An electrical signal processor is electrically coupled to the electrodes for processing the electrical signal. The system receives data regarding the patient's state (e.g. asleep, exercising). Patient state information is stored in a patient state array, thereby enabling the system to track the patient's state over time, and to select an appropriate test for detecting a cardiac event based on both past and present data regarding the patient's state.

A device for detecting acute cardiac ischemia, the device including: a sensor adapted to sense a cardiac signal from a patient's heart, a memory; and, a digital processor configured to: (i) compute a plurality of patient state values from the cardiac signal and store the plurality of patient state values in the memory, wherein at least two of the plurality of patient state values are different from one another;(ii) compute a plurality of values of a heart signal feature from the cardiac signal,(iii) store each of the plurality values of the heart signal feature so that it is associated with a corresponding patient state value;(iv) analyze the plurality of patient state values to determine whether a change in patient state has occurred such that a prior patient state has changed to a current patient state;(v) operative when a change in patient state has been detected, determine both a measure of the time that has elapsed since the change and a measure of the duration of the prior patient state;(vi) based at least in part on whether a change in patient state has occurred, select a test to detect acute cardiac ischemia, wherein the test is based on the plurality of heart signal feature values, and wherein the test is a threshold test characterized by a threshold that is a function of both the measure of the time that has elapsed since the change and the measure of the duration of the prior patient state. 2. The device of claim 1 wherein the processor is further configured to compute an additional heart signal feature value from the cardiac signal, and to apply the test to the additional heart signal feature value. 3. The device of claim 2 wherein the threshold is a function of both the plurality of patient state values and the plurality of values of the heart signal feature. 4. The device of claim 1 wherein the sensor is an electrode. 5. The device of claim 4 wherein the sensor is adapted for implantation into a human body. 6. The device of claim 1 wherein the change in patient state is a change in activity level. 7. The device of claim 1 wherein the measure of the time that has elapsed since the change is based on a number of patient state values after the change that have the value of the current patient state. 8. The device of claim 1 wherein the measure of the duration of the prior patient state is based on a number of patient state values that have the value of the prior patient state. 9. The device of claim 1 wherein the heart signal feature pertains to the ST segment of a heart beat. 10. The device of claim 1 wherein the digital processor is further configured to perform alerting operations according to a set of parameters and to operate so that at least one of the parameters of the alerting operations are modified based upon the current patient state, said parameters including at least one of modality of alerting, intensity of alerting, and whether internal, external or remote station alerting occurs. 11. The device of claim 1 wherein the digital processor is further configured to perform alerting operations according to a set of parameters and to operate so that at least one of the parameters of the alerting operations are modified based upon the change in patient state, said parameters including at least one of modality of alerting, intensity of alerting, and whether internal, external or remote station alerting occurs. 12. The device of claim 1 wherein the test includes a hold duration. 13. The device of claim 1 wherein the test to detect acute cardiac ischemia is based in part on reference data associated with a patient state that is similar to the current patient state. 14. The device of claim 1 wherein the digital processor is further configured to generate reference data and associate the reference data with the current patient state. 15. The device of claim 1 wherein the threshold changes as a function of time from a first value associated with the prior patient state to a second value associated with the current patient state, wherein the change in threshold begins to take effect upon detection of the change in patient state. 16. A device for detecting acute cardiac ischemia, the device including: a sensor adapted to sense a cardiac signal from a patient's heart, a memory; and, a digital processor configured to: (i) compute a plurality of patient state values from the cardiac signal and store the plurality of patient state values in the memory, wherein at least two of the plurality of patient state values are different from one another, and wherein each of the plurality of patient state values corresponds to an activity level;(ii) compute a plurality of values of a heart signal feature from the cardiac signal,(iii) analyze the plurality of patient state values to determine whether a change in patient state has occurred such that a prior patient state has changed to a current patient state;(iv) operative when a change in patient state has been detected, determine both a measure of the time that has elapsed since the change and a measure of the duration of the prior patient state;(v) based at least in part on whether a change in patient state has occurred, select a test to detect acute cardiac ischemia, wherein the test is based on the plurality of heart signal feature values, and wherein the test is a threshold test characterized by a threshold that is a function of both the measure of the time that has elapsed since the change and the measure of the duration of the prior patient state.
