연구목적
자궁경부암의 원인은 인유두종바이러스 (human papilloma virus, HPV)감염으로 알려져 있고 이에 대한 연구가 매우 활발히 진행이 되고 있다. 그러나 HPV 만의 감염으로는 설명하기 어려운 암화과정이 실제로 존재하고 있으며 이의 확진을 위해서는 첫째로는 자궁경부암 유전자의 변이와 발현의 차이가 함께 연구가 되어야 하며, 둘째로는 이러한 유전적 변이에 의한 변화하는 단백질을 찾아야 한다. 이러한 단백질이 혈액을 통하여 노출 된다면 조직이나 형태학적인 진단보다 앞 설 것으로 생각되며, 이를 통하여 자궁경부암
연구목적
자궁경부암의 원인은 인유두종바이러스 (human papilloma virus, HPV)감염으로 알려져 있고 이에 대한 연구가 매우 활발히 진행이 되고 있다. 그러나 HPV 만의 감염으로는 설명하기 어려운 암화과정이 실제로 존재하고 있으며 이의 확진을 위해서는 첫째로는 자궁경부암 유전자의 변이와 발현의 차이가 함께 연구가 되어야 하며, 둘째로는 이러한 유전적 변이에 의한 변화하는 단백질을 찾아야 한다. 이러한 단백질이 혈액을 통하여 노출 된다면 조직이나 형태학적인 진단보다 앞 설 것으로 생각되며, 이를 통하여 자궁경부암의 조기 진단과 치료 경과의 관찰 그리고 경우에 따라서는 새로운 치료의 시도가 가능할 것으로 생각된다. 따라서 본 연구에서는 자궁경부암의 진단, 치료방침 결정과 함께 예후를 알기 위한 DNA, protein biomarker를 찾고자 하였다.
연구방법
■양성질환으로 자궁 적출술을 시행한 17명의 자궁경부에서 추출한 RNA를 동일한 농도로 섞어 대조군으로 이용하였고, 3명의 자궁경부 편평상피암 조직의 RNA를 실험군으로 하여 differential display reverse transcription polymerase chain reaction(DDRT-PCR)을 실시하였다. DDRT-PCR 결과는 영상분석법을 통하여 발현 정도를 확인 하였고 gene ontology(GO) 분석법을 이용하여 유전자들의 다양한 생물학적 기능을 분석하였다.
■4,700 유전자로 이루어진 cDNA microarray와 gene ontology 법을 이용하여 11명의 자궁경부암 환자 조직(침윤성암 IIb-IIIa)을 분석하였다. Total RNA는 Cy3와 Cy5로 labellin화고, 대조군으로는 HaCaT 세포주를 사용하였다.
■SELDI-TOF MS 단백질 칩 기술을 이용하여 I5명의 자궁경부암 환자와 10명의 정상인의 혈청 시료에서 단백질 발현의 차이를 분석하였다.
연구결과 및 결론
■대조군과 비교하여 자궁경부 편평상피암 조직에서 1.5 배 이상 차이의 발현을 보이는 191개의 유전자를 선별하였다. 191개 중 128개의 유전자가 암 조직에서 발현이 증가하였고, 63개의 유전자가 발현이 저하 되었다. 이 중 46개의 유전자를 GO 분석법에 따라 분류한 결과, 암 조직에서 발현이 증가 된 유전자들은 desmoplakin, apoptosos inhibitor-5 등 epidermal differentiation과 anti-apoptosis의 기능에 관여하는 유전자들이었고, 감소된 유전자는 ribosomal protein s25, death associated protein 5, laminin 등 protein biosynthesis, cell death와 cell adhesion에 관여하는 유전자들 이었다. 자궁경부암의 암화과정에 관여하는 유전자들의 발현을 보기 위하여 DDRT-PCR과 GO 분석을 통하여 분석한 결과 biological process, cellular process, molecular function에 관련된 유전자를 확인 할 수 있었다. 향후 GO분석을 이용하여 더 많은 유전자의 기능 연구를 통하여 자궁경부암 발생에 관여하는 유전자를 확인 할 수 있을 것으로 사료된다.
■11명의 자궁경부암 환자와 HaCaT를 비교하여 cDNA chip 실험을 실시한 결과 11명의 환자중 적어도 8명에서 2배 이상 발현이 증가하였거나 저하된 74개의 유전자를 얻었다. 이들 유전자중 33개의 유전자는 발현이 증가한 유전자고, 41개의 유전자는 발현이 저하된 유전자였다. 유전자 발현 양상을 Gene Ontology 분석법에 의해 기능별로 분류하였다. GO 분석 결과 자궁경부암 발생은 세포사, 단백질 합성, 핵산대사가 저해되어 발생하며, 또한 nucleic acid binding 및 structural molecule activity에 관여하는 유전자들도 발현이 저하되어있었다. 이와는 대조적으로 skeletal development, immune response 그리고 extracellular activity에 관여한 유전자들의 발현은 증가해 있었다. 따라서 이러한 결론은 자궁경부암 발생의 pathogenetic cellular processes의 가능성을 제시하고, 기능이 저하된 유전자들이 pathogenic pathways에 중요한 역할을 할 수도 있다는 생각된다. GO 분석은 cellular process 수준에서 접근하여 cDNA microarry의 복잡한 발현양상을 해석할 수 있도록 하며, 뿐 만 아니라 자궁경부암 특이적 병인론에 중요한 예후인자를 중요한 예후 유전자를 발견할 수 있었다.
