1. 교육 프로그램 실태조사
1)국내 호스피스 기관과 봉사자 교육 프로그램 운영 현황
국내 호스피스 기관은 113개 기관이 확인되었고 서울, 부산, 경기도 등이 전체의 50% 로서 특정지역에 편중되어있다. 운영형태는 산재형, 가정형 등이많았다. 기관현황 조사결과 가정 호스피스 대상자 평균은 28명, 병동형 평균병상수는 18병상이었으며, 의사 수 평균2.5명, 간호사수는 5.7명, 봉사자수는 67명 순이었다. 그러나 대부분의 기관이 병상이 없었고 호스피스팀원도 고루 갖추어있지 못하였다. 113개 기관

1. 교육 프로그램 실태조사
1)국내 호스피스 기관과 봉사자 교육 프로그램 운영 현황
국내 호스피스 기관은 113개 기관이 확인되었고 서울, 부산, 경기도 등이 전체의 50% 로서 특정지역에 편중되어있다. 운영형태는 산재형, 가정형 등이많았다. 기관현황 조사결과 가정 호스피스 대상자 평균은 28명, 병동형 평균병상수는 18병상이었으며, 의사 수 평균2.5명, 간호사수는 5.7명, 봉사자수는 67명 순이었다. 그러나 대부분의 기관이 병상이 없었고 호스피스팀원도 고루 갖추어있지 못하였다. 113개 기관 중 호스피스 교육을 시행하는 기관이 77기관이었고 자격 제한이 교인, 간병인 또는 연령, 학력 등이었으며, 특별한 제한이 없는 경우가 많았다 특정 기관인 경우 일정 자원 봉사활동에 기준을 두는 경우도 있었다. 교육 프로그램운영을 보면 강의 20시간〜30시간, 실습 20기간〜60시간 정도였으며, 강의운영형태는 1〜3일 단기 교육에서 10일까지였고 장기교육은 3개월〜6개월 등의 형태였다. 강의 내용은 호스피스 개요, 암환자의 신체간호, 사별간호, 의사소통, 자원봉사자 자세, 통증관리, 영적간호,심리간호, 죽음이해, 암 발생 원인, 임종과 죽음, 실습, 식이요법, 스트레스 관리, 장례 유서쓰기, 대체요법, 호스피스 현황 및 정책 등이었다.
2) 국외 호스피스 기관의 자원봉사자 교육 프로그램
일본 호스피스 완화의료제도와 현황, 호스피스 교육프로그램 등을 조사하였다. 일본은 기본교육, 심화교육, 연수프로그램, 사별가족 프로그램 등으로 분리 되어있고, 기본교육 이수 10시간, 심화교육 1년을 기간으로 시행하고 있었다. 대만의 경우 6개월을 주기로 년 2회 시행하며 한강좌당 2〜6번의 교육을 실시하였다. 미국의 호스피스 교육은 기본교육 20시간과 심화교육으로 나뉘며 심화교육은 사별간호, 아동호스피스, 영적간호, 대체요법 등을 첨부하였다. 영국의 경우 토론을 주제로 1년 내내 교육과 실습 등을 실시하여 지속적 교육이 이루어짐을 알 수 있었다.
3) 국․내의 호스피스 자원봉사자 교육프로그램의 비교
공통적인 교육내용은 호스피스 개요, 자원봉사자 자세, 임종과 죽음, 신체,정서 케어, 자원봉사체험 등이었고, 한국과 미국은 의사소통, 통증관리, 영적간호 등이 포함되어있었으며, 영국은 가족간호, 안락사, 대만은 영적간호, 팀워크 등 차이가 있었다. 교육프로그램 운영형태는 국내는 단시간 내에 교육이 시행되나, 외국의 경우 장기간 꾸준히 교육이 이행되고 있었고, 실습이 항상 포함되어 있다는 점이 차이가 있었다. 또한 국내는 교육의 통일성이 없었고 표준화되어있지 않았으나, 국외는 모든 교육프로그램이 표준화 되어있고 체계적인 운영을 하고 있었다. 이는 국내 호스피스 자원봉사자의 교육이 사명감이나 자세를 갖추게 하는데 문제가 있다고 보이며 차후 보완되어야 할부분이다. 자원봉사자의 연수, 캠프, 토론회 등에 있어서 국외에서는 다양한 프로그램에 포함되어 있었으나 국내는 그렇지 못한 점도 앞으로 교육내용에 참고해야 할 부분이다.
