1. 1차 online 설문조사 결과: 우리나라 청소년들의 수면실태를 알아보기 위하여 전국에 분포되어 있는 150개의 중고등학교 26,395명을 대상으로 시행한 1차 조사는 online 으로 시행된 수면습관과 수면 문제, 학업성취도 및 정서에 관한 설문조사로 중학생은 52%, 13,724명, 일반계 고등학생은 32.9%, 8,681명, 특성화계 고등학생 15.1% 3,990 명이었고, 남녀 각각 51.6%, 48.4%로 고르게 분포되어 있었다. 전체 학생들의 주중 하루 수면 시간은 평균 399.4분이었고 주말 수면시간은 546.7분
1. 1차 online 설문조사 결과: 우리나라 청소년들의 수면실태를 알아보기 위하여 전국에 분포되어 있는 150개의 중고등학교 26,395명을 대상으로 시행한 1차 조사는 online 으로 시행된 수면습관과 수면 문제, 학업성취도 및 정서에 관한 설문조사로 중학생은 52%, 13,724명, 일반계 고등학생은 32.9%, 8,681명, 특성화계 고등학생 15.1% 3,990 명이었고, 남녀 각각 51.6%, 48.4%로 고르게 분포되어 있었다. 전체 학생들의 주중 하루 수면 시간은 평균 399.4분이었고 주말 수면시간은 546.7분으로 대부분의 학생들이 약 2시간 이상 주말에 수면을 보충하고 있음을 확인하였다. 주중 수면시간은 중학생이 437.1분으로 가장 많이 잤으며 특성화계 고등학생은 387.3분, 일반계 고등학생은 345.9분으로 가장 적게 잤다. 주말에는 중학생은 566.6분, 특성화계고 559.5분, 일반계고 학생들은 509.9분으로 주중에 더 적게 자던 일반계 학생들이 여전히 주말에도 적게 잤다. 66.6%의 학생들이 수면이 부족하다고 느끼고 있었고, 수면시간이 짧을수록 주간 졸림 지수가 증가하고 우울지수가 증가하였으며 자살에 대한 생각도 비례하여 늘었다. 5시간 미만으로 자는 군이나 8시간 이상 자는 군에 비하여 5시간 - 8시간 사이로 자는 그룹들이 평균 국영수 및 전체 석차가 더 좋아 적당한 수면이 학업에 중요함을 보여주었다. 1차 조사에서는 불면증 28.4%, 주간 졸림증 16.4%, 코골이 20.0%, 무호흡 8.6%, 하지불안증후군 의심증상 23.4%, 사건수면 17.8%로 상당히 높은 수면장애 또는 수면관련 증상을 가지고 있었고, 16.1%에서 수면문제로 인하여 주간기능에 자주 영향을 미친다고 대답하였다.
인터넷 중독의 수준이 높을수록, 우울 및 자살 생각의 수준이 높을수록 수면시간이 짧은 것으로 나타났다. 먼저, 인터넷 중독과 수면시간과의 관계를 살펴보면, 인터넷 중독이 심할수록 취침시간 및 기상시간이 늦어지는 경향이 있었으며 수면잠복기도 길어졌다. 다음으로 우울 및 자살 생각의 수준이 심할수록 취침시간은 늦어지는 경향이 있었다. 또한 우울 및 자살 생각의 수준이 심할수록 수면잠복기도 길어지는 것으로 나타났다. 더불어, 인터넷 중독 및 우울, 자살 생각과 관련하여 일상생활에서의 변인 및 학습과의 관계도 살펴보았는데, 인터넷 중독 및 우울, 자살 생각의 수준이 심할수록, TV시청시간 및 컴퓨터 사용시간이 많았고 낮은 자기주도적 학습능력 및 낮은 성적을 보였다. 또한 인터넷 중독과 우울, 자살생각 간에도 높은 정적 상관을 보였다.
2. 2차 online 설문조사 결과: 2차 설문조사 대상자는 1차 설문조사에서 전반적인 수면의 질 평가(global sleep assessment questionnaire) 항목에서 수면의 질이 저하된 학생들로서 대상자 15,438명 중 4,734명이 응답하였다. 분포는 남자 1,644명(34.7%), 여자 3,090명(65.3%)이었다. 전체 학생의 주중의 하루 수면시간은 평균 383.1±78.75분, 주말의 평균 수면시간은 519.9±102.25분이었다. 주중 평균 수면시간은 중학교 1학년 449.23분, 특성화계 고등학교 3학년은 364.3분, 일반계 고등학교 3학년은 312.88분이었다. 각 학년별 수면 질의 차이는 없었는데 약 3분의 1이 한 주에 한 번 이상의 공부에 열중하는데 어려움을 겪는다고 하였으며, 38.4%가 자신의 수면의 질이 나쁘다고 답하였다. 코골이는 714명(15.1%)이 있었으며 남자에서 21.2%로 여자 11.8% 보다 빈도가 높았다. 역시 수면무호흡도 남자에서 7.1%로 여자 5.6%로 남자가 많았다. 낮잠을 자는 청소년은 약 3분의 1이었고, 528명(11.2%)가 허탈발작을 호소하였다. 하지불안증후군은 전체 9.0%였으며 여자가 10.1%로 남자 6.9%에 비하여 의미 있게 높았다. 불면증 척도(Insomnia Severity Index, ISI)가 8점 이상은 36.0%로 불면증이 의심되었다.
