However, the incidence and clinical implication of BCR-ABL fusion gene mutations before and after treatment with imatinib for Korean patients have not yet been studied. Methods: A total of 139 patients diagnosed with BCR-ABL positive CML were screened by melting curve analysis, and the mutations were further confirmed by direct sequencing. Cryopreserved samples were analyzed using pyrosequencing, retrospectively. We compared the response following imatinib treatment and the development of acquired resistance for patients starting imatinib within 12 months after the initial diagnosis [early chronic phase (CP) in 42 patients] and for patients with a long pretreatment [late CP, 30 patients]. In addition, we investigated the expression of BCR-ABL mutant alleles in samples before and after imatinib treatment. We also examined the kinetics of the total mutant and BCR-ABL transcripts in sequentially followed samples. Results: The BCR-ABL KD mutation was detected in 16 (11.5%) out of 139 patients. Among 139 patients, 5 (3.6%) mutations (1 in accelerated phase and 4 in CP) were detected in the imatinib na?ve state by direct sequencing. Two patients with a mutation in early CP showed imatinib resistance. Among the imatinib treated late CP group patients, seventeen (43%) showed imatinib resistance. Twelve patients (71%) had BCR-ABL point mutations associated with imatinib resistance. The major cytogenetic response (MCyR) rate (P = 0.005), complete cytogenetic response (CCyR) rate (P = 0.003) and major molecular response (MMR) rate (P = 0.00) and the progression free survival (PFS) rate were significantly different between the early and late CP groups. In the early CP group, the cumulative rate for MCyR, CCyR and MMR after 36 months was 89%, 79% and 74%, respectively. In late CP group, the cumulative rate of MCyR, CCyR and MMR after 36 months was 60%, 50% and 28%, respectively. The PFS in the early and late CP groups was 92% and 51% at 60 months (p=0.019). The mutant alleles detected by pyrosequencing in 8 patients showed three contrasting kinetic patterns: the percentage of the mutant alleles in five patients fell in proportion to the expression level of the BCR-ABL transcripts after changing imatinib to dasatinib, nilotinib or by undergoing an imatinib dose increment (Group 1). There was no relationship of the total number of transcripts and mutant allele percentage in one patient (Group 2). In Group 3, the total level of transcripts maintained high and then never fell down. This reflected the increment of mutant clones and the patients in this group died from the progression of the disease. Conclusion: The CP patients with imatinib resisitance had detectable BCR-ABL KD mutations (64%). The presence of BCR-ABL mutations was the most common mechanism that confered resistance to this drug. Therefore, early detection of the BCR-ABL KD mutation by pyrosequencing before the emerging of resistance to imatinib may help to change treatment strategies. The dynamics of the mutant alleles obtained from pyrosequencing suggest that quantitative pyrosequencing may be useful as an alternative strategy for treatment planning.