2. 대상자의 교육 요구도
1) 호스피스 자원봉사자의 만족도와 교육 요구도
호스피스 자원봉사자의 주요활동은 신체․정서적 돌봄에 치중해 있었고 사회적 돌봄, 사별가족 등의 활동에 대해서는 인식이 부족했다. 교육프로그램의 만족도는 전 강의에서 높은 편이었으며 교육 요구도는 신체적 돌봄의 통증,안위 및 체위변경요구, 정서적 돌봄에서는 경험, 지지 등의 의사소통 기술에서 사회적 돌봄은 임종관련 정보와 장례절차 등의 교육 요구가 높았다.
2) 호스피스 환자와 가족의 호스피스 요구 및 만족도
호스피스 환자와 가족은 주1회 이상 호스피스 방문 간호 및 돌봄을 받기 원하였고 방문서비스 만족도도 매우 높았다. 호스피스 환자와 가족은 통증완화, 영양문제, 신체증상완화, 품위 있는 죽음준비, 예배 및 기도, 현실에 대한이해 환자가 원하는 일을 , 하도록 돕고, 함께 대화하고 지지해주며, 환자가 원하는 사람을 만나도록 돕고, 임종시 필요한 정보, 장례절차를 도우며, 가족대신 환자를 돌봐주고, 슬픔과 소진에 대처하도록 가족을 도우며, 사별가족의 슬픔을 지지하고, 의료비, 경제적 문제를 의논해 주기를 원하였다. 이와 같은 결과를 토대로 하여 자원봉사자의 교육교재에 포함, 개발됨이 바람직하다고 사료된다.
3. 표준화된 호스피스 자원봉사자 교육프로그램 개발
1) 교육내용 개발
강의 내용은 1차 교육(기본), 2차 교육(심화) 등으로 나뉘며 1차 교육은 20시간의 강의와 20시간의 실습으로 구성 개발하였다. 호스피스 자원봉사자의 1차 교육프로그램은 호스피스 개요(1시간), 암에 대한 이해(2시간), 말기 암환자의 신체적 간호(2시간), 심리간호(1.5시간), 영적간호(1.5시간), 죽음에 대한고찰(1.5시간), 임종간호(1,5시간), 의사소통(1.5시간), 노인호스피스(2시간), 사별간호(1시간), 호스피스 현황과 정책(1시간), 자원봉사자 자세 및 역할(1시간), 스트레스 관리(1.5시간), 사례 발표(1시간) 으로 14항목이었다. 호스피스 자원봉사자 2차 교육은 보완대체요법, 감염관리, 식이요법, 림프마사지, 생명윤리와 법, 아동호스피스, 장례절차, 유서쓰기 등 항목으로 분류하였다.
2) 위의 교육 프로그램을 토대로 호스피스 자원봉사자 교육교재를 개발하였다(별책: 호스피스 자원봉사자 교육교재).
3) 호스피스 네트워크 구축
호스피스 각 기관과 자원봉사자 네트워크 구축과 교육프로그램 교류를 위하여 홈페이지를 개발하였다.

1) Management of Hospice Organizations and Hospice Volunteer Training Programs in Korea.
