The purpose of this study was to examine the raw data of an investigation by Korea Institute for Health and Social Affairs into the state of elderly people's lives and welfare needs in 2004(Jeong Gyeong-hee, et. al., 2005) in a bid to find out what made elderly people utilize home care services. It's basically meant to make some suggestions about relevant policy setting and the practice of home care services. The investigation was conducted by 40 professional investigators that were trained by Korea Institute for Health and Social Affairs according to the lists of the institute. The investigators were divided into 10 teams that respectively included three investigators and one instructor. They visited every household in the selected regions to interview the selected subjects who were senior citizens. If there were two or more elderly people in a household, all of them were investigated. Out of the selected 3,278 elderly people, the direct responses from 3,029 elderly people(92.4%) were analyzed. The major findings of the study were as follows: First, the elderly people who used home care services got a mean of 12.43 in ADL, and those who didn't get a mean of 12.10. The former scored higher than the latter. In terms of IADL, the recipients of home care services got a mean of 11.15, and the non-recipients got a mean of 10.62. The former scored higher than the latter, and the gap between the two was statistically significant(p<0.001). Second, social support referred to the frequency of getting in touch with adult children. A high score in social support indicated a larger degree of social support. The recipients and non-recipients got a mean of 35.89 and 41.87 respectively, which showed that the latter received more social support. The gap between the two was statistically significant(p<0.001). Third, among predisposing factors involving residential area, gender, age and academic background, age was found as a determinant that had a significant impact on the use of home care services. Fourth, out of enabling factors including social support, household income and awareness of home care services, awareness of home care services was identified as a determinant that exerted a significant influence on the use of home care services. Fifth, in the event of need factors involving ADL, IADL and subjective health status, IADL had a statistically significant impact on the use of home care services. Finally, age and IADL, which were respectively one of predisposing factors and one of need factors, exercised an influence on the use of home care services when an inclusive model was employed by connecting all the predisposing, enabling and need factors. As a result of comparing the Wald test statistics of the variables, whether they used home care services(W=44.77) was the most influential variable, followed by IADL(W=9.41) and age(W=3.15). Given the findings of the study, the followings should be noted in relation to the elderly's use of home care services. First, who are eligible for home care services should clearly be determined. That is a very important factor to affect the efficiency of home care services that should be provided to needy clients by using limited resources. Most advanced countries attach the most importance to the presence or absence of disability and the extent of disability as criteria to select recipients of home care services, and the second most crucial criterion was social support, followed by income level measured by means test. In our country, however, stereotyped selection standards are applied, such as the amount of income and criteria of responsible caregiver that are approved or selected by the elderly welfare law and the national basic livelihood security law. In fact, income level may be one of criteria to select recipients of home care services, and that is one of primary prioritizing factors. But whether one is a basic livelihood security recipient is not a necessary and sufficient condition. Although a limited number of people are eligible for home care services due to a limited range of resources, elderly people who have difficulties in ADL and IADL and are devoid of social support should be selected first of all, instead of regarding just basic livelihood security recipients as eligible. In other words, ADL, IADL and disability should be viewed as the most vital criteria that take precedence over anything else, and in the event of elderly people with similar degree of disabilities, those who aren't provided social support from families or relatives should be more eligible. Second, the object of social welfare is to step up the redistribution of income among all the people to let them lead the best life. In the field of home care services, institutional policies are consequently necessary, which provides the phase-in services to ordinary elderly people including those from quasi-poor class as long as they are in want of the services. Third, the current national voucher project for the elderly just offers abstract evaluation indices that are just based on the level of independence and required assistance. So it's not easy to get an accurate grasp of in which part of life elderly people face difficulties, and a preparation of more precise, numerical evaluation indices that could make a more objective evaluation like Barthel scale is required to make a better selection of those who are eligible for home care services.