■통계적으로 유의한 차이(p<0.01)가 나타나는 26개의 단백질을 얻었으며, 그들 중 두 개의 단백질이 15.1kDa 과 kDa의 헤모글로빈 알파와 헤모글로빈 베타로 판명되었다. 자궁경부암 환자 그룹의 혈청에서 정상군과 비교하여 헤모글로빈 발현이 높게 나타났음을 확인하였다.또한 헤모글로빈과 관련하여 세포내 발병 기전을 기능적으로 밝힘으로서, 자궁경부암의 병인론적 경로를 발견하는데 중요한 단서를 제공하였다.

Cause of cervical cancer is known as human papilloma virus (human papilloma virus, HPV) infection and research is gone very actually. But, it is inexplicable to explain carcinogenesis by only infection of HPV. Therefore, change of cervical cancer gene and difference of revelation should be studied together firstly for different view diagnosis, alongside of finding the changed proteins by genetic changes. If these proteins are exposed through blood, the diagnosis of the protein in blood would stand front than diagnosis in tissue or of physical status. Early diagnosis, observation of cervical cancer with treatment, and attempt of new treatment are possible sometimes through this. Therefore, this research wished to find diagnosis of cervical cancer, DNA to know prognosis with treatment policy decision, protein biomarker.
Materials and Methods
■Cervical cancer biopsies were obtained from patients at the Department of Obstetrics and Gynecology, The Catholic University of Korea. The disease status was assigned according to the International Federation of Gynecology and Obstetrics. The squamous cell carcinoma tissue samples of 3 patients invasive cancer stage Ⅱ(1), Ⅳ(2) were investigated by mRNA differential display. As a control, we used a common reference that was mixed with equal amount of RNA obtained from 17 normal cervix to obtain variation-independent control. Also, we constructed hierarchical functional structures using gene ontology. Then, the specific function groups were correlated with differential gene expression profiles. In addition, specific gene expression patterns in several tissue samples were investigated by using DDRT-PCR analysis.
■The tissue samples of 11 patients (invasive cancer stage Ⅰb- Ⅲa) were investigated by a cDNA microarray of 4,700 genes, hierarchical clustering and the Gene Ontology (GO). Total RNA from cervical cancer and non-lesional tissues were labeled with Cy5 and Cy3. The HaCaT human epithelial keratinocyte cell line was used as a negative control cell. The stages of invasive cancer were Ⅰb to Ⅲb. All specimens were obtained by punch-biopsies and frozen in liquid nitrogen until required.
■Fifteen serum samples were obtained from cervical cancer patients (n = 15) and healthyfemales (n = 10) using the SELDI-TOF MS ProteinChip technology were investigated.Results and Discussion
■Deferentially expressed 191 genes were identified in tumor samples. Of these genes, 128 were up-regulated and 63 were down-regulated above 1.5-fold. The gene expression profiles were classified into 46 mutually dependent function sets and organized into sub-function sets depending on the cervical cancer pathway, suggesting the potentially significant genes of unknown function affected by carcinogenesis pathway. The genes related to metabolism, signal transduction, and chaperon activity were significantly up-regulated. In contrast, significant down-regulations were shown in nucleic acid binding activity, tumor suppressor and structural activity. Reliable gene expression data shows the validation of profiling method for studying the cervical cancer-specific pathway. The specific functions assigned to each expressed gene were correlated with gene ontology for the establishment of a powerful cervical carcinogenesis pathway. The results suggest that the deferentially regulated cellular process profiles have an important impact on discovery of pathogenic pathway in human cervical squamous cell carcinoma and provide the potentially significant genes of unknown function. Also, the gene ontology analysis can overcome the complexity of the expression profiles of mRNA differential display via a cellular process level approach. Thereby, a valuable prognostic candidate gene with real relevance to disease-specific pathogenesis can be found at the cellular process levels.
■We identified 74 genes showing a more than 2 fold difference in their expression in at least 8 out of 11 patients. Among these 33 genes were up-regulated, in contrast, 41 genes were down-regulated. The gene expression profiles were classified into mutually dependent 345 function sets, resulting in 611 cellular processes according to the GO. The GO analysis showed that cervical carcinogenesis underwent complete down-regulation of cell death, protein biosynthesis, and nucleic acid metabolism. Also, genes belonging to nucleic acid binding and structural molecule activity were significantly down-regulated. In contrast, significant up-regulation was shown in skeletal development, immune response, and extracellular activity. These data suggest that the regulated genes and cellular processes could be further used for predicting prognosis and diagnosis of cervical cancer patients, and further investigation and functional characterization of the identified genes is warranted.
■Twenty six protein peaks with lowest p-value (〈 0.01) were obtained from the typical spectra with SELDI-TOF-MS. Among them, 2 proteins of 15.1 kDa and 15.8 kDa were identified, and they were hemoglobin alpha and beta chain. The expression level of hemoglobin in cervical cancer serum is clearly higher than in normal serum. Also, hemoglobin-related pathogenetic cellular processes showed that has an important impact on discovery of pathogenic pathway in cervical cancer.
