1. 서론 : 간섬유화는 독성물질이나 다양한 감염성, 면역성, 대사성 질환에 대한 반응이며 치유와 반흔으로 남는 과정으로 세포외기질(ECM, extracellular matrix)인 콜라겐, fibronectin과 laminin과 같은 glycoprotein, proteoglycan 등의 과도한 축적이 특징이다. 이런 ECM 합성의 증가는 보통 간성상세포(HSC, hepatic stellate cell)와 기타 간기질을 생성하는 간-유래 근섬유아세포-유사 세포 등의 활성화가 원인일 수 있다. 그러나 이러한 간섬유화 생성에 대한 기전 연구는 어느 정도 진전되고 있지만 간섬유화 억제를 위한 약물요법은 아직까지 알려진 것이 별로 없었다. 이에 본 연구는 아스피린과 항산화작용이 있는 자연추출물인 커큐민(curcumin)을 병합투여하여 독성 간섬유화 모델인 TAA(thioacetamide)-유발 간섬유화 흰쥐 모델에서의 간섬유화 억제 효과를 연구하여 치료제로서의 개발 가능성을 탐색하였다.

In spite of advances with study of mechanism of hepatic fibrosis, there is no effective medical therapy for hepatic fibrosis. In this study, authors tried to find out the possibility of combination therapy of aspirin and curcumin as a therapeutic agent of hepatic fibrosis.
Methods : The SD mice (weight = about 270 g) were used and hepatic fibrosis was induced by injection of 20% thioacetamide (TAA) 200mg/kg intraperitonealy 2 times per week for 7 weeks. After 7 weeks of injection of TAA, aspirin and curcumin was administered orally for different doses to each groups. After administration of aspirin and curcumin, several blood test, such as total protein, albumin, ALT, AST, alkaline phosphatase, total bilirubin, cholesterol, triglyceride and review of liver tissue , and quantification of 4-hydroxyproline were done. We divided the groups into normal control group (no TAA and no therapy), experimental control group (TAA and no therapy), experimental group-I (TAA + aspirin 10mg/kg + curcumin 300mg/kg), experimental group-II (TAA + aspirin 25mg/kg + curcumin 300mg/kg), and experimental group-III (TAA + aspirin 50mg/kg + curcumin 300mg/kg).
Results : Compared with normal control group, there was significant reduction of ALT, AST, alkaline phosphatase, and cholesterol in experimental control group. In addition, there was marked fibrosis on liver biopsy slide review, and marked elevation of 4-hydroxyproline in experimental control group. There was dose dependent reduction of each biochemical markers in each experimental groups, and especially there was statistically significant reduction of AST, alkaline phosphatase, Knodell's grade score, and 4-hydroxyproline in experimental group-III.
Discussion : According to the results, the authors expect the possibility of combination therapy of aspirin and curcumin as a therapeutic agent of hepatic fibrosis.
