연구목표
본 연구는 우리나라 주요 6개도시 지역을 대상으로 하여 대기오염이 저체중아
및 조산아 출생에 미치는 영향요인을 분석하고자 하였다.

연구성과
1) 서울지역에서 장기적인 추세변동, 임신주수, 산모의 연령, 부모의 교육수준, 출생아의 성별과
출생 순위를 통제하고 대기오염 노출 기간을 임신 전체 기간, 임신 제 1, 2, 3분기, 임신 각 달로
구분하여 대기오염 노출과 저체중아의 관련성을 분석한 결과, 임신 전반기 즉, 1분기와 2분기의
CO, PM10, SO2, NO2 노출이 저체중아 발생과 관련이 있었다. 한편 기존의 연구가 임신 분기별 노
출을 주로 고려한 반면 본 연구에서는 보다 세부적인 노출 시기를 고려하였으며 임신 각 달별 대
기오염 노출을 이용하였을 때는 임신 2개월에서 5개월까지의 CO 노출, 임신 3개월에서 5개월까지
의 SO2, NO2 노출 그리고 임신 2개월과 4개월의 PM10 노출이 저체중아 발생을 증가시키는 것으로
나타났다.
2) pooled analysis를 통해 6개 도시의 연구 결과를 병합해 본 결과 임신 제 1분기와 제 3분기에
는 모수 효과 모형이 적합하였으나, 임신 2분기에는 각 도시의 추정치 간에 유의한 차이가 있는 것
으로 나타나 랜덤 효과모형을 적용하였다. 임신 1분기에서 일산화탄소가 25 percentile에서 75
percentile 증가함에 따라 저체중아의 비차비는 1.037 (95% CI, 1.014-, 1.059), PM10은 1.046 (95%
CI, 1.023-, 1.070), NO2는 1.014 (95% CI, 0.991-, 1.038), SO2는 1.032 (95% CI, 1.011-, 1.053)로 나
타났다. 한편 임신 3분기의 O3의 저체중아에 대한 비차비는 1.045 (95% CI, 1.022-, 1.068)이었다.
3) 대기오염 농도와 조산아와의 관련성을 분석한 결과 임신 제 1분기의 CO, NO2 농도는 time
trend, 제태 연령, 산모 연령, 부모의 교육수준, 부모의 직업, 신생아 성별, 출생순위를 통제하였을
때도 조산아 출생에 영향을 미치는 것으로 나타났다.
4) PM10 노출은 후기 신생아기 사망, 특히 호흡기계 사망에 영향을 주는 것으로 나타났다. 후기
신생아기는 2-64, 65세 이상의 노인에 비해 대기오염의 영향에 더욱 민감한 것으로 나타났다.

Our purpose for this study was to determine whether air pollution is associated with
low birth weight and preterm, a significant determinant of post-neonatal infant
mortality and morbidity. To investigate this relation, we examined the associations
between air pollution exposures during pregnancy and low birth weight (LBW) in six
cities of Korea for the period 1996-1998.

1) Air pollution and LBW
We evaluated the relationships between LBW and air pollution exposure levels in
Seoul, Korea. Using the air pollution data, we estimated the exposure during each
trimester and also during each month of pregnancy on the basis of the gestational age
and birth date of each newborn. Generalized additive logistic regression analyses were
conducted considering infant sex, birth order, maternal age, parental education level,
time trend, and gestational age.
2) Air pollution and LBW in six cities
We performed two-stage analyses. First, for the individual city analysis, generalized
additive logistic regression models, using a smoother to control for time trend patterns,
were applied. We adjusted gestational age, maternal age, parental educational level,
parity, and infant sex in the model. Second, we calculated the city-specific estimates
for air pollution associated with LBW and computed overall estimates using fixed and
random effect models
3) Air pollution and Preterm
The associations between ambient air pollution and preterm were evaluated with a
generalized additive model (GAM) estimating binomial distribution adjusting for
maternal age, parental educational level, parity, and infant sex.
4) Air pollution and Infant mortality
Daily counts of total and respiratory death along with daily levels of meteorological
variables and air pollutants were analyzed using generalized additive Poisson regression.
The relative risks (RR) of mortality for interquartile changes of the levels of particulate
matter less than 10 m (PM10) were calculated on the same day.
Effectiveness
of
Research
1) We found that exposure to air pollution during the first and second trimester was
associated with an increased risk for LBW. More specifically, we found that the risks
for LBW increased with CO exposure between the second and fifth months, and with
SO2 and NO2 exposure between the third and fifth months of pregnancy. PM10
exposure in second and fourth months was associated with LBW.
2) The city-specific risks varied across cities. Furthermore, they showed that the
heterogeneity found in the air pollutants among cities reflects effect modification, which
is explained by specific city characteristics. Pooled effect during the first trimester
showed air pollutants were significantly associated with LBW. Risk estimates were
1.037 (95% CI, 1.014-1.059) for CO, 1.046 (95% CI, 1.023-1.070) for PM10, 1.025 (95%
CI, 1.004-1.046) for NO2 and 1.032 (95% CI, 1.011-1.053) for SO2.
3) The preterm was associated with exposure to CO and NO2 during the first
trimester. The odds ratio of preterm was 1.06(95% CI=1.03-1.09) and 1.06 (95%
CI=1.04-1.09) in first-trimester CO and NO2 exposure, respectively.
4) We found that postneonatal infants are most susceptible to PM10 in terms of mortality,
especially of respiratory mortality. Postneonatal mortality increased by 14.2% for each 42.9㎍/㎥
rise in PM10. Regarding the cause of death for postneonatal infants, PM10 had a greater effect
upon respiratory mortality than upon total mortality, with an estimated risk ratio of 2.02 (95%
CI, 1.78-2.28).
