본 총괄 연구 개발과제의 최종 연구목표는 화상 및 창상 치료용 겔타잎 외용제제의 개발이다. 용기에 보관시에는 반고형제제이나 피부 적용시 겔화되어 박막을 형성함으로써 피부보호작용, 약물효과 지속작용을 가지는 외용제제의 개발이다. 이를 수행하기 위하여 다음 4개의 세부과제로 연구하였다.

본 총괄 연구 개발과제의 최종 연구목표는 화상 및 창상 치료용 겔타잎 외용제제의 개발이다. 용기에 보관시에는 반고형제제이나 피부 적용시 겔화되어 박막을 형성함으로써 피부보호작용, 약물효과 지속작용을 가지는 외용제제의 개발이다. 이를 수행하기 위하여 다음 4개의 세부과제로 연구하였다.
제1세부과제의 연구목표는 화상 및 창상 치료용 겔 타잎 외용제제의 최적처방에 의한 제조이다. 화상 및 창상을 치료하기 위한 반고형제제의 개발은 일반적으로 크게 두가지 관점을 고려하여야 한다. 하나는 치료과정에서 흔히 발생하는 반흔(scar)을 억제할 수 있는 약물의 선택과 그 배합성이고, 그리고 다른 하나는 그 약물을 가장 적절히 환부에 침투시켜, 치료효과를 극대화 할 수 있는 반고형제제 기제의 선택이다. 따라서 화상이나 창상을 치료하기 위한 주약물을 먼저 선택하고 그 약물에 가장 적합한 기제의 조성을 컴퓨터 최적화기법을 응용하여 최적처방을 유도, 결정하는 것이다. 이를 위하여 우선 겔타잎 외용제제를 피부에 적용하였을 때 박막을 형성하여 화상 및 창상면에서의 약물효과를 최대화 할 뿐만 아니라 상처 피부의 보호 작용을 동시에 가질 수 있는 연고기제 중 이를 만족시킬 수 있는 soft hydrogel을 본 연구과제에서는 선택하였다. 그리고 제 1차 연도에서는 soft hydrogel의 기제 구성 성분과 약물 방출 효과를 최대화하는 것을 종속변수로 하여 컴퓨터 최적화기법을 이용하여 최적 처방을 도출하였다. 제 2차 및 3차 연도에서는 이들 최적처방을 기본으로 응용하여 피부적용시에 발생하는 여러 가지 문제점등을 개선하여 보다 효과적인 처방 최적화를 유도하였으며, 화상 및 창상 치료에 많이 연구되고 있는 약물들 즉, 6-methyl uracil, tranilast, tetrandrine, 및 neomycin sulfate 등을 함유하는 soft hydrogel을 제조하여 각 세부에 공급하였다.
제2세부과제의 연구목표는 실험 동물 model을 이용한 약효 평가이다. 본 연구과제는 수용성 고분자 물질인 PVA을 이용하여 제조한 peel-off type soft hydrogel을 상처부위에 적용하여 피부표면에서 형성된 박막이 상처부위를 보호하고 삼출물을 흡수제거하며, 형성된 박막으로부터 지속적인 약물 방출로 인하여 wound healing이 보다 효과적으로 진행될 것으로 생각되어 이들의 치료효과를 여러 가지 동물모델을 이용하여 평가하고자 하였다. 실험에 사용한 동물모델은 thermal burn rat model, incision wound & excision wound rat model을 작성하여 사용하였다. 그리고 제 1차 연도에서는 여러 가지 실험 동물 모델을 작성하였으며, 또 그 치료 효력을 평가하였고, 제 2차 및 3차 연도에서는 이들 실험 동물을 이용하여 제 1세부 제조한 soft hydrogel을 적용하여 그 치료효력을 비교 평가하였다.
제3세부과제의 연구목표는 화상 및 창상 치료용 겔타잎 외용제제의 피하흡수 실험이다. 피부를 통한 약물의 침투속도의 측정에는 여러 가지 방법이 있으나 본 연구에서는 in vitro permeation method와 in vivo absorption method를 이용하여 실험을 수행하였다. 실험에 사용한 피부는 정상 피부 및 병태 피부를 사용하여 이들간의 상관성도 함께 검토하고자 하였다. 제 1차년도에서는 in vitro 및 in vivo method를 검토하고 또 실험에 사용되는 병태 피부 모델의 작성법에 관하여 검토하였다. 제 2차 및 3차년도에는 본 총괄과제에서 치료 약물로 선택한 것 중 tranilast 와 tetrandrine의 피부 투과 및 이들의 피하흡수에 영향을 미치는 피부 투과 촉진제들, 그리고 각 동물 피부 모델에 따른 약물 흡수 정도에 관하여 검토하였다.
