The device includes a guiding engagement element positioned in a vessel wall; an incising member positionable over the engagement element; and a cutting element, with a recessed notch, at a distal end of the incising member. The cutting element forms a flap on a first portion of a vessel wall, the flap having a first portion attached to the wall and second portion having a first diameter cut and a second diameter cut larger than the first diameter cut. The engagement element includes a retaining element engaging a portion of the flap; and an anchoring element engaging an external surface of the vessel, wherein the retaining element and the anchoring element exert a compressive force on the vessel wall for assisting closure of the cut.

The medical device of claim 1, wherein the cutting element is configured to create an incision in the wall of the vessel lumen with a longitudinal axis of the incision inclined relative to the plane of the vessel wall with the flap at a side of the incision. 3. The medical device of claim 2, wherein the cutting element is configured to create an incision having a first portion and a second portion, the first portion having a flap attached to the wall and the second portion opposite the flap, andwherein the engagement element is configured to bisect the incision. 4. The medical device of claim 1, wherein the retaining element is configured to pull the flap closed and the anchoring element is configured to secure the flap onto the wall until the incised wall is sealed. 5. The medical device of claim 1, wherein at least one of the engagement element, the retaining element and the anchoring element comprises a bio-absorbable material. 6. The medical device of claim 1, wherein the engagement element includes a first anchoring element and a second anchoring element,wherein the cutting element is dimensioned to incise into a portion of a wall of the vessel lumen to form a flap on the incision, such that the flap has a segment attached to a portion of the wall; andwherein the first anchoring element is configured to engage an internal surface of the flap and the second anchoring element is configured to engage an external surface of the flap, the first anchoring element and the second anchoring element configured to exert a compressive force on the flap. 7. The medical device of claim 6, wherein the cutting element is configured to form a flap movable from a sealing configuration to an access configuration to create access to an interior of the lumen. 8. The medical device of claim 6, wherein the first anchoring element is configured to engage a portion of the flap for pulling the flap closed, and further wherein the second anchoring element is configured to secure the flap to an outside of the wall. 9. The medical device of claim 1, wherein incising member comprises an elongated shaft that fixably attaches to the cutting element, the elongated shaft having a longitudinal axis that forms an angle relative to cutting element. 10. The medical device of claim 1, wherein the cutting element comprises a scissor mechanism for adjusting the cutting element between the open configuration and the closed configuration. 11. The medical device of claim 1, comprising: a handle member for coupling the sheath member and the incising member such that the incising member is positionable about the sheath member. 12. The medical device of claim 1, wherein the cutting element is configured to create an incision having an arch-shape profile. 13. The medical device of claim 1, wherein the cutting element, for adjusting the cutting element between the open configuration and the closed configuration, comprises a mechanism selected from a group consisting of a sliding mechanism and a push/pull mechanism. 14. The medical device of claim 1, wherein the incising member comprises a restraining part to restrain the cutting element in the open configuration. 15. The medical device of claim 1, wherein the restraining part comprises an outer sheath.
