The majority of activity of TRAb is in the immunoglobulin G (IgG) class. However, there may be a role of other immunoglobulin such as immunoglobulin E (IgE). IgE accumulation has been reported to occur in the thyroid gland and ocular muscles of Graves' disease. It has been also noted that recurrence of Graves' disease can be induced by pollen allergy, and pollen allergy is one of most common diseases associated with IgE. Thus IgE might play a role in the induction of Graves' disease. The aim of this study is to define whether IgE was elevated in Graves' disease, and to evaluate the potential relationship between TRAb and IgE levels in the Graves' disease.
Methods: Forty-six patients with Graves' disease, and 6 patients with chronic thyroiditis who diagnosed at Kosin Medical Center from April, 2000 to July, 2000 were included in this study. Thirty-five persons without thyroid disease and history of allergic rhinitis were included as normal controls. TRAb was measured by thyrotropin binding inhibitory immunoglobulin (TBII) method. Serum total IgE was measured by enzymeimmunoassay method. All of cases were tested thyroid function, TBII, and total IgE and their results were statistically analyzed.
Results: TBII as IgG was high in the Graves' disease and serum IgE level was also higher in Graves' disease (598.1±1112.9U/㎖) compared to the thyroiditis (98.5±79.7U/㎖) and controls (161.7±194.4U/㎖) (p<0.05). The prevalence of allergic rhinitis in the Graves' disease was 10.9%. Serum IgE level in Graves' disease with allergic rhinitis (903.1±1152.2U/㎖) was higher than those without allergic rhinitis (560.8±1117.0U/㎖), though there was no significance between two groups. According to the clinical stage, serum TBII level was higher in the Graves' patients who were not treated (49.9±23.9%) and those who relapsed (21.1±3.1%) than treated group (7.4±18.6%) (p<0.05). Serum IgE level was higher in the Graves' patients who were not treated (758.6±1250.2U/㎖) and those who relapsed (1198.5±1952.1U/㎖) than treated group (233.8±432.7U/㎖), though not significant. Conclusions: The concentration of IgE was high in Graves' patients and though statistically not significant, serum IgE level in Graves' patients with allergic rhinitis was higher than those without. In aspect of the clinical stage of Graves' disease, the change of IgE level tends to follow that of TBII. Further study is needed, to define the possible role of IgE in the pathogenesis in Graves' disease.
