의약품 안전성 정보는 ‘약물감시’ 활동을 통해 수집된 모든 정보로, 쉽게 설명하면 의약품의 부작용(side effect)과 관련된 모든 정보를 말한다. 의약품 규제기관은 약물역학 연구 수행·평가를 통해 이들을 실마리(signal) 또는 위해(risk)로 구분하고 각 특성에 따라 적절한 조치를 시행해오고 있었다. 약물역학은, 인구집단에서 약물의 사용으로 인하여 발생하는 이롭거나 해로운 결과의 빈도를 파악하고, 특정 약물복용과 질병 발생 간의 관련성을 분석하는데 역학적 지식과 연구방법론을 적용하는 학문으로, 실제 실마리 탐색에서부터 중요한 실마리로부터 새로운 위해성을 탐색·확인하는 과정에 여러 약물역학적 연구 기법들이 활용되고 있었다. 의약품 안전성 정보를 생성시키는 약물역학 연구 자료원으로는 (자발적) 부작용 보고자료, 레지스트리(registry)라고 불리는 약물투여(등록) 자료, 대규모 보건의료 데이터베이스 자료(건강보험 청구자료, 전자의무기록 및 암사망자료 등의 기타 생물정보 통계 등) 및 임상시험 자료가 있었다.
그동안은 상대적으로 단시간에 저비용으로 손쉽게 다양한 정보를 얻을 수 있는 일련의 부작용 보고자료 사례 및 이의 DB를 활용하여 실마리를 생성시켜왔다. 그러나 자료의 한계성으로 인하여 false positive 또는 false negative가 일정 부분 존재하며, 좀 더 신속하게 새로운 위해성을 확인할 수 있도록 국가 차원에서 적극적으로 약물감시를 자동적으로 수행할 수 있도록 시스템을 개선시키고자 하는 약물역학연구들이 활발해지고 있다. 이것이 가능하려면 대규모 보건의료정보 데이터베이스를 활용할 수 있어야 하는데, 이를 위해 미국 NIH는 OMOP project, FDA는 sentinel initiative, 유럽연합 EMA는 Euro-ADR project, 일본 PMDA는 MIHARI project 등을 진행하여 신뢰할 수 있는 안정적인 보건의료 정보 데이터베이스를 구축하고 여기에 의약품으로 인한 새로운 위해(부작용)을 스크리닝 할 수 있는 쿼리(query) 등의 프로토콜을 개발하는 작업들을 중장기 계획으로 진행하고 있었다.
우리나라에서도 실마리 탐색, 가설 검증 및 위해 확인을 위한 약물역학연구를 수행할 수 있는 자료원을 조사한 결과, 의약품 유해사례 보고자료 DB, 백신 투약 및 이상반응 보고자료 DB, 암등록자료 및 건강보험 청구자료 데이터베이스 등을 사용하여 실마리를 탐색하거나 검증 또는 위해성을 확인할 수 있었다. 국내에 약물감시 체계가 본격적으로 가동되게 된 것은 2000년 이후로, 연간 부작용 보고자료가 수천~수만건으로 어느 정도 활용성이 있게 된 것은 최근 몇 년 사이의 일이며 현재까지 국내에서 수행된 약물역학 연구 결과의 활용성과 파급도를 분석한 결과, 이미 안전성 문제로 시장에서 퇴출된 약물을 사용해 특정 모델/알고리즘의 적용성을 테스트하는 탐색적 연구가 대부분이었으며 새로운 안전성 이슈를 찾거나 추가 역학 연구를 촉발한 경우는 없는 등 향후 이 부분에 대한 지속적 관심과 인프라 강화에 따른 내실화가 필요할 것으로 판단되었다. 이를 토대로 국내 약물역학 연구 수행·평가에 대한 SWOT 분석을 수행한 후 KFDA/NIFDS의 안전관리 역량 강화를 위한 향후 10년간의 약물역학연구 action plan을 제안하였다.
ICH의 E 부문 가이드라인 중 약물감시에 해당되는 E2A부터 F까지를 비롯 2012년 EU EMA가 전세계에서 가장 먼저 선보인 ‘우수약물감시기준(GVP; Good pharmacoVigilance Practice)’ 등의 의약품 안전성 정보 수집·분석·처리·관리와 관련된 외국의 최신 규정·제도·지침을 분석하여 국내에서의 의약품안전성 정보 관리 방안을 마련하였고, 세부적으로는 실마리 정보 구분·평가 및 과학적 타당성 확인 프로세스 중 약물역학연구 수행에 대한 세부 평가기준을 개발·제안하였다. 또한 의약품 안전성 정보의 수집·생산·평가를 전담하기 위하여 설립되어 2012년 4월부터 운영되기 시작한 한국의약품안전관리원(KIDS; Korea Institute of Drug Safety and Risk Management)에 의약품 위해 평가 관련 기술지원을 제공하였고, AHC(아시아-태평양권역규제조화센터) 약물감시 워크숍에서는 제약회사 등을 상대로 국제적으로 점점 강화 추세에 있는 약물감시 최신 동향 정보를 제공하였다. 이 결과는 식약청/평가원의 과학적 위해 평가 역량 및 제약회사의 국제 경쟁력 강화에 기여할 것으로 판단된다.