3. 3차 수면검사 결과: 3차 수면다원검사는 2차 설문조사에서 수면무호흡증, 기면증, 불면증이 의심되는 학생들 중에 선별하여 총 106명을 검사하였다. 모두 판독을 하였으며 결과를 분석하였다.
수면무호흡이 의심되어 수면검사를 받은 60명 중 19명(31.7%)이 수면무호흡증으로 진단되었다. 중증 2명, 중등도 5명, 경증 12명이었다. 불면증을 호소하는 33명의 학생의 수면검사는 2명에서 경증 수면무호흡이 발견되었고 나머지에서는 다른 수면장애가 없는 일차성 불면증올 판단되었다. 기면증 가능성이 있는 학생들 중 야간 및 주간 수면검사를 받은 13명 중 3명이 기면증이 확진되었고, 2명은 기면증이 의심되었다(38.5%). 종합적으로 설문지의 수면무호흡과 기면증의 예측율이 38.5 – 46.2%로 유용성이 있을 것으로 생각된다.
4. 결론: 우리나라 청소년들의 수면시간은 외국에 비하여 매우 부족한 실정으로 청소년 수면시간을 늘리기 위한 범국가적인 운동과 교육이 필요하다. 상당수의 학생들이 수면 문제를 가지고 있으며, 설문지를 통한 조기 진단과 지역 수면센터를 이용하여 적절한 수면생활 개선 및 치료가 이루어질 수 있도록 노려해야 하겠다. 이에 본 연구지들은 조사결과를 토대로 청소년 수면건강 가이드라인을 개발하였다.

The survey questionnaire included sleep habits, daily life, sleep problems, school performance and emotion, Total 26,395 students participated in the online survey. The mean daily total sleep time(TST) in total students was 399.4 min during weekday and 546.7min during weekend. The daily TST of middle school students was 437.1min whereas regular high school students slept 345.9min per day and industrial high school students 387.3min. 66.6% of students felt their TST is not enough, The shorter the TST was, daytime sleepiness and depression scale were higher. Suicidal idea was increased too. School record was better in students with sleeping 5-8 hours per day than sleeping less than 5 or more than 8 hours. Insomnia was observed in 28.4% of students, snoring in 20.0%, sleep apnea in 8.6%, restless legs syndrome- like symptoms in 23.4% and parasomnia in 17.8%. 16.1% complained of frequent daytime dysfunction due to sleep problems.
The internet addition was related to high prevalence of depressive mood and suicidal ideation. The more severe the internet addition was, bedtime and wake-up time were more delayed. Students with higher depression scale and suicidal idea also showed delayed bedtime and prolonged sleep latency. Students with internet addiction had longer time of watching TV and PC use. The significant correlation was observed between internet addiction and depression and suicidal idea.

The 2nd online survey: We asked 15,438 students with decreased quality of sleep on Global Sleep Assessment Questionnaire to respond to the 2nd questionnaire. 4,734 (male 1,644, female 3,090) responded. The mean daily TSTs were 383.1 and 519.9 min during weekday and weekend respectively. The mean daily TST of 7th grade was 448.23min whereas that of regular high school (10 - 12th grades) was much shorter (373.5min). The TST of 12th grade in regular high school was shortest (312.9min). 38.4% complained of poor sleep quality and one third of students had daytime dysfunction one or more per week. Snoring was reported in 714(15.1%) and sleep apnea in 7.1% of male and 5.6% of female students. One third of students had naps and 528(11.2%) complained of cataplexy(sudden loss of muscle tone on emotional changes). Symptoms of restless legs syndrome were reported in 9.0% and more frequent in female(10.1%) than in male(6.9%). 36.0% was suspected as insomniac (8 ≥ Insomnia Severity Index).
3. The result of sleep studies: The sleep study was performed in students who are likely to have sleep disorders including obstructive sleep apnea(OSA), narcolepsy, and insomnia. Total 106 students had a sleep study. Until now the results of sleep study(all overnight polysomnography and 13 Multiple Sleep Latency Test additionally) were gathered in 106. OSA was diagnosed in 19(31.7%) of 60 students who had a sleep study for suspicious OSA.
Only one of 33 who complained of insomnia showed a mild OSA and most of them appeared to have primary insomnia such as psychophysiological insomnia. Five(38.5%) of 13 students with excessive daytime sleepiness and cataplexy) showed sleep study results consist with or partially compatible with narcolepsy.
4. Conclusion: The daily TST of Korean adolescents was much shorter than that of other countries including USA, Japan and China. To increase TST of students, nationwide education and campaign are necessary. Considerable number of students had sleep problems. We need to encourage early detection and appropriate treatment of them through each school and local sleep centers. Based on the result of our study, we developed a guideline for a sleep health of adolescents in Korea.
