There are several reasons to believe that the prevalence, characteristics, and manifestations of GERD may be altered in the elderly. Many older people, as a result of disease and medications, have decreased in salivary flow, gastric acid production, esophageal motility, gastric emptying and/or lower esophageal sphincter tone and increased frequency of hiatal hernia. There is a tendency for older people to underreport and tolerate symptoms that young people would find less tolerable. Coexistent illnesses such as chronic obstructive pulmonary disease and coronary artery disease, which can be confused with or exacerbated by GERD, are more common in the elderly. In present study I investigated the edoscopically proved reflux esophagitis and risk factor in the elderly patients.
The results were as follows :
1. The male/female ratio of reflux esophagitis was 2.3:1. The male/female ratio of reflux esophagitis in the elderly(≥60) is 1.2:1.
2. The frequency of hiatal hernia among patients with reflux esophagtitis was 7.6%. The male/female ratio of hiatal hernia with reflux esophagitis was 1.3:1. The frequency of hiatal hernia in the elderly was higher in female than in male(female vs. male : 22.7% vs 12.1%, P<0.05).
3. The frequency of accompanying hiatal hernia in patients with reflux esophagitis was found to increase according to age.
