Compositions and methods are provided for modulating the growth, development and repair of bone, cartilage or other connective tissue. Devices and stimulus waveforms are provided to differentially modulate the behavior of osteoblasts, chondrocytes and other connective tissue cells to promote proliferation, differentiation, matrix formation or mineralization for in vitro or in vivo applications. Continuous-mode and pulse-burst-mode stimulation of cells with charge-balanced signals may be used. Bone, cartilage and other connective tissue growth is stimulated in part by nitric oxide release through electrical stimulation and may be modulated through co-administration of NO donors and NO synthase inhibitors. Bone, cartilage and other connective tissue growth is stimulated in part by release of BMP-2 and BMP-7 in response to electrical stimulation to promote differentiation of cells. The methods and devices described are useful in promoting repair of bone fractures, cartilage and connective tissue repair as well as for engineering tissue for transplantation.

The culture chamber of claim 1 wherein at least one of the pair of electrodes is comprised of a self-protecting metal. 3. The culture chamber of claim 2 wherein the self-protecting metal includes one or more of niobium, tantalum, titanium, zirconium, molybdenum, tungsten, and vanadium. 4. The culture chamber of claim 1 wherein the flexible membrane is comprised of plasma-etched silicone rubber. 5. The culture chamber of claim 1 wherein the pump increases pressure in the lower chamber to flex the flexible membrane towards the top side of the culture chamber at a first time period and decreases pressure to the lower chamber to flex the flexible membrane towards the bottom side of the culture chamber at a second time period. 6. The culture chamber of claim 5 wherein the pump oscillate between an increase in pressure in the lower chamber to flex the flexible membrane towards the top side of the culture chamber and a decrease in pressure to the lower chamber to flex the flexible membrane towards the bottom side of the culture chamber. 7. The culture chamber of claim 1 wherein the load source includes a pump fluidly coupled to the culture chamber at a location above the bottom sheet, the pump to increase pressure in the culture chamber. 8. The culture chamber of claim 1 wherein the load source includes a piston located at the top side of the culture chamber, the piston to form a fluid-tight seal with the culture chamber and move in a direction towards the bottom side of the culture chamber. 9. The culture chamber of claim 1 wherein the load source includes a pump fluidly coupled to the culture chamber via a first port and a second port, the first port and the second port located opposite each other across the culture chamber and along the one or more side walls, the pump to create a flow of fluid medium within the culture chamber between the first port and the second port. 10. The culture chamber of claim 9 further comprising: a first set of baffles located within the culture chamber proximate the first port, anda second set of baffles located within the culture chamber proximate the second port,wherein the first set of baffles and the second set of baffles are arranged to promote a uniform flow of the fluid medium between the first port and the second port. 11. The culture chamber of claim 1 wherein the bottom sheet includes a scaffold that includes growth-promoting and adhesion molecules.
