Orthokeratology는 RGP 콘택트렌즈를 이용하여 각막곡률을 변화시켜 근시도를 일시적으로 감소시키는 방법이다. 수술에 의한 근시 교정술은 만족된 결과를 얻을 수 있으나 수술 후 굴절이상도를 변경할 수 없다는 제약이 따른다. 요즘 이 점을 보완하여 수술하지 않고 각막형상을 변화시키는 orthokeratology 콘택트렌즈에 관심이 집중되고 있다. 본 연구에서는 orthokeratology 콘택트렌즈의 직경변화가 굴절이상도와 각막형태에 미치는 영향을 비교 분석하기 위해, 안질환이 없고 건강한 성인 16명(31안)을 대상으로 직경이 다른 두 가지의 수면착용 orthokeratology 콘택트렌즈(10.0mm, 11.2mm)를 착용시키고, 렌즈 착용 전, 렌즈 착용 후 1일 및 7일에 각각 자동 굴절검사, 각막곡률검사, 자각식 굴절검사 및 각막형태검사를 실시하였다. 두 가지 직경의 렌즈에서 나안시력의 개선 및 자각식 굴절검사에 의한 등가구면 굴절이상도의 감소는 차이가 없었고, 자동굴절검사에 의한 등가구면 굴절이상도의 감소는 유의한 차이가 있었다(t=-3.573, p=0.001). 각막난시의 변화도는 직경이 큰 렌즈에서 감소하고 직경이 작은 렌즈에서는 증가하여 유의한 차이가 있었다(t=-2.794, p=0.008). 렌즈 착용 일주일 후 자동각막곡률계로 측정한 평균 곡률반경(t=-3.586, p=0.001), 약주경선 ...

Orthokeratology is a method of temporarily decreasing myopia by changing the corneal curvature using RGP contact lens. Myopia correction via surgical procedure can produce satisfactory result, but it is accompanied by the restraint that refractive error after the surgery cannot be corrected. Recently, interest is being focused on orthokeratology contact lens that complements this limitation and changes the corneal shape without surgery In this study, in order to compare and analyze the effects change in diameter of orthokeratology contact lens has on refractive error and corneal shape, 16 healthy individuals with no eye diseases were fitted with two different types of overnight orthokeratology contact lens with different diameters(10.0mm and 11.2mm) and auto refractometer, auto keratometry, subjective refraction, and ORBSCAN topography/pachymetry were performed for each individual before wearing the contact lens, 1 day after wear, and 7 days after wearing the contact lens. In the lens with different diameters, there were no differences in improvement of visual acuity or in reduction of refractive error measured by subjective refraction, and significant difference was found in refractive error measured by auto refractometer(t=-3.573, p=0.001). Corneal astigmatism decreased in lens with greater diameter, and increased in lens with smaller diameter, which showed a significant difference (t=-2.794, p=0.008). The change in mean K-reading(t=-3.586, p=0.001), flat K-reading(t=-5.452, p=0.000), and steep K-reading(t=-5.117, p=0.000) measured using autokeratometer at one week after wearing the lens was flatter in the lens with greater diameter. Corneal thickness decreased in the central region and increased in the periphery which showed statistically significant differences(t=3.149, p=0.001). The width of treatment zone was slightly wider in the lens with greater diameter than in lens with smaller diameter (t=2.498, p=0.013). From the above results, although lens with 11.2mm diameter did not have differences in effect of improving visual acuity or decreasing astigmatism compared to lens with 10.0mm diameter, with changes in corneal curvature and increase in the region of pressure, it is believed to be able to provide stable myopia reduction effect and further studies that can be applied in the fitting process for solving clinical problems are anticipated.
