The method of claim 1 wherein the measurement of the at least one local tissue condition is accomplished using a tissue condition monitor. 3. The method of claim 2 wherein the tissue condition monitor is linked, directly or indirectly, to a reperfusion controller that controls a parameter of reperfusate generated by a reperfusion generator. 4. The method of claim 3 wherein the reperfusion controller modulates the reperfusate to deliver differing levels of oxygenated fluid to the ischemic tissue. 5. The method of claim 4 wherein the oxygen level of the reperfusate is directly modulated by changing a partial pressure of oxygen and, thereby, an oxygen content of the oxygenated fluid. 6. The method of claim 4 wherein the oxygen level reaching the ischemic tissue is modulated by altering a flow rate of the oxygenated fluid. 7. The method of claim 2 wherein the tissue condition monitor measures conditions in a coronary sinus or other cardiac vein to determine oxygen extraction from an affected tissue. 8. The method of claim 2 further comprising administering a molecular probe to the host wherein the molecular probe is taken up by uninjured cells and is configured to be detectable as a guide to position a tissue condition monitor. 9. The method of claim 1 wherein the at least one local tissue condition being measured is selected from pH, pO2, pCO2, temperature, ion concentrations, cyclin dependent kinases (CDK), B-type natriuretic peptide (BNP), tumor necrosis factor α (TNF-α), creatine phosphokinase, lipid peroxidation byproducts, PMN elastase, NFkB, Na—H Exchange pump activity, leukocyte elastase, tumor necrosis factor (TNF), basic FGF, von Willbrand factor, p-selectin, nitrate, cytokines, nitric oxide or a nitric oxide metabolite, glutamate, aspartate, arginine, citrulline, acetylcholine, calcium, potassium, or dopamine.
