The fields of this research are development of the noninvasive blood glucose monitoring system, clinical trials, an accuracy and reproducibility improvement through fundamental study. In this study, It investigated the influence factor, such as blood components excluding glucose, water, long-term study, subject body temperature, difference of spectrometric performance, blood glucose level range, on non-invasive blood glucose monitoring using near-infrared spectroscopy. To investigate the influence of water and spectrometric difference, It measured aqueous glucose solution with transmittance method using three types near-infrared system. Glucose absorption band appeared at second overtone, first overtone, combination band of all systems spectrum and partial least squares (PLS) regression models were the alnalytical utility to determine glucose concentration. Spectrometric sensitivity evaluated by absorbance for concentration and sensitivity for glucose absorption bands, second overtone, first overtone, and combination band, were good in three types system, dispersive, PDA, FT near-infrared spectrometer, respectively. For studying of the water effect, pH 7.4 Phosphate buffer and air were used as reference in respect of transmittance and the calibration results were compared. Raw spectra using a phosphate buffer showed more glucose absorption bands at combination band. PLSR modeling results using a phosphate buffer also were better than it using the air. It was measured blood components powder and glucose in whole blood and undiluted serum, which was more complex matrix than it, with a PDA type spectrometer to investigate the influence of the interference in blood. It validated glucose absorption bands and good quantitativity under exsisting of other blood componets. Used blood components response independent of glucose in near-infrared region by Principle Component Analysis (PCA) results. PLS models of blood glucose for forearm spectra in normal subjects showed model better than others. We suspected Hemoglobin Alc (HbAlc) as the critical influence factor on non-invasive blood glucose measurement because it has irreversibly bonded glucose. The study was carried out in diabetic patients by PDA type system having fiber optic probe within and PLSR and Error grid analysis (EGA) were applied for determine of glucose and glucose included HbAlc. It presented more accuracy in PLSR modeling results and EGA results for blood glucose level included HbAlc. Female mice, normal, type I diabetes, type Ⅱ diabetes group, were used to investigate for the influence of the long-term study, body temperature of subjects and blood glucose levels range for 10 weeks. Calibration result was good for each week spectra, but total spectra result wasn't. PCA applied for 9 weeks spectra, then classification was carried out that the PCA model applied for the other week. The outliers appeared to subjects with low-body-temperature. Calibration results for total spectra excluding outliers significantly improved in standard errors of prediction (SEP) and correlation coefficient. Body temperature must control to 37℃ for good calibration model for determine of blood glucose levels using a PDA type near-infrared spectrometer. PLSR and EGA were applied to correlate the collected three groups spectra with the blood glucose levels and this obtained best result in tight and normal range of blood glucose. Results form this study validated that It was the influence of spectrometric difference, water, Hemoglobin Alc, long-term study, body temperature and blood glucose levels range on non-invasive blood glucose monitoring by near-infrared spectroscopy. This study established several influence factors on non-invasive blood glucose monitoring by near-infrared spectroscopy and showed possibility to ensuring accuracy and reproducibility through some proposed solutions.