Although pharmaceutical products can only be introduced to the market after passing stringent quality, safety, and efficacy tests, there are limitations in terms of the number of study participants and the study period. Since most pharmaceutical products are administered to a significant number of patients in various circumstances only after being commercially available, the amount of safety information about a specific product increases exponentially after being sold in the market, varying from adverse event reporting to phase 4 clinical test results. Therefore, regulating agencies must quickly identify and confirm new risk factors from available information and perform scientific evaluation to ensure safety. Promotion of safety of pharmaceutical products after commercial release has rapidly evolved from the 1990s through 2000s. There have been efforts to survey the systems for collecting, analyzing, and evaluating pharmaceutical safety data from international organizations such as ICH and WHO and regulating agencies such as FDA and EMA, and to develop means for managing safety information and specific risk evaluation processes based on the results of the surveys.
Pharmaceutical safety information refers to all of the data collected from pharmacovigilance activities, or simply all of the information related to side effects caused by pharmaceuticals. Regulating agencies have assessed pharmacoepidemiology studies to classify the information into signals or risk and implemented appropriate measures according to the characteristics.
Pharmacoepidemiology is the study of understanding frequency of beneficial or harmful results caused the use of drugs in large numbers of people and application epidemiological knowledge and study methodology for analysis of relation between taking specific pharmaceutical products and incidence of disease. A number of research techniques of pharmacoepidemiology have been deployed in the process of investigating signals and identifying new risk factors from the signals.
Pharmacoepidemiology resources for generating safety information have included (spontaneous) side effect reports, drug administration and registration data(referred to as the registry), large scale health and medical database(biomaterial statistics as HIRA;Health Insurance Review & Assessment service, EMR; Electronic Medical Record, and KNCI; Korea National Cancer Incidence), and clinical trial data.
Traditionally, signals have been generated using low-cost means for easily accessing various types of information in a relatively short period of time, such as series of case reports of side effects and their databases. However, there have been a fair amount of false positive and false negative cases due to limited resources. In order to expedite the process of identifying new risks, there are active pharmacoepidemiology studies being conducted on a national scale to improve the systems for automatically executing pharmaceutical monitoring. Access to large scale health and medical databases are necessary for such systems to be feasible. NIH and FDA in the U.S., EMA in EU, and PMDA in Japan have carried out OMOP project, Sentinel Initiative, Euro-ADR project, and MIHARI project, respectively, to construct reliable and stable health and medical databases, and mid and long term plans are being executed to develop protocols(such as query) capable of screening new risks caused by pharmaceuticals. A survey of information centers in Korea capable of conducting pharmacoepidemiology studies for detection signal, verifying hypotheses, and identifying risk factors indicates that signals are being investigated or verified, and risks are being identified using databases containing adverse drug reaction case reports, vaccine administration, abnormal reaction reports, HIRA, and KNCI DB. Pharmaceutical monitoring systems have taken full effects in Korea after 2000, and it has only been a few years since the number of side effect reports submitted has increased to tens of thousands to be of significant use. Analysis of the levels of utility and dissemination of the results of pharmacoepidemiology studies conducted in Korea shows that most were investigative research involving applicability tests of specific models/algorithms using pharmaceuticals that had already been removed from the market due to safety issues. There were no cases that prompted identification of new safety issue or subsequent epidemiology research, and more attention needs to be paid in this regard with sustained interest and by improving the infrastructure. Based on the findings, SWOT analysis was performed on the performance of pharmacoepidemiology studies in Korea and an action plan for the next decade has been proposed to strengthen the capabilities for drug safety management of KFDA/NIFDS.
Among ICH’s Section E guideline, parts that correspond to pharmacovigilance (from E2A to F) were analyzed, as well as the latest regulations, policies, and guidances in other countries related to collecting, analyzing, processing, and managing drug safety information, including GVP(Good pharmacoVigilance Practice), which was first introduced in the world by EMA, to prepare『the Korean drug safety information management practices』. Furthermore, technical support related to pharmaceutical risk assessment has been provided to KIDS(Korea Institute of Drug Safety and Risk Management), which has been in operation since its foundation in April 2012 and recent trend information of pharamcovigilance has been provided to pharmaceutical companies in the AHC(APEC Harmonization Center) pharmacovigilance workshop. These results are expected to make significant contributions in scientific risk assessment capability of KFDA/NIFDS and in enhancing global competitiveness of Korean pharmaceutical companies.