There are 113 hospice organizations in Korea and about 50% of them are concentrated in Seoul, Busan, and Gyeonggi-do areas. Most of them were managed as industrial foundations or family-owned organizations.In result of a survey, family-owned hospice organizations could handle up to 28 patients and hospital-like organizations had an average of 18 beds, 2.5 doctors, 5.7 nurses, and 67 volunteers. However, most organizations did not have beds or hospice staffs. Among the 113organizations, 77 provided hospice training and required candidates to be religious volunteers or private care-givers. Candidates were sometimes restricted by age and/or education. Some organizations required a certain level of volunteer experiences. Training programs included 20-30 hours of lectures and 20-60 hours of practical training.Lectures varied from 1- to 10-day short-term curriculums to 3- to 6-month long-term curriculums.
Lectures dealt with overview of hospice service, physical nursing, coping with grief, communication, duties of volunteers, pain management, spiritual nursing, psychological nursing,understanding the death, cause of cancer, deathbed and death, practice,diet, stress management, funeral, writing wills, replacement therapy, and hospice policies.
2) Hospice Volunteer Training Programs Abroad
This study examined Japan’s hospice and relief system and hospice training programs. Japan divided up their hospice programs into basic program, intensive program, training program, and coping with grief programs and required 10-hour basic program or 1-year intensive program. In case of Taiwan, programs are offered twice a year and each course includes 2-6 sessions. In the U.S., hospice training consists of 20-hour basic training and intensive training, which includes coping with grief nursing, children’s hospice, spiritual nursing, and replacement therapy. In England, discussion-based training and practice are offered throughout the year.
3) Comparative Analysis of Hospice Volunteer Training Programs
Programs commonly include overview of hospice, duties of volunteers, deathbed and death, physical and emotional care, and volunteer experiences. Korea and the U.S. include courses on communication, pain management, and spiritual nursing, whereas England provides courses on family nursing euthanasia and Taiwan provides courses on spiritual nursing and teamwork. Programs in Korea are generally short-term, but programs abroad are long-term and sustainable and always include practices. Korea’s training is not unified or standardized, but programs abroad are standardized and systemized.Hospice programs in Korea shall be improved in order to foster volunteers’ sense of responsibility and awareness. Programs overseas included various camps and discussion sessions for volunteers, and Korea’s programs shall adopt such systems from them in the future.
2. Training Needs of Volunteers
1) Satisfaction and Training Needs of Hospice Volunteers
Hospice volunteers’ major activities are concentrated on physical and emotional care and lack awareness of social care and families’coping with grief. Satisfaction with training programs were relatively high and training needs included physical care, such as pain, wellness, and help with movements, emotional or communicative care, such as experience and support, and social care, such as deathbed and funeral.
2) Needs and Satisfaction of Patients and Their Patients
Hospice patients and families require at least once a week hospice visits and care and are highly satisfied with visiting services. Patients and families want hospice volunteers to help them relieve pain and physical symptoms, have better nutrition, prepare for calm death, participate in services and praying, understand the reality, do what they want to do, make supportive conversations, meet people they want,provide information for deathbed and funeral, and take a good care of them like family. They also want hospice volunteers to help the families cope with grief and handle medical bills and other financial issues. In this respect, it is necessary for training materials for volunteers to include all of these issues.
3. Development of a Standardized Hospice Volunteer Training Program
1) Standardized Training Material and Curriculum
The newly developed training material consists of primary training (basic) and secondary training (intensive). Primary training consists of 20hours of lecture and 20 hours of practice. In detail, the program includes 14 categories: overview of hospice (1hour), understanding cancer (2hours), physical nursing of cancer patients (2hours),psychological nursing (1.5hours), spiritual nursing (1.5hours),contemplation of death (1.5hours), deathbed (1.5hours), communication(1.5hours),senior hospice (2hours), coping with grief (1hour), hospice policies (1hour), duties and roles of volunteers (1hour), stress management (1.5hours), and case studies (1hour). Secondary training was classified into replacement therapy, infection management, diet, lymph massage, life ethics and law, children’s hospice, funeral process,and writing wills.
2) This study developed a new hospice volunteer training material based on the above program.
3) Homepage and Network for Hospice Volunteers
Homepage was developed for the network of hospice organizations and volunteers and their interaction with training programs.
