ABC 방법에 기초한 면역조직 화학검사를 이용하여 암종조직과 정상 조직에서 survivin과 p53 단백의 발현을 조사하고, Ki-67의 발현을 이용한 증식지수를 측정하였다. Survivin 발현과 환자의 생존기간과의 상관관계, 임상적-병리학적 변수들과의 상관관계, p53 과발현과의 상관관계, 및 증식지수와의 상관관계를 분석하였다. 결 과 : Survivin은 59례 중에서 47례(80%)에서 종양세포의 핵 또는 세포질에서 발현되었고, 핵에 발현된 경우는 59례 중에서 33례(56%)였다. 림프절 전이가 있는 31례 중에서 survivin 핵 발현 양성군은 22례(71%)로써 통계학적으로 의미있는 상관관계가 있었고(p=0.014), 림프관/혈관 침범이 관찰되는 9례 중에서 survivin 핵 발현 양성군은 8례(89%)로써 통계학적으로 의미있는 상관관계가 있었다(p=0.032), 연령, 성별, 조직학적 유형, 병리학적 병기와 T 병기는 survivin의 핵 발현군과 survivin 세포질 발현군 모두에서 상관관계가 관찰되지 않았다.(p>0.05). p53은 survivin의 핵 발현 양성군 33례 중에서 21례에서 양성이었고, 이들 두 단백의 발현은 유의한 상관관계가 있었다(p=0.048). Ki-67 지수는 survivin 핵 발현 양성군 32례에서 24.2±6.9이었고, survivin 핵 발현과 Ki-67 증식지수는 유의한 상관관계가 있었다(p=0.045), Kaplan-Meier 5년 생존분석에서 survivin의 핵 발현 양성군의 생존율은 음성군의 생존율에 대하여 유의하게 낮았다.(p=0.003). 결 론 : Survivin의 핵 발현은 독립적인 불량한 예후인자로서 유용하며, 림프관/혈관 침범과 유의한 상관성을 보여 주었다. 변이형 p53의 과발현은 비소세포 폐암종에서 survivin 과발현의 가능한 기전의 하나일 것으로 보인다.

Background and purpose : Survivin is a novel member of inhibitor-f-apoptosis (IAP). Its expression is undetectable in most terminally differentiated nonproliferative adult tissue, but overexpressed in a variety of human malignancies. Survivin has dual functions, with mitosis regulation and preservation of cell viability, and is involved in cancer progression. Recently, transcriptional repression of the survivin gene by wild-type p53 have been reported, with binding the survivin promotor. Possible mechanisms of survivin up-regulation in cancers, such as amplication of the survivin locus on chromosome 17, DNA methylation in its promotor region, and recently loss of p53, have been proposed. This study aims for investigating the prevalence of survivin, its association with p53 overexpression and proliferative index, and clinicopathological significance in non-small cell lung carcinoma (NSCLC). Materials and Methods : Archival tissue blocks recruited in this study were retrieved from fifty-nine patients with primary non-small cell lung carcinoma, who underwent pneumonectomy or lobectomy from 1997 to 1999. Survivin and p53 protein expression and Ki-67 labeling index in tumor tissues were evaluated, using a standard avidin-biotin-peroxidase complex (ABC) immunohistochemistry. Correlations between the survivin expression and p53 overexpression and relationship between the survivin expression and Ki-67 index were analyzed. Clinicopathologic prognostic parameters were also analyzed. Results : Survivin expression was detected in 47 carcinomas (80%). Survivin was expressed in nucleus and/or cytoplasm of tumor cells. Nuclear and cytoplasmic immunoreactivity was found in 56% and 59% of tumors, respectively, and all of them were present in 36%. Nuclear survivin positivity revealed significantly worse prognosis in NSCLC patients (p=0.003). Nuclear immunoreactivity also correlated with lymph node metastasis (p=0.014) and lymphovascular invasion (p=0.032). Survivin expression did not correlated with sex, age, histological type, pathological stage, pT status (p>0.05). Accumulation of aberrant p53 correlated with nuclear survivin positivity (p=0.048), and Ki-67 index also correlated with nuclear survivin positivity (KI 24.2±6.9, p=0.045). Conclusion : Survivin is highly expressed in NSCLC, and nuclear expression of survivin could predict poor survival in patient having NSCLC. Expression of mutant p53 seems to be a possible mechanism of survivin up-regulation in NSCLC.
