surface of the intravenous catheter adapter, the outer surface further including one or more fenestrations that allow the fluid to pass into the second lumen after having passed between the outer surface of the pusher and the inner surface of the intravenous catheter adapter. 15. The device of claim 14, wherein at least a portion of the one or more fenestrations remains in the proximal portion of the first lumen when the pusher has bypassed the septum such that fluid flows from the proximal portion of the first lumen into the second lumen through the one or more fenestrations and out through the pusher into the distal portion of the first lumen. 16. The device of claim 14, wherein the plurality of ribs extend distally from the proximal end of the pusher but terminate prior to reaching the distal end of the pusher. 17. A pusher for use within an intravenous catheter adapter, the pusher comprising: an outer surface;a distal end;a proximal end; anda lumen extending therethrough;wherein the outer surface includes a plurality of ribs that extend proximally from the proximal end, the plurality of ribs maintaining a space between the proximal end of the pusher and a luer connecter when a luer connector is used to force the pusher through a septum of an intravenous catheter adapter to thereby allow fluid flowing from the luer connector to pass between the outer surface of the pusher and an inner surface of the intravenous catheter adapter, the outer surface further including one or more fenestrations that allow the fluid to pass into the lumen after having passed between the outer surface of the pusher and the inner surface of the intravenous catheter adapter. 18. The pusher of claim 17, wherein the length of the one or more fenestrations is configured such that at least a portion of the one or more fenestrations remains positioned proximally to the septum when the pusher has bypassed the septum such that fluid flows from a proximal side of the septum into the second lumen through the one or more fenestrations and out through the pusher on a distal side of the septum.
