A polyaxial bone screw assembly includes a threaded shank body having an integral upper portion receivable in an integral receiver, the receiver having an upper channel for receiving a longitudinal connecting member and a lower cavity cooperating with a lower opening. The receiver can have crimp tabs, but is devoid of spring tabs and collet-like flexible structures. A down-loadable compression insert (some with lock and release feature), a down-loadable friction fit split retaining ring and an up-loadable shank upper portion cooperate to provide for pop- or snap-on assembly of the shank with the receiver either prior to or after implantation of the shank into a vertebra. The shank and receiver once assembled cannot be disassembled. The receiver and insert include aligned tool receiving apertures for lock and release of the polyaxial mechanism.

A pivotal bone anchor assembly for securing an elongate rod to a bone, the pivotal bone anchor assembly comprising: a receiver having a longitudinal axis, a lower body defining a cavity with a bottom opening, an upper shank capture region, and a lower shank locking region, and a pair of upwardly extending arms with opposed interior surfaces that define an open channel for receiving the elongate rod, the open channel communicating with the cavity to define a central bore, the open channel interior surfaces having a closure-mating structure formed therein, the central bore including an interference wedging structure that extends inwardly toward the longitudinal axis below the closure-mating structure;a shank having an upper end portion and an anchor portion extending distally from the upper end portion for attachment to the bone, the upper end portion being up-loadable into the receiver cavity through the bottom opening;a retaining structure positionable within the receiver cavity upper shank capture region prior to the shank and operable to capture the shank upper end portion, upon uploading through the bottom opening, in spaced relationship from an internal surface of the receiver cavity, the retaining structure being held in the cavity upper shank capture region until the shank upper end portion is capture therein; anda compression insert having an upwardly facing surface sized and shaped for engaging the elongate rod, a laterally facing exterior surface for engaging the receiver interference wedging structure, and a recess formed into a lower portion of the insert for receiving and overlappingly engaging an upper portion of the retaining structure,wherein the compression insert is positionable within the receiver central bore prior to the shank and in a side-to-side overlapping relationship with the retaining structure, with the compression insert being prevented from moving downwardly within the central bore by at least partial engagement with the receiver interference wedging structure extending inwardly toward the longitudinal axis; andwherein after the shank upper end portion is captured within the retaining structure, the compression insert is downwardly deployable within the receiver by direct engagement with a tool until the exterior surface of the compression insert comes into a forced interference engagement with the receiver interference wedging structure so as to prevent the compression insert from moving back up within the receiver central bore. 2. The pivotal bone anchor assembly of claim 1, wherein the retaining structure is non-pivotal with respect to the receiver. 3. The pivotal bone anchor assembly of claim 1, wherein upward movement of the retaining structure during the uploading of the shank upper end portion is restricted by engagement between a top surface of the retaining structure upper portion and an overlapping surface of the compression insert recess. 4. The pivotal bone anchor assembly of claim 1, wherein the retaining structure includes at least one vertical gap defined by spaced apart end surfaces. 5. The pivotal bone anchor assembly of claim 1, wherein the compression insert is downwardly deployable by a first tool that is removably inserted into the open channel along the longitudinal axis. 6. The pivotal bone anchor assembly of claim 5, wherein the compression insert is upwardly retrievable from the forced interference engagement using a second tool so as to release the shank from a fixed angular orientation relative to the receiver. 7. The pivotal bone anchor assembly of claim 1, wherein the retaining structure frictionally engages the shank upper end portion upon capture within the receiver cavity to allow for an unlocked but non-floppy angular positioning of the shank with respect to the receiver prior to a final locking of the pivotal bone anchor assembly. 8. The pivotal bone anchor assembly of claim 1, wherein the retaining structure is downwardly deployable with the compression insert within the receiver until a lower portion of the retaining structure becomes wedged between the internal surface of the receiver cavity and the shank upper end portion so as lock the shank in a fixed angular orientation with respect to the receiver. 9. The pivotal bone anchor assembly of claim 1, wherein the shank upper end portion is spherical in shape. 10. The pivotal bone anchor assembly of claim 1, wherein the shank is cannulated. 11. The pivotal bone anchor assembly of claim 1, further comprising a closure having a helically wound guide and advancement structure formed into an outer sidewall surface and engageable with the closure-mating structure formed into the interior surfaces of the receiver upwardly extending arms to capture the elongate rod within the open channel. 12. A pivotal bone anchor assembly for securing an elongate rod to a bone, the pivotal bone anchor assembly comprising: a receiver having a longitudinal axis, a lower body defining a cavity with a bottom opening and a pair of upwardly extending arms with opposed interior surfaces that define an open channel for receiving the elongate rod, the open channel communicating with the cavity to define a central bore, the open channel interior surfaces having a discontinuous helically wound guide and advancement structure formed thereon, the central bore including an interference structure formed therein that extends inwardly towards the longitudinal axis and is located below the guide and advancement structure;a shank having an upper end portion and an anchor portion extending distally from the upper end portion for attachment to the bone, the upper end portion being up-loadable into the receiver cavity through the bottom opening;a compression insert positionable within the central bore prior to the shank and having an upper surface configured to engage the elongate rod and a curved exterior surface configured to engage the receiver interference structure; anda retainer positionable within the receiver cavity prior to the shank and operable to capture the shank upper end portion, upon uploading through the bottom opening, in spaced relationship from an internal surface of the receiver cavity, the retainer having an upper portion that is at least partially receivable, in a side-to-side overlapping engageable relationship, within a recess formed into a lower portion of the compression insert, prior to the retainer capturing the shank upper end portion,wherein the retainer and compression insert are downwardly deployable within the receiver lower body after capture of the shank upper end portion until the exterior surface of the compression insert comes into a forced interference engagement with the receiver interference structure so as to prevent the compression insert from moving back up within the receiver central bore. 13. The pivotal bone anchor assembly of claim 12, wherein the retainer gets wider to receive the upper end portion of the shank before the retainer is downwardly-deployed within the receiver central bore. 14. The pivotal bone anchor assembly of claim 12, wherein upward movement of the retainer during the uploading of the shank upper end portion is restricted by engagement between a top surface of the retainer upper portion and an overlapping surface of the compression insert recess. 15. The pivotal bone anchor assembly of claim 12, wherein the retainer frictionally engages the shank upper end portion upon capture within the receiver cavity to allow for an unlocked but non-floppy angular positioning of the shank with respect to the receiver prior to a final locking of the pivotal bone anchor assembly. 16. The pivotal bone anchor assembly of claim 12, wherein the retainer is downwardly deployable within the receiver until a lower portion of the retainer becomes wedged between the internal surface of the receiver cavity and the shank upper end portion so as lock the shank in a fixed angular orientation with respect to the receiver. 17. The pivotal bone anchor assembly of claim 12, wherein the compression insert is downwardly deployable by direct engagement with a first tool. 18. The pivotal bone anchor assembly of claim 17, wherein the compression insert is upwardly retrievable from the forced interference engagement using a second tool so as to release the shank from a fixed angular orientation relative to the receiver. 19. The pivotal bone anchor assembly of claim 12, wherein the shank upper end portion is spherical in shape. 20. The pivotal bone anchor assembly of claim 12, wherein the shank is cannulated.
