Is there any possibility that Korean Herbal Medicine induce liver injury? : Systematic review in Korean literature원문보기

Introduction
The safety of Korean herbal medicine (KHM : herbal medicine prescribed and made by traditional Korean medical doctor) is an important issue in Korea. It is more complicated because of the dualistic medical system, the concomitant use of conventional western drug and KHM, folk medicine and food supplements. Although both the western medical doctors and traditional Korean medical doctors have been studied on the safety of herbs, their results were not concordance with each other.
This study aims to review the drug induced liver injury (DILI) and the influence of KHM on liver function in Korean literature. The purpose of the present study is to estimate the proportion of causative materials - conventional western drug, KHM, herbal products, folk medicine and food supplements - and the change of liver function test (LFT) data and the incidence of liver injury after use of KHM.
Method
Systematic literature searches were performed on 3 main databases in Korea. Screening and selection of the articles and the extraction of data were performed. There were no restrictions on the types of publication including grey literatures.
Results
Twenty-one articles were included from the studies on the DILI. Only 7 studies were performed prospectively and 6 studies were published in the peer reviewed journals. The proportion of conventional drug associated hepatotoxic injuries in all DILI ranged from 15.8% to 83.3%. The proportion of herbal medicine associated hepatotoxic injuries was from 28.9% to 44.7%. However, the criteria for herbal medicine were not clearly defined and concomitant medications were not fully investigated in most studies.
Forty articles were included from the studies of the influence of KHM on LFT. Sixteen studies were performed prospectively and 15 studies collected data from outpatients. Only 8 studies reported the occurrence of liver injury after use of KHM. Nineteen cases of liver injury showed mild or no symptom and elevation of LFT was not high. All liver injury patients took conventional western drug concomitantly and causality was not assessed properly. The incidence of liver injury after use of KHM was estimated as 0.59% - 0.76% from all articles in this study. The results were conflicting on the change of alanine aminotransferase (ALT) and total bilirubin (TB) after use of KHM.
Discussion and Conclusion
There were some studies reporting high proportion of herbal medicine as cause of DILI, but the causality assessments were not performed properly and causative materials were misclassified in the most of included studies. Otherwise KHM resulted in a low rate of liver injury, analyzing the studies of the influence of KHM on LFT. But the results are not powerful enough to make the safety issue clear. These limitations and lacks of objectivity of the studies make the safety issue still ambiguous. More rigorous studies are required for answering accurately this conflicting issues with the cooperative investigation between traditional Korean and western medicine.
