This quasi-experimental intervention study, based on non-equivalent control group pretest-posttest non-synchronized design, was performed to analyze the effect of regular practice for deep breathing using the incentive spirometer on postoperative pulmonary ventilation of 40 aged inpatients who underwent lobectomy at the P university hospital. Those patients were classified into an experimental group and a control group by 20s, and data were collected from April 1 until September 22, 2008. A questionnaire survey and a pulmonary ventilation test were conducted previously to the operation, and pulmonary ventilation test was again conducted 3 days after the operation. Also previous to the operation, two group learned how to take deep breathing using the incentive spirometer. In the case of the experimental group, checklists were given so that subject could take deep breathing every 30 minutes after undergoing the operation. The research team checked whether subjects followed directions, and retained subjects who did not regular practice for deep breathing. On other hand, the control group was not separately managed. Pulmonary ventilatory capacity was measured by using Micro Plus Respirometer (Vitalograph, UK) and typical indexs such as FVC (Forced Vital Capacity), FEV1 (Forced Expiratory Volume in 1 sec) and PEF (Peak Expiratory Flow) were measured. Pulmonary ventilation test was performed on two groups the day before and 3 days after the operation. Data collected from subjects were statistically analyzed by using the program 'SPSS Win 14.0'. In order to test inter-group homogeneity 'x2-test’ and 'two sample t-test' were performed and to test the hypothesis 'two sample t-test' was performed. In result, the following results came out. 1) Statistical significance was not derived from the hypothesis "The postoperative FVC of the aged who regular practiced deep breathing will be higher than that of the one who did not practice". 2) Statistical significance was not derived from the hypothesis "The postoperative FEV1 of the aged who regular practiced deep breathing will be higher than that of the one who did not practice". 3) Statistical significance was derived from the hypothesis "The postoperative PEF of the aged who regular practiced deep breathing will be rapider than that of the one who did not practice" (t=1.820, p=0.038). To sum up, regular practiced deep breathing using the incentive spirometer did not exert effect on postoperative FVC and FEV1, but exerted effect on postoperative PEF. Thus, the results of this study are expected to be used as basic data for the respiratory care for aged patients who underwent lobectomy.
