본 연구는 수의응급의학에서 저혈량성 쇼크의 응급치료에 관한 연구를 수행하기 위하여 실시되었다. 이를 위한 실험설계는 첫째 개에서 isoflurane을 이용한 자발적 호흡마취상태를 적용하여 저혈량성 쇼크 모델을 확립하고 이에 따른 심혈역동학적 그리고 신장기능의 평가지수를 분석하였다. 둘째 확립된 개의 저혈량성 쇼크 모델을 이용한 수액치료 효과를 확인하기 위하여 두 실험군에 각각 6% hetastrach와 자가전혈을 이용한 수액치료를 실시하였으며 이때 심혈역동학적 그리고 신장기능의 평가지수를 측정 분석하고자 하였다. 연구 1 첫번째 연구는 비글견 40마리를 이용하여 isoflurane을 이용한 자발호흡상태에서 40% 혈액을 손실시켜 severe hypovolemic shock을 유도하였다. 저혈량성 쇼크상태를 유도하기 위해서 baseline을 측정하고나서 30분 동안 오른쪽 경동맥을 이용하여 40% 혈액을 체내로부터 무균적으로 손실시켰다. 이후 MAP <50mmHg의 상태를 유지하기 위해서 추가적인 혈액을 제거하면서 1시간동안 유지하여 severe hypovolemic shock상태에 도달하게 하였다. Baseline(BL)과 Posthemorrhage(PH)에서 심혈역동지수 및 신장기능 지수를 평가하였다. PH시 심혈역동지수에서 MAP, MPAP, CI, SVI, DO2은 BL과 비교하여 59.1, 59, 67.1, 66, 72.6%까지 감소하였다. 저혈량성 쇼크를 유발한 상태에서 SAP 와 DAP 의 감소가 뚜렷하게 나타났다. HR, RR 그리고 SVRI 는 babaselin 대해서 각각 20, 12.3, 2.7% 증가하였다. 또한 LVEDV는 43.9%의 유의성있는 감소를 보였다. 신장기능에 대한 평가는 UO, Cre 그리고 urine IP 의 유의적 변화가 나타났다. FE test는 본 실험에서 유의성있는 변화를 발견할 수 없었다. 따라서 개에서 isoflurane을 이용한 마취에서 40% 혈액손실을 유도한 저혈량 쇼크는 실험적으로 severe hypovolemic shock 모델로서 적절하다고 판단되며 본 실험에서 baseline 과 posthhemorrhage의 상태를 비교하였을 때 심혈역동지수와 신장기능지수의 유의성 있는 차이를 발견할 수 있었다. 연구 2 두번째 연구에서는 비글견 20마리를 이용하여 6% hetahetach(HEX) (n=10)와 whole blood(WB) (n=10)그룹으로 각각 나누었다. 첫번째 실험과 같이 동일 방법을 이용하여 저혈량성 쇼크를 유발 후에 각 군에 각 각 수액치료를 30ml/kg/h 의 속도로 적용하고 적용 후 R4까지 심혈역동지수와 신장기능 지수를 평가하였다. WB를 적용한 군의 경우는 R4에서 BL에 준하는 또는 그이상의 심혈역동지수를 얻을 수 있었다. 그러나 HEX를 이용한 군은 R4에서 CO은 BL의 값을 회복하지 못하였다. WB군의 O2ER 은 R1에서의 HEX군의 수치에 비해서 월등하지 않았으나 R4의 WB군에서 DO2와 VO2는 HEX군에 비해서 높은 수치를 나타내었다. 추가적으로 PH+30, R2+15, R2+30, R3, R3+15, R3+30, R3+45를 제외한 실험기간 동안 HEX군의 LVEDV는 WB군에 비해서 유의성 있는 차이를 나타내었다. 각 군의 R4의 LVEDV는 BL에 비교하여 유의성 있는 변화를 나타냈다. 동시에 UO과 urine IP는 각 군에서 유의성 있는 변화를 나타내었으나 군간의 비교에서는 유의성 있는 차이를 발견할 수는 없었다. 결과적으로 각 각 6% hetastarch 와 whole blood를 적용한 저혈량성 쇼크 실험에서 심혈역동지수와 신장기능지수를 비교 분석할 수 있었다. 따라서 응급수의학에 있어서 치료에 필요한 whole blood를 확보하기 어려운 상태인 경우 6% hetastarch의 적용은 임상적가치가 있다고 사료된다.

