우리나라 민간차원 실무표준화 프로그램으로 우수검사실 신임인증과 외부신빙도조사가 있음. 본 연구에서는 외부신빙도조사와 우수검사실 신임인증을 동시에 받는 검사실을 ‘높은수준 표준화’로 분류하고, 외부신빙도조사에만 참여하는 검사실을 ‘기본수준 표준화’, 모두 참여하지 않는 검사실을 ‘미표준화’로 구분하였음.
실무 표준화가 검사결과의 정확성에 주는 효과는 우리나라 외부신빙도 조사에 참여하는 약 1300여 기관의 임상화학분과, 진단혈액분과 그리고 알파태아단백에 대한 결과를 이용하여 분석하고자 하였음. 임상화학분과의 2010년부터 2013년까지의 19개 항목에 대한 자료의 분석 결과 우수검사실 인증 기관이 그렇지 않는 기관에 비하여 우수한 VIS를 보였음. 진단혈액분과의 9개년도 SDI 자료 분석에서도 유사한 결과 관찰됨. 기관들을 종별로 구분하여 기관 규모에 따른 차이를 배제한 분석에서 우수검사실 인증을 받은 종합병원이 그렇지 않은 기관의 결과 보다 우수하였음. 의원급과 병원급 전체 기관 분석에서는 병원급에 비하여 의원급의 결과가 우수하였으나, 의원급 기관 중 전문수탁의원을 제외한 분석에서는 병원급이 의원급 보다 우수하였음. 임상화학분과 항목은 일관되게 우수검사실 인증기관의 성적이 우수하였으나, 진단혈액분과의 결과는 일관된 연관이 관찰되지 않았으며, 이는 임상화학 검사 항목들이 진단혈액검사에 비하여 구조와 과정영역의 표준화 수준에 영향을 크게 받기 때문으로 추정됨.
진단 판별치(cut-off)가 존재하는 검사 결과의 계통적 치우침은 진단의 위양성과 위음성을 유발함. 혈당(glucose)의 외부신빙도조사 결과 분석에서 우수검사실 인증기관과 나머지 기관은 통계적으로 의미있는 차이를 보였으며, ‘2013년 국민건강영양조사’ 자료를 토대로 시뮬레이션한 성인(30세 이상) 당뇨병 유병률의 변화를 보였음. 2013년 당뇨병 유병률인 11.9%가 참값이라 가정하였을 때, 위양성 방향의 계통적 오차를 가정하면 우수검사실 인증기관 유병률은 12.3%, 나머지 기관 12.4%, 미표준화 13%(미표준화 1은 12.9%, 미표준화 2는 13.1%)선으로 상승하였음. 우리나라의 혈당검사가 우수검사실기관과 비우수검사실기관에서 각각 수행하는 비율을 조정하여 시뮬레이션한 결과 우수:비우수 60%:40%의 배분을 중심으로 당뇨병 유병률의 변화 폭은 크지 않았음. 표준화가 국가 유병률 자료에 영향을 미칠 수 있음을 자료로 제시한 첫 번째 시도라 생각됨.
검사 표준화 여부에 따른 유병률의 변화가 당뇨병 검진의 비용-효과에 미치는 영향을 당뇨병검진 비용-효과 분석 연구의 Markov 모형을 적용하여 분석하였음. 높은 수준의 표준화 검사실의 유병률을 참값으로 가정하고, 기본수준 표준화와 미표준화 기관에서 산출된 위양성과 위음성에 의해 변화되는 유병률로 인한 비용-효과를 비교하였음. VIS 차이를 위양성으로 적용할 경우 40대 이상 모든 연령에서 검사실의 표준화가 달성될수록 비용이 적게 지출되며 표준화 우수 검사실의 경우 다른 검사실에 비해 40대 11.9원, 50대 17.1원, 60대 19.1원까지 1인당 비용이 절감되는 것으로 분석되었음. 위음성으로 적용할 경우 표준화가 우수하게 달성되는 경우 그렇지 않은 경우에 비해 비용도 절감될 뿐 아니라 효과(QALY)도 우수한 것으로 확인되었으며, 이는 연령이 증가함에 따라 효과가 증가하는 경향을 보였음. 30대에서는 미표준화 검사실에 비해 표준화 검사실에서 1인당 평생 약 1만3천원의 비용절감 및 0.0012 QALY 증가하며 50대에서는 1인당 평생 최대 약21만3천원, 60대에서는 약16만7천원의 비용절감이 발생하였고 효과는 50대와 60대 모두 0.02 QALY가 증가하였음.
