Although micropapillary urothelial carcinoma (MPC) is known as an aggressive variant of urothelial carcinoma (UC), it has mostly been studied using a limited number of UC cases or selected MPC cases and its cut-off proportion for MPC has not been proposed yet. To define clinicopathologic features of MPC further and to provide a cut-off proportion of MPC component for the prediction of poor prognosis, MPC was compared to conventional urothelial carcinoma (CUC) in a large series of UC cases. We analyzed 353 cases of CUC, 83 MPC being defined as UC showing any proportion of the micropapillary features, and 6 other UC variants. Compared to CUC, MPC was frequently accompanied by carcinoma in situ, lymphovascular invasion, and nodal and distant organ metastases and showed higher tumor grade and stage, lower recurrence rate, and higher disease-specific mortality rate with 68% and 56% of 5-year and 10-year disease-specific survival rates, respectively. In addition to the tumor grade and stage, the presence of MPC component and its large proportion were independent prognostic factors for distant organ metastasis and disease-specific survival, respectively, but with no single cut-off proportion of MPC. Our results demonstrated that MPC is a relatively common (18.8%) aggressive variant and suggest that not only the presence of MPC but also its proportion should be reported on the pathology report for the proper patient management.
