결과: IgA 신증군과 non-IgA 신증군 간에 나이, 성별, 혈청 크레아티닌 농도, 사구체 여과율, 24시간 뇨 단백량은 통계학적으로 유의한 차이가 없었다.

Background: The mechanism of progressive renal injury in glomerulonephritis is diverse. Recently, various cytokines have been suggested to be involved in the progression of renal injury. Overproduction of transforming growth factor- β (TGF-β) results in pathologic fibrosis via the accumulation of extracellular matrix proteins. Tumor necrosis factor-α (TNF-α) is known to induce apoptosis of the mesangial cell. The aim of this study was to investigate the serum level and renal expression of TGF-β and TNF-α in patients with IgA nephropathy and non-IgA nephropathy and the effect of an ACE inhibitor on serum levels of these cytokines.
Methods: Total 30 patients with proteinuria and/or hematuria and 5 healthy adults with normal renal function as control group were selected as the subjects.
Serum TGF-β and TNF-α concentrations were measured using ELISA before renal biopsy. After renal biopsy, ACE inhibitors were administrated for 8 weeks, and then the serum TGF-β and TNF-α concentrations and renal functions were determined in 10 patients. Renal biopsy was done on 30 patients, and 29 cases except 1 case showing normal histology were divided into 2 groups according to biopsy findings: IgA nephropathy(n=19) and non-IgA nephropathy(n=10). Immunohistochemical stainings for TGF-β and TNF-α were done on biopsy specimens. The difference and relationship of serum level and renal expression of TGF-β and TNF-α in two groups were evaluated.
Results: Serum TGF-β level in IgA nephropathy and non-IgA nephropathy was 581±50 and 588±40 pg/ml, respectively, which were significantly higher than that in normal controls. Serum TNF-α level was not significantly elevated in patients with IgA nephropathy and non-IgA nephropathy compared with normal controls. Renal expression of TGF-βand TNF-α was increased significantly in IgA nephropathy compared with non-IgA nephropathy and normal controls.
There was no relationship between the serum level and the renal expression of TGF-β and TNF-α. Treatment with ACE inhibitor for 8 weeks resulted in no changes in serum TGF-βand TNF-α levels, but reduction in urinary protein excretion was noted in 7 of 10 patients.
Conclusion: TGF-β and TNF-α might roles in the progression of renal injury in glomerulonephritis, especially in IgA nephropathy. However, there was no correlation between serum levels and renal expressions of TGF-β and TNF-α, which suggest that renal injury might be more related to renal expression of cytokines than serum levels. ACE inhibitor did not decrease the serum level of these cytokines but it alleviated proteinuria. So It might inhibit renal injury via direct effect on renal tissues.
