A pivotal bone anchor assembly for anchoring to patient bone and coupling with an elongate rod via a closure, the pivotal bone anchor assembly comprising: a shank having a longitudinal axis, a head with a partial spherical shape defining a hemisphere with an outer spherical surface extending above and below the hemisphere, and an anchor portion extending downwardly from the head for fixation to the patient bone;a receiver having a longitudinal axis, a base defining a bore centered around the longitudinal axis and in communication with a bottom surface of the receiver through a bottom opening, and a pair of upright arms extending upward from the base to define an open channel configured to receive the elongate rod, the open channel in communication with the bore, the bore having an upwardly-concave lower spherical seating surface proximate the bottom opening configured to receive the shank head outer spherical surface and provide for pivotal movement between the shank and the receiver; andan insert positionable within the receiver bore and having an upwardly-facing surface engageable with the elongate rod and a downwardly-facing surface engageable with the shank head above the hemisphere,wherein, prior to the engagement of the insert downwardly-facing surface with the shank head, the shank head outer spherical surface is frictionally engaged below and above the hemisphere, respectively, by the lower spherical seating surface of the receiver and by an inwardly-concave upper spherical seating surface located in the receiver bore below the insert, so as to provide a non-floppy pivotal frictional engagement between the shank and the receiver. 2. The pivotal bone anchor assembly of claim 1, wherein the lower spherical seating surface of the receiver continuously frictionally engages around an entire circumference of the shank head outer spherical surface below the hemisphere when the shank head is disposed within the receiver and the shank longitudinal axis and the receiver longitudinal axis are co-aligned with respect to each other. 3. The pivotal bone anchor assembly of claim 1, wherein the upper spherical seating surface is integral with the receiver bore. 4. The pivotal bone anchor assembly of claim 3, wherein the upper spherical seating surface is continuous with the lower spherical seating surface of the receiver. 5. The pivotal bone anchor assembly of claim 1, wherein the upper spherical seating surface continuously frictionally engages around an entire circumference of the shank head outer spherical surface above the hemisphere when the shank head is disposed within the receiver and the shank longitudinal axis and the receiver longitudinal axis are co-aligned with respect to each other. 6. The pivotal bone anchor assembly of claim 1, wherein at least a portion of the receiver bottom surface is non-perpendicular to the receiver longitudinal axis and acutely upwardly angled toward the lower spherical seating surface of the receiver to truncate a lower portion of the lower spherical seating surface, thereby defining an acute edge portion of the receiver bottom opening that provides for increased pivotal movement of the shank relative to the receiver in at least one direction when the shank head is received within the lower spherical seating surface. 7. The pivotal bone anchor assembly of claim 1, wherein the receiver upright arms include break-off extensions. 8. The pivotal bone anchor assembly of claim 1, wherein the receiver upright arms further include outer surfaces with horizontally extending radiused tool attachment structures. 9. The pivotal bone anchor assembly of claim 1, wherein the receiver upright arms further comprise inner sidewall surfaces mateable with the closure. 10. The pivotal bone anchor assembly of claim 9, further comprising the closure and wherein the closure is a two piece closure including an outer portion and an inner portion threadably received in the outer portion, the outer portion including an outer splay control thread configured to threadably engage the upright arms inner sidewall surfaces. 11. The pivotal bone anchor assembly of claim 9, further comprising the closure and wherein the closure is a single piece closure that engages the elongate rod. 12. The pivotal bone anchor assembly of claim 1, wherein the shank is top loaded into the receiver bore and pushed down along the receiver longitudinal axis until the hemisphere is moved past the upper spherical seating surface and the shank head is captured into the non-floppy pivotal frictional engagement with the receiver. 13. The pivotal bone anchor assembly of claim 1, wherein the insert downwardly-facing surface has a spherical shape complementary with the spherical shape of the shank head above and below the hemisphere. 14. The pivotal bone anchor assembly of claim 1, wherein the insert has a central opening. 15. The pivotal bone anchor assembly of claim 1, wherein the insert further comprises a cylindrically-shaped body with upstanding arms defining a second open channel for receiving the elongate rod. 16. The pivotal bone anchor assembly of claim 15, further comprising the elongate rod and the closure, and wherein the insert upstanding arms extend above the elongate rod to engage an underside surface of the closure when the elongate rod is fully reduced into the receiver open channel via the closure. 