Recently several topical preparations containing lidocaine, a widely used local anesthetic agent, have been developed and marketed for the treatment of premature ejaculation. In this study, microemulsion based lyogels and hydrogels were prepared to enhance the skin permeation and to control the drug release. First of all, SMEDDS (Self-MicroEmulsifying Drug Delivery System) containing lidocaine was formulated with Labrafil or IPM (oil), Cremophor EL (surfactant), and ethanol (cosurfactant). Lyogels were formulated with SMEDDS by the addition of PEG 6000 as a thickener, and evaluated for pharmaceutical properties such as self-emulsifiability, the size of dispersed droplet, drug release, and skin permeation of drug. In comparison to FAPG ointment as a reference preparation, lyogels showed enhanced skin penetration and it was washed off easily with water and non-sticky as well. In addition, microemulsion-based (ME) hydrogels were prepared by the dispersion of SMEDDS to hydrophilic polymeric hydrogel bases, e.g., carbomer gels, sodium alginate gels, and CMC-Na gels. Lidocaine release from ME hydrogels was followed by apparent zero-order kinetics, but there was a little difference between preparations depending on the polymer used. Especially in comparison to alcoholic carbomer hydrogel as a reference preparation, ME hydrogels had some advantages over alcoholic hydrogel due to the increased skin penetration and controlled release. On the other hand, tail flick test in rats was introduced to compare in vivo local anesthetic effects of different hydrogels, and response time was measured. The results were as follows : 5.5 sec for untreated group, over 30 sec for ME hydrogel-treated group, and 23.8 sec for alcoholic hydrogel-treated group. Finally it was possible to conclude that ME hydrogels containing lidocaine could be a novel topical delivery system for the treatment of premature ejaculation.
