The aims of this study were to investigate the clinical aspect of CP and to analyze the characteristics according to the calcification and history of pancreatitis. Methods: Eighty one medical records from the patients who were diagnosed CP at Korea University Ansan Hospital from January 1997 through December 2006 were reviewed retrospectively. Clinical parameters were investigated according to presence of pancreatic calcification or duct stone. After patients were divided into two groups according to the previous history of pancreatitis: the group I have no history of pancreatitis (n=30) and the group II have history of pancreatitis at least more than once (n=39), the clinical characteristics of two groups were compared. Results: The mean age was 50?3.7 years and the male to female ratio was 5:1. Alcohol was the cause of CP in 72.8%, and 24.7% showed no evident cause for it. We found pancreatic calcification or duct stone in 43 cases (53.1%) and these patients had severe grade (96.6%) of Cambridge classification on ERCP. Compared with the group I and II, the etiology (idiopathic 43.3% vs. 12.8%, p=0.015) and abdominal pain (88.5% vs. 100%, p=0.030) were significantly different between the two groups. Also, the group II had higher frequency of pseudocyst (16.7% vs. 41.0%, p=0.029) and total number of admission (2.1?.5 vs 3.4?.8, p=0.022) than the group I. Conclusion: The presence of the pancreatic calcification or duct stone which reflect severe grade on ERCP does not influence greatly the clinical aspect of CP. The prospective study over natural course of patients who present CP as first manifestation is needed because their clinical aspect is different from that of those who had previous history of pancreatitis.