Hypovolemic shock (HS) is an emergency condition in which severe blood and fluid loss makes the heart unable to pump enough blood to the body. In other words, hypovolemic shock is caused by a critical decrease in intravascular volume. Diminished venous return (preload) results in decreased ventricular filling and reduced stroke volume. The primary goal of fluid therapy is restoration of adequate tissue perfusion and oxygenation. Because a decrease in circulating blood volume below a critical level (e.g., hemorrhage in dogs resulting in a persistent systolic arterial blood pressure [SAP] below 45mmHg) causes shock to become progressive and ultimately leads to irreversible multiple organ system failure, it is generally accepted that treatment must be instituted immediately. Also, inadequate resuscitation with prolonged tissue hypoxia causes irreversible cell damage in end organs. Hypoxia limits adenosine triptophate availibility to cells and alters gene expression and free radical production. The objectives of the study were to: 1) evaluate inducing canine hypovolemic shock model in beagles to standardize hemodynamic status and renal functions as experimental animals except mongrel dogs and 2) evaluate effects of 6 % Hetastarch and Whole blood resuscitations for the treatment of canine hypovolemic shock model for assessment of hemodynamic parameters and renal functions from an emergentic and clinical point of view. First project, forty beagle dogs was induced hypovolemic shock (40% blood loss) without splecnectomy. This experimental design was applied using right carotid artery cannulation for blood removal and spontaneous ventilation for typical pre-hospital status. At post hemorrhage stage, hemodynamic variables were measured using 5 Fr Swan-Ganz catheter. Also, renal functions were measured. For assessment of hemodynamic parameters and renal functions, blood samples at femoral artery and pulmonary artery were collected. At the same time, urine samples were collected in bladder using a foley catheter. Hemodynamic parameters induced hypovolemic shock showed reliable significant differences except central venous pressure (CVP), pulmonary artery occlusion pressure (PAOP) and derived pulmonary vascular resistance index (PVRI). Renal functions parameters such as urine output (UO), creatinine (Cre) and Urine IP showed significat diffrences. Second project, twenty beagles were used for assessement of fluid resuscitation. All experimental groups were devided into 6% Hetarstarch (HEX) (n=10) and Whole blood (WB) (n=10). After inducing hypovolemic status, aggressive fluid resuscitations were performed using syringe pump. From PH to R1, resuscitated dogs were administered 30ml/kg/h of HEX or WB over 1 hour (at the end of PH). Blood sampling were performed at R1 and R4. For measurements of renal functions, urine samples were collected at R1 and R4. To evaluate fluid resuscitations, hemodynamic variables were calculated from PH to the end of R4. Some hemodynamic variables such as cardiac output (CO), oxygen delivery (DO2) and oxygen consumption (VO2) showed significant differences between HEX groups and WB groups. However, oxygen extraction rate (O2ER) was not significant difference at R4. Renal functions measurements for assessment of acute renal failure (ARF) or acute renal tubular damage by induced hypovolemic shock showed significant difference in UO and Urine IP. In conclusion, we evaluated canine hypovolemic shock model in beagle dogs which produce irrevisible shock by 40 % blood loss and typical pre-hospital status. Using this model, fluid resuscitations were applied. This experimental design was useful to understand hemodynamic parameters changes and assess renal functions parameters by induced low-flow blood status. Besides, according to aggressive fluid resuscitation, the influence of cardio vascular and renal function could be measured.
