우리나라의 경우 누구나 인정되는 진료 지침이 없는 상태에서 양의사와 한의사의 견해 차이가 크기 때문에 ‘각자 알아서’ 양의사와 한의사를 국민들이 선택하고 있다. 최선의 치료는 한 가지라고 가정할 때 보건의료계는 국민들에게 최선의 건강 관리 및 치료지침을 국민에게 제시할 필요가 있다. 또한 의료의 중복과 낭비가 크다는 점이 지적된다. 같은 병을 가지고 서로 다른 진단 방법과 치료 방법(시술 및 투약)을 사용하기 때문에 의료의 낭비가 심하다. 결핵이나 암에 대한 치료를 의사에게 받으면서 몸을 보하기 위해 보약을 따로 먹는 경우는 물론이고, 한의학적인 진단, 즉 신장이 허하다든지, 간이 나쁘다는 진단을 확인하기 위해 양방병원을 찾아와서 불필요한 혈액 검사나 초음파 검사를 받는 사례가 흔하다. 그 동안 양의계에서 제기했던 의료 일원화에 대해 한의계에서는 일관되게 반대의 입장을 견지하고 있다. 근래에는 아예 일원화의 논의 자체에 대해서조차 관심을 보이지 않고 있다. 한의계가 의료 일원화를 반대하는 가장 심각한 이유는, 첫째로 의료 일원화가 되면 한의학이라는 학문이 고사(枯死)되고 그로 말미암아 전 세계 의학 발전에 막대한 손실이 초래된다고 믿고 있으며, 또 다른 이유는 양의사들도 한방 진료를 하게 됨으로서 야기되는 현실적인 불안감과 피해 의식 때문이라고 할 수 있다. 그러나 실제적으로 후자는 절실한 반대 명분이 되지는 못하는 것 같다. 어차피 의료 일원화가 됨으로써 기존 한의사에게 발생하는 현실적인 득과 실은 서로 상쇄할 만하기 때문이다. 대부분의 국민들은 양 한방의 협진을 동일 장소에서 동일 시간에 서비스 받고자 하기 때문에 그러한 범국민적인 요구를 ?i아가야 할 것이다. 그런 관점에서 보면 의사와 한의사 면허를 복수로 가진 사람은 복수의 면허가 허용하는 진료서비스 공급에 차별이나 제한을 두지 않는 것이 하나의 선결 방법이다. 현재 우리나라에는 의사와 한의사 양쪽의 면허를 가진 사람이 100여명이 되는 것으로 알려져 있다. 먼저 이들이 자신의 환자를 치료하기 위해서 양방 혹은 한방의료 내의 어떤 치료방식을 택하든 자유로이 자신의 서비스 방식과 형태를 선택하도록 하여야 한다. 즉, 양방과 한방이 한 건물내에서 서로 진찰할 수 있는 법적 정비가 되어야 하며, 의료법상 의사, 한의사로 구분된 것을 동의, 서의, 동서의의 세 그룹으로 구성된 삼원화의 의료제도가 마련되어야 한다. 이를 바탕으로 의과대학과 한의과 대학의 학제를 개편 통합시켜, 공부하는 사람들의 선택에 의해 동의사, 서의사, 동서의사를 배출시킬수 있어야 한다. 한방이든 양방이든 의료가 추구하는 바는 개인의 건강 증진과 질병치료를 통한 국민 보건의 향상이며, 이러한 원론적 공통점이 의료계의 일부에서 의료 일원화를 조속히 추진해야한다고 주장하는 근거의 하나가 되고 있다. 추구하는 바는 같은데, 그를 실현하기 위한 의료체계가 현실적으로 이원화되어 있다는 것 자체가 여러 가지 측면에서 비효율적인 요소를 가지게 한다고 해석하기 때문이다. 문제는 이원화된 한양방 체계의 개선, 곧 한․양방 결합의 주도권을 누가 쥘 것이냐 하는 것이다. 결론적으로 말하면 국민, 국민의 결집된 힘이 한․양방 결합을 이끌어나가야 한다. 이해 당사자인 의료인들이 합리적인 개선방안을 도출하기를 기대하는 것이 무척 어려운 일이라는 것은 한․약분쟁 과정에서 충분히 실감한 바 있거니와 무엇보다도 의료체계의 개선은 국민의 건강보장과 직결되는 국민 모두의 직접적인 이해관계가 걸린 사안이기 때문이다. 대의기구인 국회나 행정부에 대한 감시 및 통제를 통해 국민의 이익을 관철시키기 위한 노력을 국민모두가 기울여야 하겠지만 특히 자발적인 시민결사체의 역할에 주목할 필요가 있다. 다원화된 민주사회에 있어서 시민운동은 개별적으로 흩어져 있는 국민의 의지와 지혜를 모아 그 이익을 대변하는 훌륭한 통로가 될 수 있다. 더욱이 의료와 같이 국민 모두의 관심사이면서도 고도로 전문화돼 일반 국민들이 그 실체를 파악하기 어려운 사안에 대해서는 조직화된 시민단체의 역할이 지대하다. 한․약분쟁과정에서 극적으로 나타났듯이 정부의 중재 노력에도 아랑곳하지 않던 의료인 단체가 시민단체의 중재, 다시 말하면 결집된 국민의 의사에 승복했던 경험은 소중하다 아니할 수 없다. 다만 의료문제가 전문적인 사안인 만큼 지속적으로 연구하고 국민 다수의 이익에 부합하는 합리적인 정책을 개발하려는 노력을 게을리 하지 말아야 할 것으로 굳게 믿는다.

