Ⅳ. Study results
○ Estimate future supply ◦ of public health doctors
- As of 2008, the number of public health doctors totaled 5,040 persons, including 3,219 doctors, 915 dentists, and 906 Oriental medicine doctors. The establishment of graduate schools of medicine and dentistry, and a rise in the percentage of female students have led to the steady reduction in the number of public health doctors. It is forecast that the number of public health doctors will be merely 1,905 in 2022, and the ratio among doctors, dentists, and Oriental medicine doctors will also change: Doctors will account for 38%, dentists 12%, and Oriental medicine doctors 50%.
○ Define vulnerable areas in terms of health and medical care
- By referencing indexes used in foreign nations to choose vulnerable areas and through meetings with expert advisors, the following were defined as indexes to choose vulnerable areas in terms of health and medical care: economic factor, vulnerable population factor, health and medical care resource factor, and geographical proximity factor.
The following provides detailed information on the indexes:
① Economic factor: Fiscal independence
② Vulnerable population factor: Ratio of senior population aged 65 or older, ratio of people registered as being disabled, ratio of those receiving assistance under the National Basic Living Security Act
③ Health and medical care resource factor: Number of medical institutes (clinics, hospitals, general hospitals), number of doctors per 10 thousand persons
④ Geographical proximity factor: Number of islands and remote places that offer a poor level of accessibility to an emergency center
- The vulnerable area indexes were calculated for 249 cities, districts, and counties, and 3,555 eup, myeon, and dong across the nation. Of a total of 100 points, the vulnerable area index average for cities, districts, and counties was 40.2 points. The special city type was the lowest at an average of 15.6 points, and the district type was the highest at 66.4 points. The vulnerable area index average for eup, myeon, and dong around the nation was 39.6 points: The average for dong areas was 26.3 points, for eup areas 47.2 points, and the myeon area the highest with 62.3 points.
○ Draw up (draft) step-by-step priority-based disposition standards for public health doctors
- The objective of disposition of public health doctors against the backdrop of the estimated reduction in the supply of public health doctors is defined as "providing essential health and medical care services to establish a safety network for medical care in vulnerable communities."
- Three basic assumptions were established in terms of disposition of public health doctors, based on which a framework was developed on priority-based public health doctor disposition standards.
① Priority should be placed on dispatching public health doctors to vulnerable health and medical care areas that experience
difficulties in securing medical care personnel
② Priority should be placed on dispatching public health doctors to public organizations that work for public interests
③ Priority should be placed on dispatching public health doctors to organi- tions that need the professional skills of doctors
- Organizations where public health doctors are stationed were reclassified into 11 types (public health center, public health center and county hospital, branches of public health clinics, hospital ships and provincial mobile medical treatment teams, national and public hospitals, private hospitals, organizations related to the Ministry for Health, Welfare and Family Affairs, social welfare facilities, health-related groups, correctional institutions, and other central government-related organizations)
- The reduction in the number of public health doctors means that there is a need to gradually exclude organizations to which the doctors are dispatched starting from 2012, beginning with organizations that have a low priority. There is a need to start with "other central government-related organiza- ons" with a low level of vulnerability, public nature, and demand for expertise, health-related groups, organizations related to the Ministry for Health, Welfare and Family Affairs, and private hospitals located in general cities.
- After a step-by-step elimination of organizations on the list of organizations to which doctors are dispatched, there will be organizations that will essentially need to remain on the list: The public health clinic, which has the highest level of vulnerability, public nature, and demand for expertise (district-type public health center, vulnerable urban-rural integration city-type public health clinics), public health and county hospital, branches of public health clinics (branches located within 15% of vulnerable areas), hospital ships and provincial mobile medical treatment teams, and national and public hospitals (25% of vulnerable areas). The number of essential public health doctors (excluding dentists and Oriental medicine doctors) is 1,110 persons. It is forecast that there will be a shortage of around 290 doctors, based on supply estimations. There is therefore a need to draw up measures to address this issue.
○ the appropriateness of disposition of public health doctors
- There is a need to engage in the appropriate disposition of the limited number of public health doctors and maximize efficiency in personnel utilization, in accordance with the forecasted reduction in the number of public health doctors.
- As such, there is a need to establish an objective disposition process for public health doctors by evaluating the appropriateness of disposition of public health doctors, and to facilitate efficient management of public health doctors through the appropriate use of evaluation results.
- The evaluation on the appropriateness of disposition of public health doctors should be carried out in three areas: Input (evaluate the feasibility of disposition of public health doctors), process (evaluate the feasibility of utilization of public health doctors), and result (evaluate the performance of utilization of public health doctors).
- The evaluation method should involve receiving a "(tentatively named) work performance plan of public health doctors" from organizations where public health doctors are stationed, an initial evaluation by the corresponding city and province, a final evaluation by the central government, and a field evaluation, if needed.
- Establish a reasonable framework for disposition of public health doctors by incorporating evaluation results on the disposition of public health doctors.
