A study of 61 elderly patients (66 years or more) who received surgical treatment for aneurysmal subarachnoid hemorrhage (SAH) at the Chonnam University hospital was done in a five year period from January 1991 to December 1995 and compared with that of a younger group (those aged 65 or less: 481 patients) in terms of the clinical characteristics, operative outcome and factors affecting it in order to decide the possibility of surgical intervention. Admission grade according to Hunt-Hess of I, II for the younger group was 6%, 55.1% respectively and 3%, 41% in the elderly group. Patients presenting with grades III, IV and V were 29.5%, 7.1% and 2.3% respectively in the younger group and 41%, 15% and 0% for the elderly group. There was no significant difference in the site of aneurysms between the two groups but aneurysms of 12 to 24 mm were more frequently found in the elderly group (29.5%) compared to 16% for the younger group (p<0.05). The incidences of rebleeding and symptomatic vasospasm were not different between the two groups but there was an increased incidence of hydrocephalus in the elderly group (19.7% versus 8.1% for the younger group)(p<0.05). The postoperative mortality was 8.2% in the elderly group and 4.1% in the younger group. Favorable outcome defined as good recovery or moderately disabled by the Glasgow outcome scale was 84.2% for the younger group and 72.2% for the elderly group. There were no significant differences of the surgical outcome by the preoperative grades. Major causes far disability and death in the younger group were direct effect of bleed (7%), rebleeding (7%), vasospasm (6.8%) and operative complications (6.4%) and for tile elderly group was operative complications (14.7%) medical complications (11.5%), vasospasm (11.5%) and rebleeding (8.2%) in declining order. We conclude that far elderly patients presenting with good admission grades and general condition, aggressive approach with surgical treatment with regard to intraoperative hemodynamic fluctuations and preexisting conditions is indicated.
