Methods and devices are provided for facilitating delivery and implanting of a bone anchor into bone. In one exemplary embodiment, a bone anchor extension is provided for coupling to a bone anchor to facilitate delivery and implanting of the bone anchor in bone. The bone anchor extension can have a generally elongate configuration that allows it to extend from a skin incision in a patient to a site proximate a patient's spine, and it can include a lumen extending therethrough between proximal and distal ends thereof. A distal end of the bone anchor extension can be adapted to engage a bone anchor, such as a bone screw. Various techniques are provided for locking the distal end of the bone anchor extension into engagement with a bone anchor.

The bone anchor extension of claim 1, wherein a proximal end of the locking mechanism is positioned on one side of the outer tube and a distal end of the locking mechanism is positioned on an opposite side of the outer tube when the locking mechanism is in a locked position. 5. The bone anchor extension of claim 1, wherein the locking mechanism comprises: an arm having a proximal end, a distal end, and a central portion;a linkage having a proximal end and a distal end;wherein the central portion of the arm is pivotally coupled to the outer tube at the first pivot point, the distal end of the linkage is pivotally coupled to the inner tube at the second pivot point, and a distal end of the arm is pivotally coupled to a proximal end of the linkage at a third pivot point. 6. The bone anchor extension of claim 1, wherein the inner tube further comprises a relief slit formed thereon and configured to provide relief to the stress applied to the locking mechanism when the locking mechanism is in a locked position. 7. The bone anchor extension of claim 1, wherein the distal end of the outer tube is configured to rotate relative to a bone anchor to engage a bone anchor. 8. The bone anchor extension of claim 1, wherein the distal end of the inner tube comprises a contact surface configured to engage a generally arcuate contact surface of a bone anchor. 9. The bone anchor extension of claim 1, wherein at least one of the inner tube and the outer tube includes an anti-rotation mechanism configured to inhibit rotation of a bone anchor relative to the inner and outer tubes. 10. The bone anchor extension of claim 1, wherein the outer tube is sized to span from at least a skin incision in a patient to a predetermined site proximate a spine of the patient. 11. A bone anchor extension, comprising: an outer tube having proximal and distal ends with a lumen extending therebetween;an inner tube at least partially disposed within the lumen in the outer tube, the inner tube having proximal and distal ends with a lumen extending therebetween; anda locking mechanism pivotally coupled to each of the outer tube and the inner tube and extending continuously across a central axis of the outer tube, the locking mechanism having a proximal portion and a distal portion,wherein the locking mechanism is movable between a first position, in which the proximal portion of the locking mechanism extends longitudinally relative to a longitudinal axis of the outer tube along one side of the outer tube and the distal portion of the locking mechanism extends longitudinally relative to a longitudinal axis of the outer tube along an opposite side of the outer tube, and a second position in which the locking mechanism extends transversely outward relative to a longitudinal axis of the outer tube. 12. The bone anchor extension of claim 11, wherein the locking mechanism comprises: an arm having a proximal end, a distal end, and a central portion;a linkage having a proximal end and a distal end;wherein the central portion of the arm is pivotally coupled to the outer tube, the distal end of the linkage is pivotally coupled to the inner tube, and a distal end of the arm is pivotally coupled to a proximal end of the linkage. 13. The bone anchor extension of claim 11, wherein the proximal portion of the locking mechanism is pivotally coupled to the outer tube and the distal portion of the locking mechanism is pivotally coupled to the inner tube such that pivotal movement of the locking mechanism advances the inner tube distally relative to the outer tube to engage a hone anchor between the distal ends of the outer and inner tubes. 14. The bone anchor extension of claim 11, wherein a location where the proximal portion of the locking mechanism is pivotally coupled to the outer tube is proximal to the proximal end of the inner tube. 15. The bone anchor extension of claim 11, wherein a location where the proximal portion of the locking mechanism is pivotally coupled to the outer tube is approximately centrally disposed along a centerline of the outer tube. 16. The bone anchor extension of claim 11, wherein the inner tube further comprises a relief slit formed thereon and configured to provide relief to the stress applied to the locking mechanism when the locking mechanism is in a locked position. 17. The bone anchor extension of claim 16, wherein the relief slit is a spiral cut slit extending radially around the proximal end of the inner tube. 18. The bone anchor extension of claim 11, wherein the outer tube is sized to span from at least a skin incision in a patient to a predetermined site proximate a spine of the patient. 19.
