본 연구에서는 치매 환자를 대상으로 한 국내외 비교대조임상시험연구에 대한 메타분석 및 체계적 고찰을 통해 인지훈련, 인지 자극, 신체활동, 회상요법, 현실감각훈련, 음악치료로 이루어진 지역사회 거주 경증치매 환자를 위한 복합인지재활프로그램(Multimodal Cognitive Enhancement Therapy: MCET)을 개발하고(1차년도), 18개월 간의 다기관, 무작위배정, 대조군, 이중맹검, 교차 임상시험을 통해 MCET의 효과를 과학적으로 검증함(2, 3차년도). 총 4개 병원(분당서울대학교병원, 건국대학교병원, 경기도립
본 연구에서는 치매 환자를 대상으로 한 국내외 비교대조임상시험연구에 대한 메타분석 및 체계적 고찰을 통해 인지훈련, 인지 자극, 신체활동, 회상요법, 현실감각훈련, 음악치료로 이루어진 지역사회 거주 경증치매 환자를 위한 복합인지재활프로그램(Multimodal Cognitive Enhancement Therapy: MCET)을 개발하고(1차년도), 18개월 간의 다기관, 무작위배정, 대조군, 이중맹검, 교차 임상시험을 통해 MCET의 효과를 과학적으로 검증함(2, 3차년도). 총 4개 병원(분당서울대학교병원, 건국대학교병원, 경기도립노인전문병원, 전주노인복지병원)에서 등록된 지역사회에 거주하는 경도인지장애 및 경증치매 환자 64명이 본 임상시험에 참여하여 55명이 완료하여 연구 계획 대비 우수한 완료율과 치료 순응도를 보였음. 임상시험 결과에 대한intention-to-treat 분석에서 MCET은 대조요법(MCET과 동일 시간 동안 제공된 수동적 인지자극과 통상적 사회교류활동)에 비해 1차 유효성 평가 변수 (전반적 인지 기능)인 Mini-mental status examination(MMSE) 및 Alzheimer’ s Disease Assessment Scale-Cognitive Subscale(ADAS-Cog)에서 우수한 효과를 보였고, 2차 유효성 평가 변수인 Revised Memory and Behavior Problems Checklist(정신행동증상 심각도)와 Quality of Life-Alzheimer's Disease(치매 환자의 삶의 질) 개선에도 우수한 효과를 보여, MCET이 통상적인 그룹활동에 비해 지역사회 거주 경도인지장애 및 경증치매환자의 인지기능, 문제행동 및 삶의 질 개선에 효과적임을 증명함. 1차 및 2차년도에 진행된 공립요양병원 현황조사 결과, 지역사회 경증치매 환자에게 인지재활서비스를 제공하는 기관은 전무하였으며, 입원 치매 환자를 대상으로 시행하고 있는 인지재활요법은 과학적 근거와 효과보다는 시행 편의성 중심으로 진행되고 있는 곳이 많았음. 3차년도에는 MCET에 대한 표준매뉴얼을 개발하고, 공립요양병원(특히 치매거점병원으로 지정된 공립요양병원)을 주 서비스 제공 경로로 지역사회 경증치매환자에게 MCET을 제공하도록 하는 사업모델을 설계함. 사업은 총 4단계로 설계 되어 100명의 MCET 치료사 양성과 100개의 MCET서비스 제공 요양병원(70개 국공립요양병원, 30개 사립요양병원) 구축을 3년에 걸쳐 진행하도록 설계되었으며, 사업비는 89억 2,385만원으로 추산하였음. 아울러 공립요양병원 중심의 본 사업 모델 시행 성과를 기반으로 서비스 공급자를 보건소와 주/야간보호센터 등으로 확장하는 중장기 발전 방안에 대한 추가 연구가 필요할 것으로 판단됨.

The current study developed a Multimodal Cognitive Enhancement Therapy (MCET), which aims at community-dwelling early-stage dementia patients and consists of cognitive training, cognitive stimulation, physical therapy, reminiscence therapy, reality orientation, and music therapy. It was developed based on a meta-analysis and systematic review of controlled clinical trials, and its effectiveness was verified through an 18-month multicenter, randomized, placebo-controlled, double-blind, cross-over clinical trial. Sixty-four mild cognitive impairment(MCI) and early-stage dementia patients dwelling in communities registered by 4 hospitals participated in the clinical trial, with 55 of those participants showing high completion rates and adherence to treatment.
Primary outcome variables were Mini-mental status examination and Alzheimer’ s Disease Assessment Scale-Cognitive Subscale and secondary outcome measures were Revised Memory and Behavior Problems Checklist and Quality of Life-Alzheimer’ s Disease. Analyses of intention-to-treat from the clinical trial results showed that MCET was superior to mock therapy in both primary and secondary outcomes, demonstrating that MCET is more effective than regular social activities in the improvement of cognition, problematic behavior, and quality of life of community-dwelling MCI and early-stage dementia patients.
In the first and second year, an investigation of the public geriatric longterm care hospitals found that there are no institutions providing cognitive rehabilitation services for community-dwelling early-stage dementia patients. Many of the currently existing cognitive rehabilitation therapies for hospitalized dementia patients were based more on ease of administration rather than on scientifically proven effectiveness or evidence. In the third year, we developed a standard manual for MCET and a business model in which public geriatric longterm care hospitals (especially those designated as regional base dementia hospital) would serve as the main medium through which MCET would be provided to mild dementia patients in the community. The business consists of 4 phases and was designed to produce 100 MCET therapists and 100 MCET-serving geriatric longterm care hospitals (70 public, 30 private) within 3 years, with an estimated cost of 8,923,850,000 won. It is speculated that further research, based on the success of the current geriatric longterm care hospital-centered business model, should investigate ways to expand service providers to include health care centers and senior centers as well.
