WHO 연구 자료에 의하면 주요 우울장애로 고통 받는 기간이 결핵이나 교통사고에 따른 장애 기간보다 더욱 긴 것으로 나타났으며, Murray와 Lopez는 WHO-World Bank-Harvard University 연구에서, 주요 우울장애로 인하여 국민이 받는 장애도(disability)가 1990년에는 4위이며, 2020년에는 2위로 국민에게 부담을 주는 대표적 질환이 될 것이라고 예측함. 이러한 우울증으로 인한 사회경제적 부담이 1990년도 미국에서 총 437억불에 달하는 것으로 보고 되었고, 질환 자체의 치료에 소비된 비용이 120억불로 28%이었으며, 질환으로 인한 수입 감소, 실직, 자살 등 이차적인 비용이 전체의 55%인 387억불에 달하는 것으로 연구 결과가 보고 됨. 최근 면역조절과 신경전달에 관여하는 유전자들과 주요 우울증의 병인과의 관련성이 부각되고 있는 상황이나 국내에서는 아직까지 우울증과 관련된 유전학적 연구가 미흡한 실정으로 역학 조사에 의한 유전연구를 제외하면 생물학적 연구 실적이 그리 많지 않은 형편임. 따라서 유전성향이 매우 강한 우울증에서의 체계적이고 과학적인 방법에 의한 기초 연구가 절실히 요청되고 있는 실정임. 이러한 배경에서 본 연구자는 우울증의 발병과 관계되는 유전자 특히, 면역유전학적 기능과 관계되는 유전자를 조사하여 이 질환의 유전적 배경을 연구하고 이러한 유전적 변화와 우울증의 발생, 임상양상, 면역학적 지표(싸이토카인 생성능)와의 관계를 관찰함으로써, 우울증에 있어 유전적 특성을 확인하고 생물학적 지표에 의한 동질성 질환 집단으로 재분류하고자 본 연구를 시행하였음. 또한 이들 결과를 토대로 하여 향후 우울증의 유전 상담, 위험군에서의 환자 조기발견 및 치료, 새로운 치료법 개발 및 예방대책 등을 수립하고자 하는 것이 이차적 목표이었음. 연구방법 : 1) 대상선정 미국정신의학회 정신장애 분류편람의 진단기준(DSM-IV)과 해밀튼 우울증 증상척도(Hamilton Depression Rating Scale)에 의하여 주요 우울증 환자를 선정하였다. 정상 대조군은 환자군의 성별과 연령에 어울리며 정신질환의 병력이나 가족력이 없는 사람을 선발하였다. 모든 대상자들은 본 연구에 대한 설명을 들은 후 서면 동의를 하였다. 현재 우울증 환자군 및 정상대조군의 혈액과 DNA는 해당 임상 정보들과 함께 유전자 정보 데이터베이스에 계속하여 자료로 구축되고 있다. 2) HLA 형별분석 HLA class I 에 대하여는 미세림프구세포독성방법, 그리고 class Ⅱ 형별은 역점적교잡법을 이용하여 유전자형을 동정하였다. 현재 우울증 환자에 대한 형별 분석이 지속적으로 시행되고 있는 상태이나 자료의 숫자가 부족하여 통계분석은 하지 않고 있음. 3) DNA 추출 말초정맥혈에서 Ficoll-Paque 비등차용액을 이용하여 임파구를 분리하였으며 분리된 임파구에 Proteinase K를 처리하여 DNA를 추출하였다. 4) Genotyping ① PCR-Nco I Restriction Fragment Length Polymorphism (RFLP) ② PCR-Single Strand Conformation Polymorphism (SSCP) ③ ARMS-PCR (Amplication Resistant Mutation System-PCR) 5) 통계 분석 통계분석은 SPSS 9.0 for WINDOWS를 이용하여 전산분석 하였다. 분석에는 χ2 test with two-tailed Fisher's exact test를 이용하였고, 상대 위험도 (relative risk, RR)는 odds ratio로 계산하였다. 통계학적 유의성 수준은 p<0.05로 하였다. 결과 및 기대 : 주요우울증에서 몇가지 cytokines과 면역관계물질 유전자의 형별분석을 하고 질환과의 연관성를 확인하여 다음과 같은 결과를 얻었으며, 일부는 현재 보고하여 출간 처리 중이다. 1) Tumor necrosis factor-β 유전자의 다형성과 한국인 우울증 사이에는 상관관계가 없었다. 2) 한국인 우울증 환자에서의 tumor necrosis factor-α 유전자형과 대립유전자 분포는 대조군과 비교하여 유의한 차이를 보였다. 3) -G308A TNF-α 유전자 다형성이 한국인 제 1형 양극성장애 환자에서 의의 있는 분포를 보였다. 4) 한국인 우울증 환자에서 MCP-1 유전자형과 대립유전자 분포는 대조군과 비교하여 유의한 차이가 있었다. 5) 한국인 양극성장애 환자에서 MCP-1 유전자형과 대립유전자 분포는 대조군과 비교 시 의의를 발견할 수 없었으나 임상적 이질성과 관련이 있었다. 6) 한국인 우울증 환자에서 cPLA2 유전자형과 대립유전자 분포는 대조군과 비교하여 유의한 차이가 있었다. 7) 한국인 양극성장애 환자에서 cPLA2 유전자형과 대립유전자 분포는 대조군과 비교하여 유의한 차이가 없었다. 이러한 연구 결과들로 볼 때 우울증의 임상양상과 면역학적 특징에 따른 유전적 배경의 상호관련성을 일부 있음을 확인 할 수 있었으나 아직 그 의의는 밝히지 못한 상태이다. 앞으로 이들 연구를 지속함으로써 우울증에서의 면역유전학적 기전을 밝히고, 이 질환의 이질적이며 다양한 임상유형을 생물학적 지표에 근거한 동질적 집단으로 재분류가 가능하게 하여 질환에 대한 이해 수준을 높일 것으로 생각된다. 또한, 우울증의 유전적 병인을 보다 구체적으로 확립하는데 많은 기술적 및 학술적 기여를 할 것이며, 향후 유전자 치료법 혹은 예방 대책의 수립에 귀중한 자료가 될 것으로 생각된다.