17. The pivotal bone anchor assembly of claim 1, wherein the insert is twisted into place within the receiver bore. 18. The pivotal bone anchor assembly of claim 1, wherein the shank is cannulated along its longitudinal axis. 19. The pivotal bone anchor assembly of claim 1, wherein the receiver bore includes a recess with a downwardly facing surface to receive and at least partially overlap an outer surface of the insert. 20. The pivotal bone anchor assembly of claim 1, wherein the assembly further comprises a retainer attachable to a shank head capture structure integral with the shank anchor portion, andwherein an outer surface of the retainer at least partially defines the outer spherical surface of the shank head. 21. The pivotal bone anchor assembly of claim 20, wherein the shank head capture structure is up-loadable through the receiver bottom opening and into the receiver bore. 22. The pivotal bone anchor assembly of claim 1, further comprising a retainer to capture and hold the shank within the receiver. 23. A pivotal bone anchor assembly for anchoring to patient bone and coupling with an elongate rod via a closure, the pivotal bone anchor assembly comprising: a shank having a head portion with an at least partial spherical shape defining a hemisphere, and an anchor portion extending downwardly from the head portion for fixation to the patient bone;a receiver having a longitudinal center axis, a base portion defining an inner cavity in communication with a bottom surface of the receiver through a bottom opening, and a pair of upright arms extending upward from the base portion to define an open channel configured to receive the elongate rod, the open channel in communication with the inner cavity, and the inner cavity configured to receive the shank head portion and provide for pivotal movement between the shank and the receiver; andan insert positionable at least partially within the receiver inner cavity above the shank head portion and having a downwardly-facing surface configured to engage the shank head portion above its hemisphere and an upwardly-facing surface configured to engage the elongate rod to transfer a downwardly-directed force from the elongate rod onto the shank head portion,wherein at least a portion of the receiver bottom surface is non-perpendicular to the longitudinal center axis and upwardly angled into the receiver inner cavity to enlarge the receiver bottom opening and provide for increased pivotal movement of the shank relative to the receiver in at least one direction when the shank head portion is received within the receiver inner cavity, andwherein, prior to the positioning of the insert within the receiver inner cavity, the shank head portion is held in frictional contact within the receiver inner cavity between and against a lower spherical surface and an upper spherical surface located below and above the shank head hemisphere, respectively, to provide a non-floppy pivotal frictional engagement directly between the shank head portion and the lower and upper spherical surface. 24. The pivotal bone anchor assembly of claim 23, wherein the lower and upper spherical surface further comprise a spherical seating surface formed into a lower portion of the receiver inner cavity and configured to frictionally engage the shank head portion around an entire circumference of the shank head portion, both above and below a hemisphere thereof, to provide a non-floppy pivotal frictional engagement directly between the shank head portion and the receiver spherical seating surface. 25. The pivotal bone anchor assembly of claim 24, wherein the receiver spherical seating surface has a diameter substantially equal to a diameter of the shank head portion at its hemisphere such that the shank head portion is received within the receiver spherical seating surface with a tight, but movable, interference fit engagement. 26. The pivotal bone anchor assembly of claim 23, wherein the shank is top loaded into the receiver bore and pushed down along the receiver longitudinal axis until the hemisphere is moved past the upper spherical surface and the shank head portion is captured into the non-floppy pivotal frictional engagement with the receiver. 27. The pivotal bone anchor assembly of claim 23, wherein the insert downwardly-facing surface has a spherical shape complementary with the spherical shape of the shank head portion above and below the hemisphere. 28. The pivotal bone anchor assembly of claim 31, wherein the shank is top loaded into the receiver bore and pushed down along the receiver longitudinal axis until the hemisphere is moved past the upper spherical seating surface and the shank head is captured into the non-floppy pivotal frictional engagement with the receiver. 33. The pivotal bone anchor assembly of claim 30, wherein the upper spherical seating surface is proximate the shank head hemisphere when the shank head is engaged and supported by the lower spherical seating surface and the shank and receiver longitudinal axes are co-aligned.