In the condition in which there is no guideline for medical directions publicly recognized, and there is big difference in the opinion between Western medical doctor and Oriental medical doctor, the people choose their doctors among Western doctor and Oriental doctor by their own standard. On the assumption there is only one best medical treatment, the medical and health societies need to present the best directions for the public health management and treatment to the public. In addition, there is much waste of resources due to the duplication of medical institutions. Much medical waste occurs because the different diagnostic and treatment methods (medical operation and medicine) are applied to the same disease. The cases where a patient take restorative medicine to strengthen his body while he is being treated for tuberculosis or cancer by doctors are needless to be mentioned here, and the cases where a patient visits the Western hospital and takes needless examinations like blood examination or ultrasonic examination to confirm the traditional Korea medicine's diagnosis, such as the weakened kidney or the liver in poor condition are common in Korea. For the unified medical system proposed by Western medical society, the Oriental medical society consistently stick to its position of objection, and recently have paid little attention to the discussion about the unified medical system itself. For the chief reason for the Oriental medical society's objection to the unified medical system, firstly, they believe that, if the unified medical system is enforced, the science of traditional Korean medicine will be withered, which will result in the great loss to the medical development of the world. For another reason, it should be pointed out that the traditional Oriental medical doctors feel practical uncertainty about economic loss, and sense of disadvantage if Western doctors are allowed to use traditional Oriental medicines for the medical treatment. However, it seems that the latter cannot be actually justified for the adamant and urgent objection. The reason is that, if the unified medical system is made, the loss and gain for the traditional Korean medical doctor can be offset. As most of people want the cooperative treatment service of Western medicine and Oriental medicine to be available at the same place and same time, the doctors should follow such demand of the people. From such view point, it can be another preceding solution to abolish the discrimination or limit against the medical service allowed to the doctors licensed with both Western and Oriental medicines. It is known that now, the number of the doctors licensed with both Western and Oriental medicine comes to about 100 in Korea, and first of all, in order for them to provide medical treatment to the patients, it should be legally established that they can choose the methods and forms of the treatments, whichever one they may choose between Western or Oriental medicines. In other words, it is legally established to provide the medical examination services of Western and Oriental medicines to the patients within the same hospital, and in order to realize this idea, the 2 way medical system of the Western and Oriental doctors under the current medical law should be restructured into the 3 way medical system of the Oriental, the Western and Oriental-Western doctor. Based on this plan, the medical education system in the college of Western and Oriental medicine should be restructured and integrated, so that the students in the medical college can choose their medical majors among the 3 areas, such as, Western, Oriental and Western-Oriental. Regardless of the type of medical systems, like Western or Oriental medicine, the goal of the medical treatment is the improvement of the public health through the health improvement and disease treatment of individuals, and such basic common grounds have become one of the bases on which part of the medical societies insist on the fast establishment of the unified medical system. The reasons for such insistence are based on the interpretation that, while the medical systems pursue the same goals, the currently established 2 way medical system in order to realize such goals is supposed to contain inefficient elements in the various areas. One of the problems with the improvement of the 2 way medical systems of Western and Oriental medicine is which side is going to take the lead in integrating the 2 medical systems. In conclusion, the public and the united power of the public should draw the combination of the 2 medical systems. Because, everybody has already experienced enough in the process of the conflict in the traditional Korean herbal medicine and Western medicine, how hard it is to expect that the persons in the medical business, the parties concerned in the conflict of business, can draw up the rational improvement plan for the integration of the 2 way medical system, and first of all, the improvement of the medical system is directly linked to the interest of all the people whose health securities are staked on it. Although all the people should pay attention to the efforts to realize the public interest through the supervision and control on the National Assembly and the Administration, and especially, we need to pay attention to the roles played by the voluntary civil organizations. In the diversified democratic society, civil organizations can be a good communication channel representing the interest of the public by getting together the people's intentions and wisdom, which are scattered around separately. Furthermore, the well-organized civil groups can play a great role of solving the matters difficult to understand for the public, because such matter as the medical treatment system is highly specialized, despite one of the main interests for the public. As it was dramatically shown in the process of the conflict in the traditional Korean herbal medicine and Western medicine, it should be highly valued that the medical organizations, which had paid no attention to the government mediation on the medical conflicts, agreed to the mediation by the civil organizations, the integrated opinion of the people. However, as the medical matters belong to the highly specialized area, they should be continuously researched, and I firmly believe that the efforts to develop the rational policies matching with the interests of the majority of people should not be neglected.
