During follow up, PET/CT can be used to find recurrent or metastatic tumor, and to monitor response to treatment. But, up to now, there have been a few published reports about the role of PET/CT in breast cancer. The purpose of this study is to investigate the usefulness and the role of PET/CT in detecting of primary malignant lesion, finding recurrence or metastases, and in monitoring response to treatment in breast cancer patients. Materials and Method : From September 2004 to July 2007, 166 patients with breast cancer underwent PET/CT. 37 patients underwent PET/CT for palpable breast mass or positive finding on mammogram or ultrasonogram, 103 patients for detection of recurrence or metastasis after breast cancer treatment, and 26 patients for monitoring response to treatment. For detection of the breast cancer, sensitivity, positive predictive value of PET/CT, mean maxSUV, and mean lesion size were analyzed. For detection of recurrence and metastases, we compared the efficacy of PET/CT with that of whole body bone scan. And for monitoring of treatment, we elavulated the impact of PET/CT on the results from intermodality or regimen change in treatment. Results: The sensitivity and positive predictive value of PET/CT for detection of breast cancer were 94.6% and 97.2%, respectively. For detecting recurrence or metastases, the sensitivity and positive predictive value were 100% and 96%, respectively. PET/CT detected axillary lymph node metastasis in 1 patient with negative finding on whole body bone scan. For monitoring response to treatment, 10 patients showed aggravation of metastases, and 7 patients of the 10 patients showed improvement after inter- or intramodality treatment change. For two patients with suspicious finding of recurrence on other imaging modality, PET/CT assessed these lesions to be postoperative fibrosis, and patholgy confirmed. Conclusion: PET/CT showed high sensitivity and positive predictive value for detection of breast cancer, recurrence, and metastases. And, PET/CT was useful to find extraskeletal metastases. For monitoring response to therapy, PET/CT distinguished non-responders and changed treatment plan. Also, it was useful to differentiate postoperative fibrosis from recurrence.
