문헌연구 및 사례조사 결과를 바탕으로 노인장기요양보험에 적용할 수있는 방문재활서비스 모델을 제시하면 다음과 같다.

셋째, 서비스 제공 방식: 노인장기요양보험제도 내에서 방문재활서비스를 재가급여의 항목으로 도입하며, 일본과 마찬가지로 방문형 급여로서 방문재활서비스, 통원형 급여로서 주야간 보호 기관에서의 재활서비스,퇴원/퇴소후 집중재활 서비스 형태로 제공한다. 아울러 시군구에서 제공하는 장기요양 비인정자인 등급외자를 대상으로 한 보건소 맞춤형 방문건강관리사업을 확대 실시한다.
넷째, 기본적으로 방문재활에 있어서 의사, 간호사, 물리치료사, 작업치료사 등 전문치료인력의 팀접근을 기본으로 하며, 서비스 제공인력: 방문재활서비스 제도 도입 초기에는 물리치료사와 작업치료사가 참여하고 향후 서비스가 확대되면 언어치료사 등 타 직종 치료사까지 참여할 수 있다.
다섯째, 성과측정: 신체기능의 회복 및 향상을 평가할 수 있는 성과지표가 마련되어야 하며, 성과지표는 De Morton Mobility Index (DEMMI)와 같이 실제적으로 노인의 신체활동 동작의 기능 개선을 평가할 수 있고 변화에 대한 민감도가 높은 평가도구를 선정한다.

The home-based rehabilitation system should be included in long-term care and health insurance in the public sector. The model that would be applicable to Korea is as follows. First, the primary targets for the home-based rehabilitation service would have to be people who qualify for the third grade within the Korean long-term care system, or who are not in the system at all. The focus here is on a preventive approach to improve elderly people’s functioning status and to keep them living independently, as well as preventing those with mild disabilities from entering a higher grade within the system.
Second, the priority of home-based rehabilitation therapy is motor function, not pain, or cognitive impairment. Motor functions are more related to daily living and can be objectively assessed. Rehabilitation programs include physical rehabilitation, such as basic rehabilitation, physical training in daily living activities, and training with devices needed for the rehabilitation.
Third, home-based rehabilitation should be introduced to the long-term care system, and it can be delivered during home visits, or in conjunction with outpatient treatment or rehabilitation that is more intensive. In addition, a home-based rehabilitation system in health insurance and in the public sector should be operated in such a way as to achieve maximum continuity of care.
Fourth, a multidisciplinary team approach in care and service is the basis for all rehabilitation treatment. The team most often consists of a rehabilitation physician, a rehabilitation nurse, a physical therapist, an occupational therapist, and a clinical social worker.
Fifth, the De Morton Mobility Index (DEMMI), is an outcome measure that is recommended in order to evaluate improvement in physical functioning. It can be administered to the patients both inside and outside of the community setting, and has displayed great responsiveness to change and a minimally clinically important difference.
Home-based rehabilitation services within long-term care system have to be expanded in phases. The first step is to test this model within a regional rehabilitation hospital with a central role. The second step is to provide mild and motor-functional-focused rehabilitation services, and the third step is to develop comprehensive rehabilitation services that target the long-term care system.