2007년부터 2010년까지의 국가대장암검진결과 자료의 분석에서, 우수검사실과 비우수검사실의 대장암검진 양성률은 각각 2.6%와 7.0%, 위양성률은 각각 2.5%와 6.8%, interval cancer rate (per 1,000)는 각각 0.99와 0.96이었음. 종합병원급만 분석하였을 때 우수검사실과 비우수검사실의 결과는 검진 양성률이 각각 2.7%와 5.6%, 위양성률이 각각 2.6%와 5.5%, interval cancer rate (per 1,000)는 각각 1.01과 0.92임. 모든 종합병원급 기관이 우수검사실 인증을 받음을 가정하면, 위양성률 감소로 인한 암검진 및 확진에 필요한 건강보험 재정은 165억에서 128억으로 절감하고, 한 건의 대장암을 찾아내기 위한 건강보험재정을 12,355천원에서 10,965천원으로 낮출 수 있음. 중합병원급에 해당하는 한 개 기관을 우수검사실 인증에 참여하게 되어 얻는 재정절감은 20,449천원(per unit), 병원급 11,331천원(per unit), 의원급 7,477천원(per unit) 순임. 현재 국가암검진사업에서 투입되는 건강보험재정은 대장암 한건 당 12,566천원인데, 모든 검진기관이 우수검사실 인증을 받는 경우, 한 건 발견 비용이 9,063천원으로 절감됨. 연구결과는 822만건에 대한 재정 절감 효과를 추정한 것으로 100만건의 분변잠혈검사에 4,736,724천원의 재정 절감 효과를 보이는데, 2012년 기준, 국가암검진사업에서는 388만명의 수검자가 검진을 받고 있어 모든 기관이 높은 표준화에 도달했을 때 재정절감 효과는 18,378,490천원에 이를 것으로 예상됨.
결과들을 종합하여 표준화 비용 대비 비용절감 효과를 당뇨병검진과 대장암검진 모두에서 확인하였음. 당뇨병검진에서 당검사의 표준화를 위하여 최대 9억1천만원의 연간 표준화 비용을 투자할 경우, 연간 절감 가능한 비용은 위양성에서 약 1억원, 위음성에서 약 46억원으로 추정되었음. 국가대장암검진에서 분변잠혈검사를 위해 최대 13억 8천만원의 연간 표준화 비용을 투자할 경우, 연간 약 183억원의 비용 절감 효과를 국가대장암검진에서 얻을 것으로 추정되었음.

It is said that the impact of diagnostic test (laboratory test) on clinical decision is approximately 70%. In general, reliability of a diagnostic test is based on two factors; standardization of diagnostic product and standardization of practice. In Korea, standardization of diagnostic product is established on legal basis with the administrative assistance. Whereas standardization of practice is less well established and much of it depends on a private sector, namely clinical laboratory accreditation program (CLAP) which is driven by the academic society or the external quality assessment (EQA).
In clinical laboratory, the word 'standardization' is often regarded as 'quality improvement' or 'quality assurance'. The lack of standardization is well known to affect both accuracy and reliability of a diagnostic test which is also related to an increased false-positivity or negativity of a test. False-positive results can cause unnecessary treatments leading to an increment of medical and social expenses. Whereas, false-negative results can be associated with even more detrimental outcomes due to missed diagnosis. In this study, we tried to clarify this interrelation and to provide scientific basis for legislation of standardization of laboratory test.
Main components of standardization of practice in Korea are CLAP and EQA. CLAP is an on-site peer review involved in an evaluation of structure, process and the results. Whereas, EQA compares the test results between different laboratories for a single specimen. The disadvantage of EQA lies on its omission of evaluation for process areas. We classified the laboratories that take both CLAP and EQA as 'highly standardized' and those that take EQA only as 'basically standardized'. Laboratories that are not involved in any standardization programs were classified as 'unstandardized'.
