목적 : 악안면골의 손상은 사회의 산업화에 따른 증가된 교통수단 및 폭력사고의 증가, 스포츠 및 여가문화의 발달, 그리고 경제적, 지역적 요소에 따라 점차적으로 증가하는 추세이며 특히 하악골 골절은 악안면골 골절 중 비골 골절 다음으로 높은 빈도를 차지하고 있다. 하악골은 다론 안면부와 더불어 호흡, 감각, 교합 및 저작 운동을 담당하는 중요한 구조이다. 또한 하악골은 안면부 하부를 구성하여 전체적인 윤곽과 대칭성 및 돌출 정도를 가늠하는 척도로 미용상 큰 비중을 차지하고 있다. 따라서 안면부 손상으로 초래될 수 있는 외모의 변형을 막고 기능적 장애를 최소화하기 위해 정확한 진단과 해부학적인 정복, 안정된 고정이 필요하고 합병증 없이 빠른 시일 안에 사회생활을 할 수 있도록 해야 한다. 이를 위해 내원한 환자의 임상기록을 종합 검토하여 얻은 결과를 문헌고찰과 함께 소개하고자 한다. 방법 : 1997년 3월 1일 부터 2002년 2월 28일까지 만 5년간 인제대학교 부산백병원 성형외과에 입원하여 치료 받았던 211명의 하악골 골절 환자의 임상기록을 종합 검토하여 성별 및 연령분포, 발생시간, 월별 및 연도별 환자수, 발병원인, 골절부위 양상에 따른 분류, 하악골이외 동반손상정도, 수술시기 및 방법, 합병증에 대한 통계분석을 하였다. 결과 및 결론 : 하악골 골절의 발생에서 성별분포는 남성이 159례(75.4%), 여성이 52례(24.6%)로 남녀 성비는 4.1:1 이었다. 연령별 분포를 보면 최연소 19개월에서 최고령자가 74세로 평균 연령은 29.3세였고 20대가 69례(32.7%)로 가장 높은 빈도를 나타냈으며 특히 사회 활동이 활발한 20대와 30대가 전체의 56%를 차지하였다. 시간별로는 오후 9시부터 다음날 새벽 3시까지가 51.2%로 가장 많았으며, 월별로는 6월이 24례로 가장 높았으며, 4월에서부터 9월까지 다소 증가하는 형태를 보였다. 발생 원인은 교통사고(35%)가 가장 많았으며, 해부학적인 부위에 따른 발생 빈도는 하악 결합부 골절이 36.6%로 가장 많이 발생하였으며, 다음이 하악각골 28.9% 과상들기 골절 24.2% 순이었다. 하악골 골절과 동반된 안면골 골절은 ...

Objective : There is a tendency that facial bone damage is increasing due to the growing traffic accidents caused by industrialization, act of violence and investment to leisure time. Especially, the frequency of mandible fracture is the second highest next to nasal bone fracture among the facial bone fractures. The mandible is an important part for respiration, sense, occlusion and chewing with other facial bones. In functional aspect, mandible fixture causes pain and difficulty in chewing. Therefore, this study will present the result of the retrospective study of 211 patients with mandibular fracture and other research outcomes. Method : This retrospective study comprised 211 patients with mandibular fractures who had been treated in the department of plastic and reconstructive Surgery of the Inje university Pusan Paik Hospital from March, 1, 1997 to February, 28, 2002. The medical records of these 211 patients with mandibular fractures were reviewed and analyzed. The studied items are sex and age distribution, time of the fracture occurrence, seasonal and monthly incidence, causes of the fracture, fracture sites, accompanied soft tissue and bone injuries, time interval between the accident and operation, treatment methods, and the complications. Results & Conclusions : The following results were obtained. 1) The male and female ratio of mandibular fractures was 4.1:l. The mandibular fractures of the young men in twenties were the highest(32.7%). The greatest numbers of patients were in twenties and thirties(56%). 2) The monthly incidence was the highest in June, and the incidence had been rather increased from April to September. The greatest numbers of incidence occurred between 9 p.m and 3 a.m.(51.2%). 3) The most common cause of mandibular fracture was the traffic accident(35%). 4) The most common fracture site was symphysis(36.6%), followed by angle(28.9%), condyle(24.2 %). 5) The mandibular fractures were usually accompanied with zygoma, maxilla, orbit and nasal fractures. 6) The most common time interval between onset and surgical intervention was less than 10 days (67.3%). Most patients were treated by intermaxillary fixation with open reduction. 7) The long term follow up longer than 6 months was possible in 103 patients. Malocclusion (4.9%), sensory disturbance of lower lip (3.9%), malunion or nonunion (2.9%), and infection (2.9%) were the complications found in these patients.
