Method: HCV 항체 검사가 의뢰된 검체 중에서 총 447예(양성 결과로 판정된 219예, 음성 결과로 판정된 228예를 포함)와 seroconversion panel를 이용하여 Vitros Eci와 ADVIA Centaur에 비교 검사를 실시하였다. RIBA 3.0(Recombinant immunoblotting assay) 확인검사의 결과를 황금 기준으로 이용하여 두 장비에서 일치, 불일치하는 검체 100예를 대상으로 RIBA 확인검사를 시행하여 민감도와 특이도를 평가하였다. RIBA의 미확정 결과의 검체 24예는 HCV RT-PCR을 실시하여 확인하였다. Results: Vitros Eci 와 ADVIA Centaur의 일치율은 83%(371/447)이고, Kappa일치도는 0.67이었다. Vitros Eci 와 ADVIA Centaur에서 일치, 불일치하는 검체 100예에서의 RIBA 확인검사에서 미확정 결과의 검체는 민감도, 특이도의 평가에서 제외하였다. Since the first introduction of the anti-HCV ELISA test in 1989, the sensitivity and specificity have consistently been improved through the second and third generation assays. However, the results of CIA for anti-HCV have shown discrepant results and high false positive rates. HCV 3.0 of CIA for anti-HCV was evaluated by comparing Vitros ECi (Ortho-Clinical Diagnostics Inc, USA) with ADVIA Centaur (Bayer HealthCare LLC Diagnostics Division). Method: A total of 447 clinical samples (219 positive and 228 negative samples) and seroconversion panel underwent comparison between Vitros ECi and ADVIA Centaur. The sensitivity and specificity were evaluated by using the RIBA 3.0 (Recombinant immunoblotting assay) confirmatory test as gold standard. Samples with discrepant results were retested with each assay, and further tested with RIBA. Twenty-four RIBA results that were indeterminate were retested by HCV RT-PCR test. Results: The overall concordance rate between Vitros ECi and ADVIA Centaur was 83% (371/447). Kapper agreement was 0.67. Discrepant samples of results by Vitros ECi and ADVIA Centaur were confirmed by RIBA test. RIBA indeterminate results were not included in the final determination for resolved sensitivity and specificity. The resolved sensitivity and specificity were 100.0% and 48.0% in Vitros ECi, and 90.9%, and 90.3% in ADVIA Centaur, respectively. Most of the discrepant results between Vitros ECi and ADVIA Centaur showed low positives and false positives. HCV seroconversion panel test was used to compare the positive readings in the initial stage of HCV infection by screening tests. ADVIA Centaur detected HCV antibodies earlier than Vitros ECi did. Conclusion : Serological assays for the detection of anti-HCV are inadequate to discriminate acute from chronic or resolved infection. Detection of HCV RT-PCR and RIBA test may be useful in confirming the detection of antibodies to hepatitis C virus. Low positive CIA results require confirmatory RIBA testing. Attention must be paid to the low positive results in the screening tests, because there is a possibility of false positives for the anti-HCV results. Positive results of CIA that recommend another sample should be collected for repeat anti- HCV testing at least (≥1 month later) or for RIBA confirmatory testing and HCV RT-PCR testing. The S/C ratio of Vitros ECi recommended as a useful screening test for characterizing positive results for anti-HCV. The effects of potentially cross-reacting or interfering factors on the performance of concordant and discordant results of ADVIA Centaur by positive results of Vitros ECi were significantly associated with AST level. Although the false positive rate was high with the CIA, it seems to be a useful method for screening for the detection of anti-HCV in the clinical laboratory.