Background: Depression is the most common psychiatric disorder which causes social and economical problems. Lifetime prevalence is 3.37% and 20.7% of female and 12% of male have depression in Korea. WHO reported that days of disability caused by depression is longer than that caused by tuberculosis or traffic accident. According to Global Burden of Disease (GBD) study, depression was the 4th leading cause in 1990 and expected to be the 2nd leading cause in 2020. The economic burden of depression in US was estimated to be 43.7 billion dollars in 1990. Of the 1990 total, 12 billion dollars (28%) were direct medical costs, 38.7 billion dollars (55%) were secondary costs such as income loss, unemployment and suicide. Although the relationship between depression and genes involved in the immune system and neurotransmission is highlighted, genetic studies of depression in Korea are scarce and elementary. The first purpose of this project was to investigate the genetic background of the development of depression related to immune function. Secondly, to find out genetic marker in depression, we investigated the relationship between genetic polymorphism and clinical factors. The third was to classify depression into homogeneous group according to the biologic marker. Methods: 1) Subject selection Patients with major depressive disorder according to the criteria of Diagnostic and Statistical Manual for mental disorders (DSM-IV) and Hamilton Depression Rating Scale were included. Controls were age- and sex-matched to patients group and included only when they have no previous or present psychiatric illnesses or family history of psychiatric illnesses. All participants were given explanation of study process and signed informed consent. We built genetic bank that includes blood samples, extracted DNAs, clinical database such as demographic variables, clinical phenomenon and subtypes. And the genetic bank is increasing enormously with the accumulation of databases. 2) HLA typing Class I serology typing was done by using the standard microlymphocytotoxicity method and class II typing was performed by the polymerase chain reaction-sequence specific oligonucleotide probes method. HLA typing for patients with depression have been performing. 3) DNA extraction Lymphocyte was obtained from peripheral blood by Ficoll-Paque solution and DNA was extracted by using Proteinase K. 4) Genotyping ① PCR - Restriction Fragment Length Polymorphism (RFLP) ② PCR - Single Strand Conformation Polymorphism (SSCP) ③ ARMS - PCR (Amplication Resistant Mutation System-PCR) 5) Statistical analysis SPSS 9.0 for WINDOWS was used for the statistical analysis. χ2 test with two-tailed Fisher's exact test was used to analyze the difference in allele and genotype frequencies. Relative risk was estimated as odds' ratio. The probability value (P) less than 0.05 were considered statistically significant. Results and expectations: The results are as follows. Some of them are published already or the others are on the process of publishing. 1) There was no association between polymorphism of tumor necrosis factor-β gene and depression in the Korean population. 2) There was significant differences in the frequency of tumor necrosis factor-α gene alleles and genotypes in the Korean patients with depression. 3) There was significant differences in the distribution of -G308A TNF-α gene polymorphism in the Korean patients with bipolar I disorder. 4) There was significant difference in the frequency of MCP-1 genotypes and alleles in patients with depression. 5) There was no association between the distribution of MCP-1 genotypes and alleles and controls in the Korean patients with bipolar disorder. But the distribution of MCP-1 genotypes and alleles was associated with clinical heterogeneity. 6) There was significant difference in the frequencies of cPLA2 genotypes and alleles in the Korean patients with depression. 7) There was no difference in the frequencies of cPLA2 genotypes and alleles in the Korean patients with bipolar disorder. These results suggest that clinical phenomenology of depression might be related to immunogenetic basis, but the significance of these findings were not elucidated fully. Further studies to verify the immunogenetic mechanism of depression would help to classify depression into more homogeneous group based on biological trait markers and to enhance our understanding of this illness. Also further studies could contribute to the establishment of genetic etiology of depression and the genetic treatment or prevention.
