Polyurethane foam dressings are becoming widely used in partial and full thickness skin wounds since they are safe and convenient to patients and physicians. However, the influence of polyurethane foam dressings on wound healing has not been fully determined yet. This study was designed to compare two internationally available polyurethane foams and three locally made polyurethane foams as to their effects on the wound healing in vitro and in vivo. For in vitro study fibroblasts from the dermis of healthy adults were isolated and cultivated. For the first study, 5 x 103 fibroblasts/well were incubated in a 96-well culture plate with either one of five polyurethane foams(Allevyn?, Biatain?, Medifoam-N?, chitin mixed Medifoam-N, and chitosan mixed Medifoam-N) on top not to contact the air. The medium used in this study was Dulbecco''s Modified Eagle''s Medium including 10% fetal bovine serum and the polyurethane foams were cut using 5mm punches. On the 4th day of plating, fibroblast proliferation was determined by MTT method. For the second study, 2 x 104 fibroblasts/well were incubated in a 24-well culture plate with either one of five polyurethane foams mentioned above, but the polyurethane foams were cut using 8mm punches in order that the cells might contact the air. Fibroblast proliferation was assessed at 5 days postplating by hematocytometer. Each polyurethane foam dressing was tested 8 times. For in vivo study, 20 white rats were used. A 5 mm punch was employed to excise skin and subcutaneous tissue in a round fashion at six sites on the back area of 10 rats. After creating 6 open wounds, above mentioned 5 polyurethane foams and gauze dressings were applied. On the 5th day the lengths of the nonepithelialized area were measured under light microscope. In addition, partial thickness wounds were created using carbon dioxide laser at six sites on the back of the another 10 rats. Six dressings were applied on the wounds as same as above. On the 4th day the degree of the epithelization was measured and compared. In vitro study the best fibroblast survival was seen in chitin mixed Medifoam-N group. In full thickness wound study the best wound closing was also seen in chitin mixed Medifoam-N group. In partial thickness wound study the best epithelization was seen in chitin mixed Medifoam-N and chitosan mixed Medifoam-N groups. These results demonstrated that polyurethane foam dressings influenced the wound healing, and chitin mixed Medifoam-N showed the best result.