제4세부과제의 연구목표는 최적처방에 의해 제조된 외용제제의 유변학적 특성(rtheological properties)을 체계적으로 파악함에 있다. 이를 위한지성 기제인 바셀린(vaseline, petrolatum)을 대상으로 하여 각종 조건변화에 따른 정상유동특성(steady shear flow properties) 및 동적 점탄성(dynamic viscoelastic properties)을 측정·평가하여 연고기제의 유변학적 특성에 관한 기초적 정보를 확보하였다. 그리고 1999년도 후반기(99. 11. ∼ 00. 4.)에는 실제로 제조된 외용제제의 기제 및 약물(마데카솔) 함유(1wt%) 제제에 대해 Advanced Rheometric Expansion System(ARES)을 사용하여 피부적용온도(Τ = 37 ℃)에서 정상유동특성, 동적 선형 점탄성 및 비선형 점탄성을 측정·평가함으로서 기제 및 약물함유 외용제제의 유변학적 특성을 비교·검토하였다. 나아가서 2000년도 전반기(00. 5. ∼ 00. 10.)에는 회전형 레오메터 사용을 통해 얻어지는 반고형 연고제제의 유변학적 특성에 미치는 레오메터 중에서의 벽면 미끄러짐(wall slip) 효과의 영향을 규명하기 위하여 바셀린을 대상시료로 선정하여 스트레인 제어방식(strain-controlled) 레오메터 및 응력 제어방식(stress-controlled) 레오메터 내부에서 발생하는 벽면 미끄러짐 현상을 관찰하였다. 끝으로 2000년도 후반기(00. 11. ∼00. 4.)에는 바셀린 및 제조된 외용제제 (기제 및 약물함유 제제)에 대해 ARES를 사용하여 피부적용온도 (Τ = 37 ℃)에서 일정전단속도 부여조건 및 전단속도의 단계적 증감에 따른 조건에서의 시간의존성 유동거동을 관찰함으로써 기준물질 (바셀린), 기제 및 약물함유 제제의 시간의존성 유변학적 특성을 비교·검토하였다.

Soft hydrogel has unique characters as follows: When soft hydrogel existing as semi-solid form in the container applies on the skin, it forms thin layer film on the skin within few minutes. By forming thin layer film, soft hydrogel shows controlled releasing effects of therapeutic drugs as well as skin protection effects. The final objective of this project was preparation of novel soft hydrogel dosage form and examination of their physico-biochemical characterization by pharmacokinetic and rheological methods. To achieve these goals, we have approached by four different ways from four different institutes.
The first research targeted to develop novel soft hydrogel for burn and wound healing by computer optimization technique. During last three years, they have succeeded to prepare new soft hydrogels containing 6-methyl uracil, tranilast, tetrandrine and neomycin sulfate. These drugs are studied for wound healing recently. In this research, they have developed new dosage form for soft hydrogel. During last three years, they have considered two major points for developing semi-dosage form in wound healing treatment. One is to find new drug for inhibiting spreading scar around wound site. The other is to discover the optimal vehicle for penetrating the drugs efficiently to wound site and for increasing the recovery from wound or burn. At first year, they have used computer optimization technique to find the optimum formulation, Finally, they have found the optimal formulation for preparing soft hydrogel by using several vehicles and several conditions for maximum drug releasing. At second and third year, they have applied the first year´s established formulation to rat skin for optimization in practical points. They have found quite important factors for revising optimal soft hydrogel formulation. Finally, they have established the most optimal formulation by combination several drugs and enhancers. They have prepared large amounts of new soft hydrogel and provided to other research groups to examine physico-biochemical properties.
The second research team are aimed to estimate the pharmacological effects of soft hydrogel by using rat animal model system. They have used peel off type soft hydrogel by using water-soluble PVA polymer as vehicle. This hydrogel was assumed to protect wound site and release drug slowly by forming thin layer film. Also it was suggested that new hydrogel might reabsorb and exclude the exudate from wound site. To obtain clear evidence, they have used three animal models. One is thermal burn rat model and the others is incision wound & excision wound rat model. For drug components for treatment, they have used 6-methyl uracil, tranilast, tetrandrine and neomycin sulfate. At the first year, they have prepared animal models and checked the preliminary experiments. Second and third year, they have estimated the pharmacological effects of prepared soft hydrogel. They have obtained satisfactory results as suggested hypothesis.
The third research team are aimed to measure the skin absorption rate by using prepared soft hydrogel. They have used mainly in vitro permeation method and in vivo absorption method. They also used control skin and disease-induced skin as skin model to estimate the biological relationship between these two skin. At first year, they have established the disease-induced skin model and in vitro or in vivo absorption model. During second and third year, they have examined penetration rate by using tranilast and tetrandrine as drug. Also, they have checked several enhancing factors for skin penetration. Finally they have intensively examined the absorption rates by using animal skin model.
The objective of the 4th Sub-project was to investigate systematically the rheological properties of gel-type soft hydrogel. In a first step, the fundamental informations on the rheological properties of oinment bases were obtained by means of characterizing the steady shear flow and dynamic viscoelastic properties of vaseline (petrolatum). In a second step, the steady shear flow and dynamic viscoelastic properties of gel-type soft hydrogel (vehicle) and madecasol-containing soft hydrogel were measured and characterized using an Advanced Rheometric Expansion System(ARES at actual usage temperature. In a third step, the wall slip phenomena of vaseline in rotational rheometry systems (stress-controlled as well as strain-controlled) were thoroughly observed in order to elucidate the wall slip effects on the measured rheological properties of semi-solid gel-type soft hydrogels. In a final step, the time-dependent flow behaviors of gel-type soft hydrogel (vehicle), madecasol-containing, and madecasol-HPE-containing soft hydrogels were measured and characterized using an ARES at actual usage temperature.
In conclusion, we have obtained several good expected scientific results from four research groups. These results can be applied to develop new soft hydrogel drugs for wound healing.
