According to one aspect, various methods and apparatus are used for treating a condition of a patient's heart, and for monitoring cardiac operation. In one approach consistent therewith, an electrode arrangement is placed in a right ventricle of the heart. The electrode arrangement is used to capture the myocardium for re-synchronization of the left and right ventricles by providing first and second signal components having opposite polarity on respective electrodes. The electrode arrangement is connected to an implantable CRM device that has the capability of pacing/sensing atrium, pacing/sensing ventricles, and deliver defibrillation therapy from the right side of the heart. The CRM device captures ventricular contractions to treat conduction abnormalities in one or more of the ventricles.

An implantable medical device for providing cardiac defibrillation and single-chamber pacing therapies, the implantable medical device comprising: an implantable lead assembly including a first electrode and a second electrode, the first electrode comprising a coil electrode configured to provide a defibrillation energy to a subject heart, and the second electrode configured to locally provide a pacing signal to a region at or near a His bundle of the subject heart; anda pulse generator circuit, wherein the pulse generator circuit is configured to provide, to the implantable lead assembly, non-defibrillating electrostimulation pulses including the pacing signal and including at least two pulses of opposite polarity and occurring sufficiently proximate one another in at least one of time or distance to elicit myocardium capture of the subject heart. 2. The implantable medical device of claim 1, wherein the pulse generator circuit is configured to provide the at least two pulses of opposite polarity to the second electrode and a third electrode, respectively, wherein the second and third electrodes are configured to be disposed in the right ventricle of the subject heart. 3. The implantable medical device of claim 1, wherein the first a second electrodes are configured to be disposed in the right ventricle of the subject heart. 4. The implantable medical device of claim 3, comprising a fourth electrode configured to deliver an electrostimulation pacing therapy in a left ventricle of the subject heart. 5. The implantable medical device of claim 1, wherein the implantable lead assembly comprises a first lead from a can of the implantable medical device to the first electrode, and wherein the implantable lead assembly comprises a different second lead from the can of the implantable medical device to the second electrode. 6. The implantable medical device of claim 1, wherein the pulse generator circuit is configured to provide the at least two pulses of opposite polarity sufficiently proximate in time and distance to elicit myocardium capture of the subject heart. 7. The implantable medical device of claim 1, wherein the pulse generator circuit is configured to provide the at least two pulses as first and second pulses that include respective portions that are at least partially overlapping in time. 8. The implantable medical device of claim 7, wherein the pulse generator circuit is configured to provide one or more of the first pulse and the second pulse including a biphasic waveform. 9. The implantable medical device of claim 7, wherein the pulse generator circuit is configured to generate electrostimulation pulses having a pulse polarity that is reversed after N beats. 10. The implantable medical device of claim 1, wherein the pulse generator circuit is configured to generate the defibrillation energy in response to information indicative of a tachyarrhythmia, the information obtained using an electrical signal received using one of the first and second electrodes when the electrodes are disposed in a right ventricle of the subject heart. 11. The implantable medical device of claim 1, wherein the pulse generator circuit is configured to provide the at least two pulses of opposite polarity sufficiently proximate in time and distance to elicit myocardium capture of the subject heart. 12. The implantable medical device of claim 1, wherein the pulse generator circuit is configured to provide the at least two pulses as first and second pulses that include respective portions that are at least partially overlapping in time. 13. The implantable medical device of claim 12, wherein the pulse generator circuit is configured to provide one or more of the first pulse and the second pulse including a biphasic waveform. 14. The implantable medical device of claim 12, wherein the pulse generator circuit is configured to generate electrostimulation pulses having a pulse polarity that is reversed after N beats. 15. An implantable medical device for providing cardiac defibrillation and dual-chamber pacing therapies, the implantable medical device comprising: an implantable lead assembly including a first electrode, a second electrode, and a third electrode, the first electrode comprising a coil electrode configured to provide a defibrillation energy to a subject heart, the second electrode configured to locally provide a pacing signal to a region at or near a His bundle of the subject heart, and the third electrode configured to provide a pacing signal to an atrium of the subject heart; anda pulse generator circuit, wherein the pulse generator circuit is configured to provide, to the implantable lead assembly, non-defibrillating electrostimulation pulses including the pacing signal and including at least two pulses of opposite polarity and occurring sufficiently proximate one another in at least one of time or distance to elicit myocardium capture of the subject heart. 16. The implantable medical device of claim 15, wherein the pulse generator circuit is configured to provide the at least two pulses of opposite polarity to one of the second and third electrodes. 17. The implantable medical device of claim 16, wherein the second and third electrodes are configured to be disposed in the right ventricle of the subject heart. 18. The implantable medical device of claim 17, comprising a fourth electrode configured to deliver an electrostimulation pacing therapy in a left ventricle of the subject heart. 19. The implantable medical device of claim 15, wherein the implantable lead assembly comprises a first lead from a can of the implantable medical device to the second electrode, and wherein the implantable lead assembly comprises a different second lead from the can of the implantable medical device to the third electrode. 20. The implantable medical device of claim 15, wherein the pulse generator circuit is configured to generate the defibrillation energy in response to information indicative of a tachyarrhythmia, the information obtained using an electrical signal received using one of the second and third electrodes when the electrodes are disposed in a right ventricle of the subject heart.
