The present study was undertaken to evaluate the effect of Euonymus alatus (EA) extract on antitumor activity and toxicity of doxorubicin (Dox), and to investigate some mechanisms responsible for its action. Water or ethyl acetate EA extract was used in this experiments. Antitumor activity of EA, singly or in combination with Dox, was evaluated by measuring tumor-size in tumor cell implanted (sc) mice, by numerating the tumor spots of lung surface in B16F10 melanoma cell injected (iv) mice, and by determining in vitro proliferation responses of tumor cells. Antidotic activity of EA was estimated by revivified prolieration of Dox-treated lymphocytes, by restoration of Dox-induced skin ulceration, and by increment of lethal dose of Dox. And angiostatic effect of EA was also investigated by observation of neovascularization of chorioallantoic membrane (CAM) induced by tumor cell-conditioned media in chicken embryo, and by in vitro proliferation responses of human umbilical vein endothelial (HUVE) cells. EA itself diminished not only the tumor induction but also the growth of tumors in tumor cell implanted mice, and strongly manifested the synergistic effect on antitumor activity of Dox. Three intermittent iv administrations of Dox at intervals of 3 day resulted in a significant inhibition of lung metastasis of B16F10 cells in the experimental metastasis models. Intraperitoneal administrations of EA was found to be effective in preventing lung metastasis of melanoma cells. When EA was administrated into mice in combination with Dox, the tumor metastasis was completely abolished. Whereas CAM of conditioned media-treated chicken embryo showed cloudiness owing to numerous sprouting new-oriented blood vessel, inclusion of EA in conditioned media resulted in complete inhibition of neovascularization throughout the 18-day evaluation period. EA itself significantly decreased the proliferation of HUVE cells. These results strongly suggest that the inhibition of tumor growth and metastasis by EA may be in part due to the inhibition of tumor-associated angiogenesis. When B16F10 cells, 3LL cells and human tonsillar lymphocytes were cultured in the presence of Dox with or without EA, Dox alone decreased remarkably the proliferation of malignant cells as well as normal lymphocytes, in a dose dependent fashion. On the other hand, the EA itself reduced weakly the responses of tumor cells but increased those of normal cells. EA also slightly enhanced the Dox-induced inhibition of the tumor cell-proliferation, but it restored the Dox-derived decrease of the lymphocyte-proliferation. In rat model, intradermal injection of Dox alone produced predictable uniform skin necrosis and ulceration, and all rats died in 55 days of observation. However, intradermal injection of Dox plus EA into rats developed only minor transient edema and mild inflammation, which healed quickly, and all rats survived throughout the experimental periods. EA induced the tolerance of mice to systemic toxicity of Dox, increasing LD_50 by 30 times. These results suggest that EA itself has no side effects, and that EA has an antidotic activity against systemic Dox-administration. Taken together, the present study reveals that EA itself has a strong anticancer activity, and that its anticancer effect might be manifested chiefly by obstruction of neovascularization to tumor. These results also suggest the possibility that EA could be developed and used as a new anticancer drug as well as antidotic and reinforcing agent of Dox.