To evaluate the impact of standardization towards the accuracy of test results, we've collected data for clinical chemistry, diagnostic hematology and alpha-fetoprotein from more than 1300 sites. Clinical chemistry data for 2010 to 2013 was adopted and compared for variance index score (VIS). Diagnostic hematology data (hemoglobin) for 2005 to 2013 was adopted and compared for standard deviation index (SDI). Alpha-fetoprotein data for 2010 to 2014 was adopted and compared for SDI. Institutions joining for EQA were grouped according to their CLAP participations and the statistical analysis was performed for VIS or SDI.
As CLAP-participants were likely to be from larger institutions, all organisations were classified as per their sizes. Entrusted laboratory agencies were analysed separately from the hospitals. Results showed that CLAP-participants outperformed CLAP-non-participants in almost every item and the differences were statistically significant.
We classified medical institutions as general hospital, hospital, clinic and entrusted laboratory agency. The geometric means of VIS and SDI were significantly lower in general hospitals and entrusted laboratory agency groups.
Between general hospitals, CLAP-participants excelled non-participants in clinical chemistry items but in diagnostic hematology items, there were no significant differences. We assume that clinical chemistry items, compared to the diagnostic hematology items, are much more affected by the levels of standardization in both structures and processes. So, we choose blood glucose level test as an indicator to study for the prevalence of diabetes and the cost-effect analysis for diabetes check-up, as this is the most common and the clinically significant test available for diabetes.
The geometric means of VIS and SDI were also significantly lower in general hospitals and entrusted laboratory agency groups for alpha-fetoprotein. But in hospital and clinic groups, CLAP-participants didn't show excellence in terms of geometric means of SDI. This might have been due to the fact that the most of CLAP-non-participants only reported the qualitative results, masking the differences in accuracy between the two groups.
Systemic bias for a test which have cut-off value, leads to diagnostic false-positivity and negativity. Review of EQA results for glucose showed that CLAP-participants surpassed CLAP-non-participants and the differences were statistically significant. If we took 11.9% as true prevalence for diabetes in 2013, under the influence of systematic bias for false-positivity, the prevalence of highly standardized, basically standardized and unstandardized organizations were 12.3%, 12.4%, and 13% each. Simulating change of prevalence for the composition rate of CLAP-participants/CLAP-non-participants didn’t show much change. This is the first case to show the effect of standardization for modification of national prevalence.
If glucose test has systematic bias for false-negativity, unstandardized group dominated in cost-effectiveness analysis. This could be explained as unstandardized group generally have much larger false-negativity and leads to minimized diabetes prevalence, making reduced cost with increased effect. If we assume systematic bias for false-positivity, standardized group was dominant in cost-effectiveness analysis. This might be the consequence of lower false-positive rate in standardized group comparing with unstandardized group. To weigh three options up, a league table was made according to cost to compare incremental cost-effectiveness ratio. In median false-negativity scenario, unstandardized group was dominant. But in median false-positivity scenario, highly standardized becomes dominant. In median false-negativity scenario, diabetes prevalence of standardized group increased for unstandardized group while prevalence decreased for standardized group in median false-positivity scenario, causing changes in cost-effectiveness. Criteria for false-positivity and negativity varies according to standardization of diagnostic test and it changes prevalence and finally has effect on cost-effectiveness of diabetes screening.
Employing data from National Cancer Screening Program for colorectal cancer in 2007-2010, positivity for colorectal cancer in CLAP-participants and CLAP-non-participants were 2.6%, 7.0% each and false-positivity were 2.5%, 6.8% while interval cancer rate (per 1,000) was 0.99 and 0.96.
Comparison of CLAP-participants and CLAP-non-participants within general hospital group, positivity and false-positivity was 2.7%, 5.6% and 2.6%, 5.5% each. Interval cancer rate (per 1,000) of participants and non-participants was 1.01 and 0.92.
Assuming that all general hospital become CLAP-participant, falling of false-positivity cause decrease of national health insurance expenditure from 165 billion KRW to 128 billion KRW. Health insurance expenditure to find a single colorectal cancer case drops from twelve million KRW to ten million KRW. Cost saving effect from joining CRAP is twenty million KRW for a general hospital unit, eleven million for a hospital unit, and seven million for a clinic unit. Currently, the government puts twelve million KRW per each colorectal screening test. If CRAP participation becomes universal, the cost drops to nine million KRW.
This study has based on eight million cases from which 4.7 billion KRW saves, and if we apply this assumption to National Cancer Screening Program in 2012, eighteen billion KRW could be saved under the universal CRAP-participation of healthcare institutions.
