This paper proposes to design and implement a system that diagnoses arrhythmia using an electrocardiogram(ECG) in real-time. This system is divided into two parts. One is the interval analysis algorithm using heart rate variability. The other is the beat analysis algorithm using P-QRS-T periods. Both algorithms use a neural network with weighted fuzzy membership functions(NEWFM) to diagnose arrhythmia. We use the MIT-BIH arrhythmia database for the benchmark. The proposed interval analysis algorithm diagnoses arrhythmia using 32 RR-intervals that are 25 seconds on average. We extract six statistical values from the 32 RR-intervals, which are used to input data of the NEWFM. The NEWFM used in this algorithm classifies normal and arrhythmia. The performances by Tsipouras using the 48 records of the MIT-BIH arrhythmia database was below 80% of SE(sensitivity) and SP(specificity) in both. The interval analysis algorithm shows 88.75% of SE, 82.28% of SP, and 86.31% of accuracy. The proposed beat analysis algorithm diagnoses premature ventricular contraction (PVC) using the P-QRS-T periods, which are 0.75 seconds on average. As the input of the algorithm, we use 14 coefficients of d3, d4, and d5, which are transformed by a discrete wavelet transform(DWT). The 14 coefficients are used to input data of the NEWFM. The NEWFM discussed in this algorithm classifies a normal beat and a PVC beat. The size of the window of DWT is -31/360∼+32/360 second(64 samples) whose center is the R wave. Using the seven records of the MIT-BIH arrhythmia database used in Shyu's paper, the classification performance of the beat analysis algorithm is 99.91%, which outperforms the 97.04% of Shyu's analysis. Using the forty records of the MIT-BIH arrhythmia database used in Inan's paper, the classification performance of the beat analysis algorithm is 98.01%, which outperforms 96.85% of Inan's one. The SE and SP of the beat analysis algorithm are 84.67% and 99.39%, which outperforms the 82.57% and 98.33%, respectively, of Inan's study. In this paper, we apply the real-time arrhythmia diagnostic algorithms to "CENTURY" which is an event stream-based infrastructure. CENTURY is an open and scalable infrastructure that provides a number of medical services using a variety of medical data from the users. This system is tested in two ways. One is way uses an ECG simulator device. The other uses a sensor simulator program. The ECG signal is transmitted for 1 second in a real-time environment. The computation time of diagnostic output is 750 msec on average and 1,500 msec on maximum. In this case, the start point is the 32nd R wave in the interval analysis algorithm and the R wave in the beat analysis one, respectively. Therefore, the system is verified in real time to have the result of performance as in the algorithm results. The proposed algorithms can be used in a heart-monitoring system using a variety of ECG devices in a ubiquitous environment. The interval analysis algorithm can be used for low-level devices such as heart rate monitor watches and portable ECG devices, while the beat analysis algorithm can be used for high-level devices such as patient monitors.
